Shelly’s Food Restaurant LLC, 7304 Staples Mill Road – The following violations were reported during a routine inspection: hand sink doesn’t have paper towels; back hand sink doesn’t have soap; dried food debris found on can opener blade; incorrect temperature of pork in prep unit; meat and sauce in the walk-in cooler do not have date marks; time is not being marked for cheeses and beans used to make papusas; food is not cooling properly because it is covered and in deep pans; prep cutting board is heavily stained and scratched; observed dust and food debris on utensil bins, surfaces of equipment, inside reach-in coolers, on fan grate and on wire racks in the walk-in cooler; observed dirt and food debris on walls and floors; ceiling is not smooth and easily cleanable.

Live Crawfish & Seafood, 9020 West Broad Street – The following violations were reported during a routine inspection: hand sink doesn’t have hand soap or paper towels; person in charge was unable to locate the tags for the box of oysters currently being used; shellstock tags are maintained but not kept in chronological order; the date when the last oyster is sold is not recorded on the tags; raw eggs incorrectly stored over raw veggies and raw lobster incorrectly stored over potatoes in the reach-in cooler; temperature of unsalted butter at hot holding is too low; potatoes, pasta and corn cooked yesterday are not date marked; Zyrtec, Robitussin and hydroperoxide are incorrectly stored near food; concentration of bleach sanitizer at three-compartment sink is too high; two containers of salt are unlabeled; single-service paper plate was stored facing up and uncovered.

The Answer Brewpub Co., 6008 West Broad Street – The following violations were reported during a routine inspection: sanitizer level at bar dish machine is at 0 ppm; incorrect temperature of fries that were cooked four hours prior; temperature of chicken that has been cooling for one hour is elevated because it is covered; cooked chips are being stored in a non-food safe container; observed dried food debris on top of dish machines and on gaskets in refrigeration units; observed grime buildup on floors, especially under dish machines; a light is out in the hood system; observed grease buildup in hood system.

Wendy’s, 4231 Pouncey Tract Road – The following violations were reported during a routine inspection: line items on time control didn’t have time stickers on the pan (stickers were placed on the plastic wrap used to cover the pans so they were lost when these covers were removed); observed grease buildup on fryer casters and mildew on the outside of the dining room’s Coke machine chute.

Picnic RVA, 7311 Capehart Road – The following violation was reported during a routine inspection: observed rust on bottom shelf.

Natalie’s Taste of Lebanon, 3601-A Cox Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap or paper towels; hand sink in back kitchen is obstructed by plastic wrap; observed dust/food buildup on oven; potato/fries cutter on the wall is dirty with food debris; inner lip of ice machine has buildup; metal scrapers incorrectly stored between wall and sign; several plastic bins are stacked while still wet; chest freezer has a torn gasket; sides of fryer are greasy; back part of soda machine is dirty; leak found under hand sink near cook line; walls throughout the facility are dirty; ceiling tile is missing in dry storage/prep room; caulking along the dish area is moldy; floor on cook line is pitted.

Taziki’s Mediterranean Café, 4024-C Cox Road – The following violations were reported during a routine inspection: certified food protection manager certificates were not available during inspection; can opener blade has residue on it; cutting boards are stained; walk-in fan guards are dusty; observed debris on two racks in walk-in; wall panel at the mop sink is loose from the wall.

Breaker’s Sports Grille, 9127-V West Broad Street – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; facility does not have written procedures for how to respond to vomiting/diarrheal events; cook returned from a smoke break and did not wash hands upon entering the kitchen; hand sinks in kitchen, smoking bar and non-smoking bar do not have paper towels; hand sink in kitchen and non-smoking bar do not have hand soap; both hand sinks behind the bars are blocked; observed several cans dented at the seams in dry storage; raw chicken, raw oysters and raw dark meat incorrectly stored over ready-to-eat food in walk-in cooler; chipotle ranch/mayo sauce made in house is past its expiration date; personal backpack incorrectly stored on warmer on the cook line; raw shrimp, gyro meat and meatballs are incorrectly thawing at room temperature in the three-compartment sink; pretzel salt on cook line, bulk sugar in dry storage and batter on cook line are unlabeled; at both bars, jugs of cranberry juice are being stored uncovered in cooler; bottle of mixer is touching ice in the ice bin; all cutting boards are heavily stained and scratched; microwave is dirty; paper towel and soap dispensers in the kitchen are broken; lights are out under the hood; observed heavy carbon buildup on several pots; facility does not have quat test strips for tablets at bar three-compartment sinks; fryer baskets are very greasy and sides of fryer have grease buildup; grease trap bucket is heavily soiled; inside of reach-in freezer is dirty with food debris; observed low grouting throughout the facility; floor tiles are chipped at the smoking bar; ceiling tiles are missing throughout the kitchen; some ceiling tiles in the kitchen are not kitchen-grade (some are made of an absorbent material); floors, walls and ceilings are dusty and greasy.

Kokonut Box, 10921-F West Broad Street – The following violation was reported during a routine inspection: canned/bottled drinks are incorrectly stored on the walk-in floor.

Lewis Ginter Botanical Garden Tea House, 1800 Lakeside Avenue – The following violation was reported during a routine inspection: did not observe a hand wash sign at service station or restroom.
