Subway Sandwiches & Salads, 4386 S. Laburnum Avenue – The following violations were reported during a routine inspection: employee did not wash hands for 20 seconds before putting on gloves; men’s restroom does not have soap or paper towels at hand sink; toilet and hand sink in men’s restroom are dirty; hand sink in front was used to wash off scoop for guacamole (hand sink is for handwashing only); scoop for guacamole was rinsed off but not sanitized; sanitizer levels at three-compartment sink and front sanitizer buckets were below 150 ppm; temperatures of chicken and Swiss cheese in the two-door low boy are elevated (unit is not keeping foods cold enough); temperatures of lettuce and tomato on the line are elevated; containers of salt and parmesan are unlabeled; small black sauce containers are incorrectly stored under the hand sink; to-go containers are not stored inverted; observed cardboard and trash on ground around dumpster; four ceiling tiles are missing; ceiling vents above bread baking center are dusty; floors throughout are dirty, especially under three-compartment sink.

SEPT. 19

Eastern Carry Out, 9024 W. Broad Street – The following violations were reported during a routine inspection: employee washed hands in the three-compartment sink (use hand sink); observed cardboard lining shelves; several lights in the kitchen are not working properly.

Burger King, 3720 Mechanicsville Turnpike – No violations reported during a follow-up inspection.

Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a routine inspection: observed food splatter in the microwave; many small flies found in mop room; heavy ice build-up in walk-in refrigerator; observed build-up of food debris and grease in conveyor oven; observed food debris inside units and grease/debris on clean dishes rack, shelving at cook station and under the oven; a significant leak is coming from water pipes and filter in mop sink room (bucket underneath is full of water and there’s water on the floor); ceiling and equipment at biscuit station are dusty; ceiling tiles and wall near cooking equipment are discolored; dead bugs found in storage area; caulking at three-compartment sink is moldy and caulking is missing at hand sink (hand sink is not sealed to the wall); light is burned out in mop room; grout is worn in some areas; mop incorrectly stored in water bucket (hang to dry).

Nick’s Roman Terrace, 8051-A W. Broad Street – The following violations were reported during a routine inspection: observed a torn gasket on prep units; shelves in the prep unit are rusty; inside of ice machine is rusty.

AUG. 20

Original Gino’s Pizza, 10444 Ridgefield Parkway – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate; could not locate a written time control policy; lasagna in pan incorrectly stored on floor in walk-in; bread incorrectly stored in crates on the floor; cardboard is being used to line shelves; wood is propping up shelving in the walk-in; racks for clean dish storage are rusty; observed food debris in refrigerator/freezer units; drip pan in hood system is greasy; faucet at three-compartment sink is leaking; faucet handle at hand sink is missing; employee restroom door is not self-closing; mop incorrectly stored in mop bucket (hang to dry).

Mezeh Mediterranean Grill, 11710 W. Broad Street – The following violations were reported during a routine inspection: observed a pitcher in the hand sink (hand sink is for handwashing only); sanitizer level at three-compartment sink is at zero; temperature of all cold foods on the line are elevated (arugula/salad mix, couscous, cuke salad, sundried tomato mix, cabbage, etc.); numerous pans of food in the reach-in are improperly cooling (they are stacked and wrapped with plastic); sanitizer level in sanibuckets is at zero; single-service items are not stored inverted; observed a patch of broken coving in back kitchen; observed an abundance of food debris on cook line floor.

Noodles & Company, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: observed debris on zoodle maker stored clean; cutting boards throughout the kitchen are scratched and/or stained; facility does not have an irreversible thermometer or another means of testing internal dish plate surface on the high temp dish machine; floor under the fryer has build-up of grease at wall junction; wall below the back hand sink is dirty; hood filters over cook line and at pasta cooker are dirty.

Frida’s Mexican Restaurant, 2903 Williamsburg Road – The following violations were reported during a routine inspection: observed a layer of grime on inside of ice machine; temperatures of rice, chicken, salsa and beans in the walk-in are elevated (they were made yesterday); foods made yesterday (rice, chicken, beans, horchata, etc.) do not have date marks; to-go trays are not stored inverted; salt and chicken soup mix incorrectly stored on the floor on the cook line; observed cardboard atop the grease trap under three-compartment sink; two steak knives are rusty; freezer next to the prep unit has torn gaskets and heavy ice build-up; walk-in cooler is not keeping foods cold enough; observed debris surrounding the outdoor dumpster area and the enclosure was not kept closed; metal top of grease trap under three-compartment sink is heavily rusted; wall behind three-compartment sink is dirty.

Rico’s Mexican Grill, 2316 John Rolfe Parkway – The following violations were reported during a routine inspection: person in charge was not able to identify the “big six” foodborne illnesses and their associated symptoms (besides fever); facility does not have a written employee health policy or way to verify employees have received training; observed an employee take out trash and return to the cook line to prepare food without changing gloves and washing hands; pink slime found in ice machine; sanitizer is not at the proper level at dish machine; fries and chicken nuggets were left out by the fryer; no date marks found on prepared TCS foods (chicken, guacamole, beef); medication and first aid items incorrectly stored above condiments; chemical spray bottles are unlabeled; deep containers of beans improperly cooling; food is incorrectly stored on the floor in dry storage; observed an employee wipe counter at cook line with a soiled cloth hanging from their apron (wet wiping cloths should be stored in sanitizer bucket); wood cutting board along the steam table is not sealed; containers of chips stored on cardboard table; cutting board at refrigerator is heavily scratched; racks in the walk-in are soiled with food debris and grime; light in the walk-in has been removed, leaving one very dim light by the door.
