JULY 21

Tazza Kitchen, 3332 Pump Road – The following violations were reported during a routine inspection: observed an employee touch his/her hat with gloved hand and did not change gloves; dishes on the clean dish rack had food debris; can opener is dirty; items on ice at the pizza/bar area had elevated temperatures (sauces, greens); observed small flies around drains in kitchen and at bar; utensils incorrectly stored in an equipment crevice; two cold holding units in kitchen have torn gaskets; bulb under the to-go hood is burnt out; cutting boards are scratched and stained; drip trays and sides of equipment are soiled; observed a leak under the three-compartment sink; floor at cook line is greasy; floor tiles at the back door are cracked.

JULY 22

Hawk’s Fish & BBQ, 3050 Williamsburg Road – The following violations were reported during a follow-up inspection: three loaves of bread in side room are moldy; temperature of mac n cheese is elevated because it was not cooled properly; temperature of bologna in drawer is elevated; no date marks found on mac n cheese, ribs or BBQ; sanitizer level of wet wiping cloth bucket is at 0 ppm; scoop for mac n cheese does not have a handle; wall behind three-compartment sink is dirty; floor is wet with fish water.

Roots Natural Kitchen, 6602 W. Broad Street – The following violations were reported during a routine inspection: top level of delivery unit is not keeping foods at the correct temperature; improper cooling methods used for shrimp, roasted potatoes, and roasted broccoli in the walk-in cooler (items were cooling in tightly-wrapped and stacked containers); observed containers that had been cleaned and sanitized stacked on a shelf while wet.

Las Delicias Restaurante, 100 W. Nine Mile Road – The following violations were reported during a routine inspection: no date marks found on any TCS foods in refrigeration; observed a “thank you” bag being used as a food contact surface to cover cut melon; facility could not provide chlorine test kit.

JULY 23

Mama Cucina, 4028-O Cox Road – The following violations were reported during a routine inspection: person in charge was not able to identify the “big six” foodborne illnesses and their associated symptoms; facility does not have a written employee health policy or written procedures for cleaning vomit or diarrhea; observed an employee wash hands at spray hose in dish area (use hand sink); hand sink is missing paper towels; observed an employee bread raw chicken with gloved hands and then prepare salad and handle cooked meat with same soiled gloves; raw shrimp incorrectly stored next to ready-to-eat herbs in top of prep unit; no sanitizer detected at the dish machine (container was empty); temperatures of raw chicken, half-and-half, spinach and garlic in oil are elevated; temperatures of spaghetti, bowtie, rigatoni and penne are elevated because they were not cooled properly (they were stored tightly covered with plastic wrap in deep containers); observed wet wiping cloths and soiled wiping cloths throughout the kitchen; utensils stored in water that is not hot enough; wood at bar is unsealed; cutting boards are heavily scratched; observed food debris and grease on shelving and equipment; hand sink faucet leaks; floor at bar, especially in corner to left of sink, is soiled; ceiling tiles are cook line are dirty and discolored; observed heavy grease on hood filters.

Country Style Donuts, 8902-B W. Broad Street – The following violations were reported during a follow-up inspection: observed small flies; back door has gap at the bottom; one gasket on proofing box is still torn; wood near cash register is unsealed; ceiling and vent in the back are very dusty; hood filters are dusty and greasy.

Urban Greenlight, 6329 Mallory Drive – The following violations were reported during a routine inspection: wood shelves are unsealed; threaded faucet at mop sink does not have a backflow preventer; restroom door does not have a self-closure device.

DaVinci, 4933 Nine Mile Road – The following violations were reported during a routine inspection: facility could not provide a certified food protection manager certificate; back hand sink is blocked by a rolling table; temperatures of multiple TCS foods (shredded lettuce, chicken) are elevated; degreaser stored in an unlabeled bottle; squirt bottles of various oils are unlabeled; prep top unit by cook line is not holding foods at the correct temperature; observed multiple holes in wall of storage area.

JULY 26

Anokha Indian Cuisine, 4015 Lauderdale Drive – No violations reported during a follow-up inspection.

Chick-fil-A, 10176 Brook Road – The following violations were reported during a routine inspection: ensure thermometer outside of walk-in is accurately showing temperature; metal bins are stacked wet; facility could not provide a testing kit for the high temp dish machine; many surfaces found dirty including door inside of thaw cabinet (grease build-up), side of bulk sugar container, ladder next to shake machine and wall at mop sink; observed water pooling on floor of salad prep line; floor drains contained food debris and trash; observed a few cracked floor tiles near the freezer.

Rose Marie Inn, 8923 Patterson Avenue – The following violations were reported during a routine inspection: observed debris in the iced tea dispensers, mold in the ice machine and food debris in the microwaves; no consumer advisory located on the menu for eggs and burgers that can be cooked to order; employee restroom doesn’t have a door (only has a curtain); observed grease on the hood vents.

Arby’s—Tuckernuck Square, 9080 W. Broad Street – The following violations were reported during a routine inspection: person in charge could not describe procedure for cleaning up vomit or diarrhea; TCS foods in top of sandwich unit (turkey, chicken) have elevated temperatures; temperature of shake mix in refrigerated unit is elevated; shake machine does not appear to be maintaining mix at proper temperature; temperatures of potatoes and salads are elevated because proper cooling methods were not followed (they were stored covered); many small flies observed in food prep area near hand sink; utensils incorrectly stored in sanitizer solution; exposed spray foam found at end of cook line; observed low grout near hand sink; observed cracked tiles by back door and dry storage area.
