Tres Machos, 2313 Westwood Avenue – The following violations were reported during a routine inspection: observed an employee drink with no lid at the drink station; fork and food debris found in hand sink by cook line (hand sinks are for handwashing only); raw chicken and bacon incorrectly stored above ready-to-eat foods; tortilla bowls stored on a low shelf are not covered; observed dried food debris on knives and utensils in clean storage and on ice scoop at bar; dish machine sanitizer level is at 0 ppm; observed an employee use dishes, wash but not sanitize, and then use again; observed mildew buildup in ice chute at soda dispenser and dried residue on tea dispensers and soda gun nozzle; temperatures of multiple foods (rice, refried beans, queso, black beans, carnitas) are too low; temperatures of multiple foods (raw steak, chicken, cheese, cooked carnitas, broth) are too high; tomatoes, cooked pasta and cooked meat do not have date marks; physical and online menus do not indicate that huevos rancheros can be served undercooked; multiple spray bottles are unlabeled; degreaser incorrectly stored near food; raw meat incorrectly thawing at room temperature on counter; observed small flies and mouse droppings throughout kitchen and storage; dead mouse found in a trap in back dry storage area; single-service foil pans are not being stored inverted; corn tortillas incorrectly stored in grocery bags; cardboard is being used to store items on prep table; observed standing water in lowboy unit; back side of small prep unit (with raw beef) is in disrepair; pitted stone bowls are being used to serve guacamole and other foods; observed dust, grease, dried food residue and/or mold and mildew on many surfaces (sides of grills, outside of trash cans and chip cans, on steam table, sauce and spice containers, shelves, shelf liners, inside refrigerators); mop sink doesn’t have a proper backflow prevention device installed; dumpster lid is open; floor tiles throughout kitchen and walk-in cooler are cracked; lights are burnt out or missing in hood system, kitchen by walk-in cooler, dish room and in storage areas.

Hyderabad Biryani House, 4040-H Cox Road – No violations reported during a follow-up inspection.

RVA Pizza, 9742 Gayton Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; back hand sink doesn’t have a handwashing sign; sanitizer level at three-compartment sink is too high; white powders on cook line and shelf under prep table are unlabeled; flour incorrectly stored on the floor in front kitchen and food in freezer incorrectly stored on the floor; pans incorrectly stored on Styrofoam in front of kitchen and on cardboard near the oven; microwave is dirty with food splatter; observed heavy carbon buildup on pan on stovetop; several lights are out in the kitchen; hand soap dispenser in front and outside of equipment are dirty; foil in oven is full of food debris; grease and food debris are covering the surroundings of fryer; floors under equipment are dirty with food debris; walls behind pizza prep line and back door wall are both heavily soiled.

Chicken Salad Chick, 9631 West Broad Street – The following violation was reported during a routine inspection: observed trash and food debris on the floor behind tables and equipment.

Hilton Richmond Hotel & Spa, 12042 West Broad Street – The following violations were reported during a routine inspection: observed buildup on can opener blade; high temp dish machine did not reach the required 160 degrees after multiple runs; three pans/boxes of food incorrectly stored on walk-in freezer floor; beer cooler racks and chemical storage rack are rusty; wall at the dish machine has mildew; floor drain at the bar is dirty.

Vietnamese Deli Inc., 6215 West Broad Street – The following violations were reported during a routine inspection: observed debris/mold on rolling racks, trays, bread trays and refrigerator; several pots on shelf under the prep table have encrusted grease on them; observed accumulated flour on the floor behind mixer; hole in the wall between two storage racks.

Capriccio Pizza, 9127-A West Broad Street – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed employees washing hands in the three-compartment sink (use hand sink); there isn’t a working hand sink in the cook and pizza area; lasagna in the walk-in doesn’t have a date mark; fryer is greasy; an indirect drain is not installed on the sink for cleaning lettuce; observed dirt and food splatter on walls, floor in the sauce area and floor in walk-in cooler; filters on the hood above fryers are greasy.

Subway Sandwiches & Salads, 3135 Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed dried food debris inside microwave; sanitizer level in three-compartment sink and knife sanitizer level are at 0 ppm; teriyaki chicken prepped four hours earlier is not at the proper temperature; shelves in the walk-in cooler are in disrepair; mop sink is broken; observed dried food debris on sides of prep line units; observed grime in mop sink and on walls near sinks; fan grates in walk-ins are dusty; several ceiling tiles are damaged; wet mop is not being hung to dry.

Tay-Ho Restaurant, 7927 West Broad Street – The following violations were reported during a routine inspection: observed employees handling food (spring rolls, limes, basil) with bare hands; observed old food particles on slicer and food splatter on plates that were stacked to use; an employee washed pans but did not sanitize them; bar soda gun is leaking; observed food splatter on wall near the rice cooker.

Shoney’s Restaurant, 8415 West Broad Street – The following violations were reported during a routine inspection: menu does not have a consumer advisory about undercooked eggs; cutting board is badly stained and scratched; walk-in cooler rack is rusty; rack at the end of the three-compartment sink is rusty and dirty; observed dirty brown carts in the prep area; vent on prep line is dusty; food debris found on floor in dry storage area; observed low grout in several places in the kitchen; tiles are missing in the walk-in cooler; walk-in cooler door has a hole; vent is missing over three-compartment sink.
