Einstein Brothers Bagels, 1368 Gaskins Road – The following violations were reported during a follow-up inspection: observed small flies at the mop sink; small under-counter fridge up front is not working; cutting boards are dirty and some have deep scratches; the space under the shelf near the egg cooker is dirty; floor tiles at the entry to the service line are missing; back kitchen walls are stained; mop sink drain is dirty; two light shields are missing in back kitchen.

Cook Out Short Pump, 4245 Pouncey Tract Road – The following violations were reported during a routine inspection: prep area hand sink has food particles in it (hand sinks are for handwashing only); Styrofoam containers next to the cook line are not stored covered or inverted; soap has leaked down the wall near the back hand sink; hood system has grease buildup and there is a gap greater than one inch between a set of filters.

Harvie Elementary School, 3401 Harvie Road – No violations reported during a routine inspection.

El Rey International Supermarket, Inc., 6006 West Broad Street – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed employees washing hands at three-compartment sink instead of the hand sink; observed dried food debris buildup inside microwave; temperatures of chicken soup and whole chicken are too low because they have been sitting out; temperature of individual portions of rice is elevated because they were combined in a larger bag before cooling properly; observed small flies throughout the kitchen; gasket on two-door reach-in is torn; lights are burned out; observed dried food debris inside reach-in freezer and on refrigerator handles; ceiling vents are dusty; observed food debris in storage area.

Godwin High School, 2101 Pump Road – The following violations were reported during a routine inspection: walk-in freezer has cracked floor tiles; coving tiles are missing near walk-in units and under two-compartment sink.

Gayton Elementary School, 12481 Church Road – No violations reported during a routine inspection.

Starbucks Coffee Company, 8137 Brook Road – The following violations were reported during a routine inspection: a certified food protection manager is not present during inspection; floors in dry storage area and floor drain under three-compartment sink are dirty.

McDonald’s, 5106 Nine Mile Road – The following violations were reported during a routine inspection: observed grime buildup in self-serve ice machines; temperature of sausage gravy in walk-in is elevated and person in charge could not verify how long it had been cooling; sliced tomatoes and shredded lettuce on the line are on a time control policy but no times are marked; cutting boards are deeply scratched and stained; observed food debris on shelves in refrigeration units; observed cracked floor tiles at fryers and walk-in freezer.

Glen Allen High School, 10700 Staples Mill Road – The following violation was reported during a routine inspection: observed dried milk accumulation on wire racks in the walk-in cooler.

Henry Ward Elementary School, 3400 Darbytown Road – The following violations were reported during a routine inspection: observed food debris in microwave on the line; employee restroom door is open (should be kept closed except when cleaning).

Mehfoud Elementary School, 8320 Buffin Road – The following violations were reported during a routine inspection: sanitizer solution in sani bucket is less than 150 ppm; Styrofoam trays are not stored inverted to prevent contamination; employee restroom door is open (should be kept closed except when cleaning); mop is sitting in mop water instead of being hung to dry.

Elko Middle School, 5901 Elko Road – No violations reported during a routine inspection.

Panda Express, 1782 North Parham Road – No violations reported during a routine inspection.

Bright Spot Coffee, 6114 Lakeside Avenue – The following violations were reported during a routine inspection: temperatures of egg patty and smoked salmon in dorm style refrigerator are elevated; this unit is unable to keep foods at the proper temperature; cooked eggs, sausage and bacon do not have date marks; observed foods being taken from original containers without a label.

MOD Pizza, 8671 Quioccasin Road – The following violations were reported during a routine inspection: observed food debris on dicer blades and press; dicer blades and press are in disrepair; temperature of ham in container on the line is elevated because it was overstacked; cutting boards are deeply scratched and stained.
