Pizza Republic, 5607 Lakeside Avenue – The following violations were reported during a routine inspection: an employee stated that there isn’t a “person in charge” when the manager is not present; a certified food protection manager was not on site; back hand sink and employee restroom are missing paper towels; sanitizer bucket is in the hand sink (hand sinks are for handwashing only); several cans of pineapple are severely dented; microwave is dirty with food debris; sanitizer level in sani bucket is at zero; temperatures of marinara, cheese, garlic in oil, shredded cheddar cheese and grilled chicken on the pizza prep line are elevated; containers of parmesan, salt, flour and dough water are not labeled; a dusty computer/keyboard is being stored above the pizza prep line; pizza prep unit has torn gaskets and the unit is not keeping food at the proper temperature; observed heavy ice buildup in reach-in freezer; side of pizza prep unit, outside of microwave, sides of equipment, mop sink and employee restroom toilet are dirty; wall behind pizza prep unit is splattered with sauce; fan guard in walk-in is very dusty.

SB’s Lakeside Loveshack, 6935-37 Lakeside Avenue – The following violations were reported during a routine inspection: hand sink is obstructed by a trash bin; hand sink is missing paper towels and soap; scoops and utensils are being stored in standing water with zero sanitizer solution; temperatures of gravy and chili made yesterday in the walk-in are elevated; temperatures of tomatoes and shredded cheese on the cook line and quiche and unsalted butter in the small reach-in cooler are elevated; ibuprofen is incorrectly stored on shelves near hand sink and over hot-holding area; gravy, dense soup and chili cannot cool properly because they were placed in deep bins to cool; salt on the cook line and a powder next to the hot cocoa station are unlabeled; observed several flies in dry storage; wet wiping cloths are sitting out instead of being stored in a sanitizer bucket with the proper sanitizer concentration; several ceiling tiles in the kitchen are out of place leaving openings in the ceiling.

Franco’s Italian Restaurant, 9010 Staples Mill Road – The following violations were reported during a routine inspection: back hand sink is missing the hot water handle; temperature of chicken wings cooked yesterday in the walk-in cooler is elevated because they were not cooled properly; prep unit cutting board is heavily scratched and stained; soiled cardboard is being used to line storage racks; wire racks in the walk-in cooler are rusty; observed dust buildup on wire racks and food debris inside reach-in freezer; walls under three-compartment sink and towards the front are damaged.

Subway, 9105 Quioccasin Road – The following violations were reported during a routine inspection: temperatures of two containers of tuna made this morning at 8 a.m. are elevated because they were not cooled properly; a clean tomato slicer is hanging beside the hand sink (hand sinks are for handwashing only); single-service items are incorrectly stored under and by the sewer line; observed bare/unsealed wood under the fax machine; cardboard is being used to line some shelves in the storeroom; cabinet under the soda machine is dirty.

Firebirds Wood Fired Grill, 11800 West Broad Street – The following violations were reported during a routine inspection: cook line hand sink doesn’t have soap; bar hand sink doesn’t have paper towels; observed heavy buildup on can opener blade; crevices of ice machine are dirty; temperatures of bean salad on cook line and Cajun butter in walk-in are elevated; lexan pans are wet-nested on the dish rack; cutting boards at cook line and in the prep area are scratched and stained; racks in the beer cooler and bottom of the dish rack are rusty; three lights are burnt out under the back kitchen’s hood system; facility doesn’t have an irreversible thermometer for the high temp dish machine; observed grease buildup behind the cook line and behind refrigerators on the cook line; floor drain at the bar is dirty; floor tiles near floor grates are cracked.

Whole Foods Market, 11173 West Broad Street – The following violations were reported during a routine inspection: dish machine doesn’t appear to be effectively sanitizing; temperatures of several foods at the hot bar are too low (chicken legs, samosa, fried chicken); temperature of shredded cheese on ice at customer self-serve bar is elevated because the ice level is low; hold times are not being tracked for pizza slices today; mop sink has an improper backflow prevention device; caulking/sealing at prep sink is moldy and peeling; a light is out in the kitchen; ceiling in dish room is dusty; floor drain and walls are soiled.

El Lopal II Inc., 5205 South Laburnum Avenue – No violations reported during a follow-up inspection.

Sticks Kebob Shop, 1700 Willow Lawn Drive – The following violations were reported during a routine inspection: cardboard is lining shelves; surface on PVC pipe in rear has been compromised; wall between water heater and mop sink has deteriorated.

Chipotle Mexican Grill, 8675 Quioccasin Road – The following violations were reported during a routine inspection: salt on the cook line is unlabeled; observed several flies throughout the kitchen.

Everyday Gourmet, 6010-B North Crestwood Avenue – The following violations were reported during a routine inspection: observed dried food debris inside blender; walk-in fan grates are dusty and grimy; wall and ceiling by the ovens is stained; ceiling tiles above prep table by the hood system are damaged.

Frida’s Mexican Restaurant, 2903 Williamsburg Road – The following violation was reported during a follow-up inspection: metal top of grease trap is rusted.

JS Lakeside Tavern, 5406 Lakeside Avenue – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; there are no written procedures for cleaning up vomiting/diarrheal events; whole tomatoes incorrectly stored on carton with raw eggs; freezer and low boy unit are not functioning; no chlorine test strips were available; observed grease and grime buildup on outside of cooking equipment and refrigerators, on walk-in fan grates and walk-in shelves; observed grease buildup and food debris on floors around cooking equipment and walk-in floors; buildup of dust found under ice machine at the end of the bar; observed heavy, dripping grease residue in hood system.

Subway Sandwiches & Salads, 4386 South Laburnum Avenue – No violations reported during a follow-up inspection.

Rock Bottom Restaurant & Brewery, 11800 West Broad Street – The following violations were reported during a follow-up inspection: equipment surfaces on the cook line need cleaning; observed old food and trash on floor under cook line.

Daddio’s Grille, 12385 Gayton Road – The following violations were reported during a routine inspection: observed an employee cut onions with bare hands (to be served raw/uncooked); ash trays found in hand sink at the bar (hand sinks are for handwashing only); observed food residue on slicer and dicer; observed black mold/mildew inside ice machine; temperatures of chili and soup reheating in warmers are not hot enough; used breading mixture for chicken had dough balls/drippings from use in previous days and it was out at room temperature under the prep table; when employee was advised to make new breading, the same soiled container was used; chemical spray and squeeze bottles are unlabeled; Goo Gone incorrectly stored above pizza boxes and cans of food; small flies seen at the bar; ice scoop handle is in contact with ice at the bar; toothpicks and wrappers stored in metal containers are inside a soiled cardboard box; foil is incorrectly lining a shelf at the grill; red cutting boards at the prep unit are heavily scratched; observed food debris and/or grease on walk-in cooler racks, sides of fryer, inside freezer units and on shelving of prep unit; walk-in fan grates are dusty; slime found inside chest refrigerator at bar; observed leak at dump sink faucet at bar in smoking section; soda gun holder drainage hose empties onto the floor; trash cans in women’s restroom don’t have lids; a light is out in the hood; coving is missing behind three-compartment sink; floor tiles are damaged or missing in a few areas; observed grease on floor at cook line and food debris on floor of walk-in; walls under sinks in kitchen and bar are soiled; hood filters are greasy.
