Broad Street – The following violations were reported during a routine inspection: pan found in front handsink (handsinks are for handwashing only); front handsink is missing paper towels; facility does not have parasite destruction records from their salmon provider; covered raw chicken incorrectly stored next to other covered raw meats and raw shell eggs in walk-in; observed debris on utensils in storage; dark substance is starting to form inside ice machine; temperature of spicy tuna in the sushi case is elevated; containers of brown rice in the walk-in are not date marked; take-out menus do not have a consumer advisory, but raw/undercooked items are identified; three bags of onions incorrectly stored on kitchen floor; take-out containers on shelf above grill are not stored inverted; top shelves of storage areas throughout the kitchen are dusty; shelves under the grills have splatters on them; metal wall sheet behind dryers is dirty.

New Asian Panda, 10430 Ridgefield Parkway – The following violations were reported during a follow-up inspection: raw chicken incorrectly stored over broccoli in the walk-in; cookline wok line has build-up.

Frida’s Mexican Restaurant, 910 E. Parham Road – The following violations were reported during a follow-up inspection: no dates found on cooked foods; no disclosure written on menu; no test strips available for ProGuard sanitizer.

International House of Pancakes, 9820 W. Broad Street – The following violations were reported during a follow-up inspection: observed live German roaches under the cookline; observed heavy grease build-up under grill top and on top of prep table; mopsink has build-up; milkshake machine has ice cream splatter; observed low grouting with dirt and standing water; old food and grease found under cookline.

Boychik’s Deli, 4024-B Cox Road – The following violations were reported during a routine inspection: handsink at cookline is obstructed by trash can and buckets on the ground; temperatures of cream cheese, tomatoes on salads, tuna salad and chicken in walk-in cooler are elevated; temperature of sliced cheese in reach-in on the cookline is elevated because it was left out; observed person in charge use Clorox cleaner plus bleach at level above 200 ppm to clean slicer; bucket of potatoes incorrectly stored on floor at cookline; spray nozzle attached to hose at mopsink does not have proper backflow device; observed build-up of grease on wall at cookline and on sides of the hood system; ceiling tiles in this area are heavily discolored; ceiling around vents is dusty.

Turmeric Indian Cuisine, 3621-C Cox Road – No violations reported during a follow-up inspection.

Honest Restaurant, 8900-A West Broad Street – No violations reported during a follow-up inspection.

Dominion Club, 6000 Dominion Club Drive – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; high temperature dish washer did not reach 160 degrees after multiple runs; pink slime found on interior of the ice machine; opened unsliced portions of deli meats are not date marked; proper cooling methods not followed for ribs on cooling rack covered with plastic wrap and drunken beans in a deep container; did not observe internal hanging thermometers in two new fridges; bulk bins of white powder ingredients are not labeled; found unsealed wooden boards that may be used for food service; gasket in True Freezer is torn; facility does not have test stickers or an irreversible thermometer for monitoring the dish washer; facility does not have a test kit for the quaternary ammonium sanitizer; floor is worn/pitted in front of the cookline; ceiling tiles throughout the kitchen are discolored; floor under/behind fryers is greasy; walls in dish area are soiled with mildew, especially under the rinse sink.

McDonald’s, 10000 Brook Road – The following violations were reported during a follow-up inspection: lettuce on sandwich board was given a 24-hour time mark (lettuce has only four hours once placed on the line); overflow tray at soft-serve machine still has build-up; coving by walk-in is missing; grouting in mopsink is missing.

Starbuck’s Coffee, 9004 W. Broad Street – The following violation was reported during a routine inspection: did not observe test strips for high temperature dish machine.

The Lilly Pad, 9680 Osborne Turnpike – The following violations were reported during a routine inspection: handsink near pizza prep does not have handwash signage; temperatures of shredded mozzarella cheese, marinara and crab dip in pizza prep unit are elevated; coleslaw in the walk-in cooler and whipped cream in the low boy unit are past their expiration marks; no consumer advisory found on menu for raw/undercooked tuna or burgers; sani bucket tested at 500 ppm (should be kept between 200-400 ppm); single-service trays on top shelves and boat-style platters are not stored inverted; observed unfinished wood around window in prep area and above bar area; trash cans in women’s restroom are uncovered.

Kabuto Japanese House of Steaks, 8052 W. Broad Street – The following violations were reported during a routine inspection: person in charge could not produce certified food protection manager credentials during inspection; facility does not have parasite destruction records from salmon provider; observed food debris on various utensils on a sheet pan at the prep table; ice machine and soda nozzles at bar are dirty; hot holding temperature of miso soup at the sushi station is too low; sanitizer spray bottles on the chemical rack are not labeled; observed lettuce stored in large, deep container in the walk-in (it is recommended to use smaller containers); boxes of food incorrectly stored on walk-in freezer floor; tempura station low boy has torn gasket; racks in the walk-in and chemical storage are rusty; cutting board at sushi station is deeply scored and stained; fan guards in walk-in are dusty; observed white residue on racks in walk-in and the storage rack in front of the meat reach-in; bar area handsink leaks excessively; ceiling tile linings appear worn or discolored; grouting is low and discolored; observed cracked floor tiles in dry storage.
