JULY 27

Burma, 11216 Patterson Avenue – The following violations were reported during a routine inspection: person in charge was not able to identify the “big six” foodborne illnesses and their associated symptoms; temperatures in sushi refrigerator (eel, crabstick, tuna, crab salad, shrimp) are elevated; tuna incorrectly thawed in reduced-oxygen packaging.

Carini Italian Restaurant, 3718 Williamsburg Road – The following violation was reported during a follow-up inspection: temperatures of sliced ham, sliced cheese, ricotta, lasagna and chicken in the prep unit are elevated.

Hardee’s, 3815 Mechanicsville Turnpike – No violations reported during a follow-up inspection.

America’s Best Wings, 4730 Finlay Street – The following violations were reported during a follow-up inspection: slicer is dirty under the blade; chest freezer has a torn gasket.

JULY 28

Rise Biscuits & Donuts, 11561 W. Broad Street – The following violations were reported during a routine inspection: observed an employee’s drink uncovered near the mixer; line hand sink has a sanibucket in it and dish hand sink has cleaning tools (hand sinks are for handwashing only); cutting board at cookline is stained and scratched; several areas found dirty including sides of cookline equipment, walk-in fan guard, bottom walk-in rack, and cookline floor and walls, especially under the hand sink; back hood filters are dusty.

The Bombay Company, 4821 Old Main Street – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate; microwave is dirty; baked potato and lamb/sauce do not have date marks; powder red and orange food coloring is not approved by the FDA; tilapia incorrectly thawing in reduced-oxygen packaging; did not observe a thermometer in the kitchen; many bulk spices are unlabeled; ice bucket is facing up (invert to allow to drain); mop is sitting on mop room floor (flip over to air dry).

Chick-fil-A at Willow Lawn, 4920 W. Broad Street – The following violations were reported during a routine inspection: tongs and biscuit cutter have dried food debris on them; observed grease build-up and food debris on outside surfaces of cooking equipment.

Starbucks Coffee, 11735 W. Broad Street – The following violations were reported during a routine inspection: observed dishes air drying above the three-compartment sink that were not inverted; gasket in the under counter fridge near the back kitchen entrance is torn; facility does not have thermo labels or an irreversible thermometer for measuring the internal temperature of the high temp dish machine; floors under refrigerators in the back are soiled.

JULY 29

Antonio’s Pizza, 63 S. Airport Drive – The following violations were reported during a routine inspection: sanitizer level at three-compartment sink is at 0 ppm; observed grime build-up in self-serve drink machine ice dispenser; observed grease build-up and dried food debris on floor mixer; Styrofoam to-go containers not stored inverted; dried food residue found on shelves in walk-in cooler; observed cracked floor tiles throughout the kitchen.

Domino’s Pizza, 5526 Lakeside Avenue – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed an abundance of small flies coming from the mop sink; facility does not have quat test strips for sanitizer at the three-compartment sink; observed dust and debris accumulated behind the fan grate on the prep unit.

Teppanyaki Grill & Buffet, 9043 W. Broad Street – The following violations were reported during a routine inspection: open drinks found on the cookline; observed an employee wash hands at the prep sink (wash hands at hand sink only); observed an employee put on gloves without washing hands first; employees cutting watermelons used bare hands; employee at sushi bar not wearing gloves; employee bathroom hand sink and the service station are missing paper towels; several hand sinks in kitchen are obstructed; employee bathroom hand sink is missing soap and the toilet is broken; trash can in women’s restroom is not covered; found cans dented at seam; butter in the walk-in cooler is moldy; oyster packing records are not dated with date finished; raw chicken incorrectly stored over raw beef in walk-in cooler; ice machines at both service stations have a layer of grime; tea nozzles and can opener are dirty; bleach sanitizer at dish machine is empty; observed food debris on utensils, trays and a large pot; leftover sushi is being fried in order to re-serve; temperatures of chicken feet, raw chicken, shrimp, raw beef and rice are elevated; observed incorrect date labeling throughout the walk-in cooler; other units had no date marking at all; observed a bin of food from yesterday’s buffet in the walk-in cooler (per time control policy, it should have been discarded after four hours); no consumer advisory sign posted next to sushi; multiple sanitizer buckets have too high a concentration of bleach; display at sushi bar says crabmeat but it is imitation crab; bulk bin of cornstarch and other white powders are unlabeled; observed several wet wiping cloths out on food prep tables; ice scoop in ice machine is in the ice; bulk bins have scoops without handles; observed metal bins stacked wet; oyster shells are being reused for stuffed oysters; low boy and top of prep unit on cook line are not keeping proper temperatures; racks in two-door reach-in cooler are rusty; several cutting boards are heavily scratched and stained; observed grease on foil-wrapped pipes next to wok; prep unit containing chicken feet is dirty; floor drains contain a lot of food debris; floors and walls are dirty; mops incorrectly stored on the ground (hang to air dry); lights are out throughout the kitchen; ceiling tiles are falling in dry storage; hood filter has a gap; hood filters on cook line are heavily soiled with grease dripping down.

JULY 30

Roy’s Big Burger, 5200 Lakeside Avenue – The following violations were reported during a routine inspection: build-up of grime found inside ice machine; lid on chest freezer is broken; observed dried food debris on non-food contact parts of Astro blender and food debris on interior of two-door reach-in cooler; trash receptacle in employee restroom doesn’t have a cover; observed cracked and stained ceiling tiles in storage area; wet mop is sitting in an empty bucket instead of being hung to air dry; observed grime and food debris on lower parts of walls in kitchen and dish areas.

Canastas Chicken, 5233-35 S. Laburnum Avenue – No violations reported during a follow-up inspection.

O’Charley’s Restaurant, 9927 Mayland Drive – The following violations were reported during a follow-up inspection: cutting boards are scratched and stained; racks throughout the kitchen are rusty; observed encrusted baking pans; observed accumulation of grease on sides of fryers; floor under/around fryers is dirty; there are multiple cracked floor tiles in the kitchen; hood filters are greasy.

Cheesecake Factory, 11800 W. Broad Street – The following violations were reported during a follow-up inspection: black mold build-up found in ice machine; temperatures of multiple items in all make tables on the cook line are elevated; cook line equipment is dirty.
