SEPT. 23

KickBack Jacks, 9318 West Broad Street – The following violations were reported during a routine inspection: a server quickly rinsed hands without soap after handling dirty dishes before preparing a drink; hand sink in kitchen doesn’t have hot water; temperature of hot water in women’s restroom isn’t hot enough; observed a whisk, lemons and straws in the hand sink at the bar (hand sinks are for handwashing only); raw burgers incorrectly stored next to cheese and cooked mushrooms in grill drawer (all are uncovered); observed a heavy buildup of black and pink mold/mildew inside ice machine; soda gun nozzles are soiled; observed food debris and metal shavings on can opener blade; temperature of two large containers of blanched fries prepared last night is elevated because they were not cooled properly; temperatures of cooked chicken, sliced turkey, cheese, unsalted butter, lemon butter, lettuce and salsa are elevated; several large flies observed in fry prep area/dry storage area; soiled wet wiping cloths are sitting out on prep surface at make table; food storage containers are being stacked wet; gaskets at bar refrigerator are torn and moldy; hood filters above fryer are missing; cutting boards are heavily scratched and stained; facility doesn’t have an irreversible temperature measuring device for high temp dish machine; spray hose with nozzle connected to faucet below three-compartment sink doesn’t have a backflow prevention device; observed low grout throughout the kitchen, especially at the cook line; coving near walk-in door is damaged; walk-in door does not shut tightly; light is out in the fry prep area; caulking is missing from three-compartment sink and where the hand sink is sealed to the wall.

Bubba’s Bar & Grill, 4000 Williamsburg Road – The following violations were reported during a routine inspection: no certified food protection manager present; hand sink in bar area doesn’t have handwash signage; found moldy tomatoes on the cook line and moldy white onions in the walk-in; pork rinks in walk-in are past their expiration date; handle of ice scoop at the bar is touching the ice; white tubs labeled “pickles” in the walk-in cooler are covered in mold; observed heavy buildup of ice in mini freezer on the cook line; observed leak from ceiling in walk-in; outside of blender in the bar area is moldy/dirty; ceilings are not made of an easily cleanable material and they are chipping and peeling; several ceiling tiles are missing throughout the kitchen; floor tiles under three-compartment sink are chipped.

Chick-fil-A at Short Pump Town Center, 11800 West Broad Street – The following violations were reported during a routine inspection: observed residues along gaskets in the thawing reach-in; floor drain at the ice machine is dirty; walls at breading station and dish wash area have splatter stains; observed mildew on three-compartment sink.

Chamberlayne Elementary School, 8200 St. Charles Street – The following violation was reported during a routine inspection: observed peeling paint on hood of dish machine.

Brookland Middle School, 9200 Lydell Drive – The following violation was reported during a routine inspection: temperature of milk in kitchen “K” milk unit is elevated.

Pizzaro, 9966 Brook Road – The following violations were reported during a routine inspection: cooked chicken, sauce, noodles and other prepared foods do not have date marks on them; chemicals incorrectly stored with food; many items and areas need cleaning including prep unit shelves, shelves above prep unit, shelves in walk-in cooler, pizza unit, floor under microwave, and pizza wall; several lights in the kitchen are not working; protective corner piece going to the walk-in cooler is loose.

SEPT. 28

Short Pump Elementary School, 3425 Pump Road – The following violations were reported during a routine inspection: chicken salad in the walk-in is past its expiration date; red cutting board under the prep table is worn; a patch of ceiling in the dish room is cracked and peeling.

Greenwood Elementary School, 10960 Greenwood Road – The following violations were reported during a routine inspection: observed white mold on shelves in the walk-in cooler; ceiling tiles around heat vents are dusty.

Glen Allen Elementary School, 1101 Mill Road – No violations reported during a routine inspection.

Nuckols Farm Elementary School, 12351 Graham Meadows Drive – No violations reported during a routine inspection.

Crossings Golf Club, 800 Virginia Center Parkway – The following violations were reported during a routine inspection: observed debris in soda gun and mold on deflector in ice machine; hot dogs and pre-cooked chicken in the prep unit are past their expiration dates; observed food debris on the table under the flat top grill.

Buttermilk and Honey, 12246 West Broad Street – The following violations were reported during a routine inspection: single-service containers on the line are not stored inverted or covered; waste basket in the unisex bathroom closest to the kitchen door does not have a lid.
