JULY 14

Red Lobster, 4415 S. Laburnum Avenue – The following violations were reported during a routine inspection: facility could not provide a parasite destruction letter from salmon distributor; scoops for the seasoned breader, fish batter and pico de gallo do not have handles; observed ice buildup on ice cream freezer and in walk-in freezer; latch on low boy reach-in cooler is broken; leak is coming from worn out caulking at the back hand sink; loose fries found at the bottom of low boy freezer on the cookline; observed heavy grease buildup on back of fryers; cardboard boxes on the ground in dumpster area need to be cleared; door to chemical storage and other doors are dirty; ceiling vents above coffee/tea are dusty; ceiling tiles are stained.

Shagbark Restaurant, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: hand sink near grill is slow to drain; observed food debris in hand sink; facility could not provide documentation that raw salmon is farm raised and pellet fed; dish machine at bar had zero sanitizer; salmon and duck, which person in charge states can be served undercooked, are not currently indicated as such on the menu; observed pickled grapes and okra with dates marked March and April, respectively; employee restroom does not have a covered receptacle; light bulb is out in hood system; observed heavy dust buildup in dish area.

Pho 1 Grill, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: employee restroom does not have paper towels; raw beef products incorrectly stored over spring rolls in the low boy; temperature of cabbage stored above the air curtain in the prep top is elevated; two spray bottles of cleaning chemicals in the kitchen are unlabeled; observed rice scoops stored in standing water; one light is burned out under the hood.

JULY 15

Peking Restaurant, 8904-F W. Broad Street – The following violations were reported during a follow-up inspection: slicer is dirty; wood behind three-compartment sink is unsealed; ceiling is charred and stained, especially in back of the kitchen; three lights are out in the kitchen.

JULY 16

Terra Nova Bar & Grill, 4700 S. Laburnum Avenue – The following violations were reported during a routine inspection: hand sinks at bar and front of kitchen do not have hot water; hand sink at drink service area before kitchen does not have a functioning paper towel mechanism; found moldy orange and melons in the walk-in; interior of both ice machines have a layer of pink slime buildup; slicer in back of kitchen and several stored utensils in a bin have food debris; several foods found past their expiration date (mashed potatoes, cooked beets and oatmeal in the sandwich prep unit on the cookline, and mashed/cooked potatoes and cut melon in the walk-in); no consumer advisory located on the menu for eggs and burgers that can be cooked to order; observed salmon filets in the walk-in that were in the reduced-oxygen packaging to thaw; small flies observed near a soiled towel in the service area; plates are not stored inverted; observed three utensils incorrectly stored in standing room-temperature on the cookline; microwave is broken and propped up with a piece of cardboard; two lights on the hood are not working well; dish machine is not sanitizing properly; observed three broken metal skimmers hanging above the three-compartment sink; facility could not provide any test strips for quat or bleach sanitizer or non heat sensitive test strips for the high-temperature dish machine; fryer baskets are soiled with grease buildup greater than one day; reach-in cooler next to the cookline is covered in mold; shelves with spices are soiled with food debris; rolling carts and sides of equipment are dirty; broken and unused stove top is sitting in kitchen across from prep sink; three lights are out in the kitchen; tiles are missing or damaged throughout the kitchen; kitchen ceiling has holes; floors under cooking equipment are greasy and dirty; hood filters are greasy.

Jimmy John’s, 11301 W. Broad Street – The following violations were reported during a routine inspection: sliced meats and cheeses were put directly into the prep unit to cool along with large bins of shredded lettuce covered with plastic (cooling should be done uncovered in the walk-in); walk-in air vent is dusty; corners of walk-in floor are dirty.

International House of Pancakes, 4840 S. Laburnum Avenue – No violations reported during a follow-up inspection.

JULY 19

Rick’s Rock Café, 1500 Eastridge Road – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; no handwashing sign is posted; raw shell eggs incorrectly stored above ready-to-eat foods in the reach-in; interior of ice machine has black slime; temperatures of turkey and pastrami in lower section of the make table are elevated; container of cleaner and container of sugar are unlabeled; bus tub incorrectly used as a shelf; make table door has a torn gasket; observed paper lining shelves; facility does not have test strips for sanitizer used; drip trays to stove and top of grease traps are dirty; exposed foam found around pipes.

Leonardo’s Pizza & Pasta, 10883 W. Broad Street – The following violations were reported during a routine inspection: facility could not provide a certified food protection manager certificate; bucket and pans of sauces incorrectly stored on both walk-in floors; observed multiple cracked/chipped lexan pans; wood shelves in the prep area are unsealed; wire racks in various locations are rusty; facility does not have an irreversible thermometer or temperature stickers for the high temperature dish machine; gaskets in refrigeration/freezers and shelving on pass thru are dirty; fan guard in walk-in is dusty; observed cracked floor tiles in one walk-in and in the prep area; coving is missing under the prep sink; kitchen has some patches of low grout; ceiling tiles are discolored; vents are rusty; floors under and behind equipment, walls behind equipment and ceiling vents are dirty.

Five Guys Burgers and Fries, 10841 W. Broad Street – The following violations were reported during a routine inspection: cloths are being stored in sanitizer with too low of a concentration; hood filters are dented (one is dented in a way that caused a gap).
