Rock Bottom Restaurant & Brewery, 11800 West Broad Street – The following violations were reported during a routine inspection: did not observe a ServSafe certificate during inspection; both hand sinks on line had objects or food debris in them (hand sinks are for handwashing only); neither paper towel dispenser on the cook line worked; bar hand sink is missing paper towels; temperatures of all foods on the rail line are elevated (babaganoush, deviled egg filling, shrimp, corn salsa, lettuce, pickled onions, sauteed mushrooms, etc.); breading flours are on a time control policy but times are not marked; spray bottles of cleaners at the bar are unlabeled; single-service items at the bar are not stored inverted; multiple lexan pans are cracked or chipped; cutting boards on the storage rack are worn and stained; equipment surfaces along both cook lines need cleaning, as well as the rack near the walk-in and the meat cage racks; observed old debris on floor under cook line; walls throughout the kitchen are dirty; two ceiling tiles over the dishwasher are cracked and beginning to peel; both hood system filters are dirty; hood filter over the kettles is extremely dusty.

Longan Elementary School, 9200 Mapleview Avenue – The following violation was reported during a routine inspection: hand sink in kitchen doesn’t have a handwashing sign.

Crafted, 4900 Libbie Mill East Boulevard – No violations reported during a follow-up inspection.

Glen Lea Elementary School, 3909 Austin Avenue – No violations reported during a routine inspection.

Montrose Elementary School, 4901 Brittles Lane – No violations reported during a routine inspection.

Frida’s Mexican Restaurant, 2903 Williamsburg Road – The following violations were reported during a follow-up inspection: observed an employee touch raw steak and chicken with gloves on and then grab veggies out of the refrigerator without changing gloves or washing hands; a knife contaminated with raw chicken found in the hand sink (hand sinks are for handwashing only); several foods did not have date marks (chicken, beans, horchata, salsa, etc.); food is not being properly covered in the walk-in cooler; outdoor dumpster had debris surrounding the area and the enclosure is not being kept closed; metal top of grease trap under three-compartment sink is heavily rusted.

Capital Ale House, 4024-A Cox Road – The following violations were reported during a routine inspection: an open cup of water belonging to an employee is at the prep unit; can opener blade has thick, sticky buildup on it; lids on the clean dish rack have food debris and residues on them; temperatures of spinach and lettuce in the salad unit are elevated because they are stored above the air curtain; temperatures of garlic and herb butter, butter and roasted garlic mayo are elevated; all foods in the make table across from the grill have elevated temperatures; small flies seen throughout the kitchen and bars; cutting boards at the make tables are scratched and stained; observed some cracked and chipped plastic containers/lids on the clean dish rack; racks in dry storage are rusty; facility’s irreversible thermometer for the high temp dish machine has a dead battery; observed grease buildup on pipes behind the cook line; observed debris in refrigerator gaskets and dust on the beer cooler’s fan guards (at main bar); mop sink has a hose under pressure without a proper backflow prevention device; multiple floor tiles at the main bar are cracked; dark, thick puddle found in the back corner of the meat walk-in; floor drains at the “up” bar and dish area are grimy.

Glory Days Grill of Gleneagles, 10466 Ridgefield Parkway – The following violations were reported during a routine inspection: water pitcher found in the hand sink (hand sinks are for handwashing only); facility could not provide parasite destruction letters from salmon or mahi mahi distributors; observed a black residue in the ice machine; temperature of chicken filets cooked yesterday is elevated because they were not cooled properly; temperatures of raw burger, raw chicken and salsa are elevated; several units are not maintaining foods at the proper temperature; quat sanitizer at three-compartment sink is over 400 ppm; sanitizer solution in sanibuckets is too strong; facility does not have a thermometer for the refrigerated grill drawer; observed small flies in the mop bucket storage area and at the bar; metal strainer is damaged and flat spatulas are cracked and chipped; foil is lining shelves and equipment at the cook line and dish area; dish machine at bar doesn’t have a working visual or auditory alarm; oven on cook line is greasy; racks in the walk-in are moldy and dirty; observed food debris in refrigerator/freezer gaskets; a spray hose under continuous pressure at the mop sink doesn’t have a proper backflow prevention device; light is out in the hood; observed low grout in a couple areas; wet mop is not being hung to dry; cook line ceiling is dusty; walls at dish and mop sink areas are moldy.

Fairfield Middle School, 5121 Nine Mile Road – The following violations were reported during a routine inspection: sanitizer dispensers in cafeterias G and Q are not working; fan grates in the veggie walk-in are broken; fan grates in the milk walk-in are rusty; gasket on ice machine in cafeteria G is in disrepair.

Varina Elementary School, 2551 New Market Road – The following violations were reported during a routine inspection: several cans in dry storage are dented on the seam; salt on shelf and white powder laundry detergent near washer/dryer are unlabeled; employee restroom door is propped open (should be closed unless being cleaned).

OCT. 7

Henrico High School, 302 Azalea Avenue – The following violation was reported during a routine inspection: sanitizer and soap dispensers at three-compartment sink in main cafeteria are not working.

Seven Pines Elementary School, 301 Beulah Road – No violations reported during a routine inspection.
