Glenn’s Forest Café, 7202 Glen Forest Drive – The following violations were reported during a routine inspection: temperatures of bacon and sliced cheese are elevated; low boy units in kitchen have torn gaskets; cutting boards are heavily scratched; observed debris on racks throughout, on sides of equipment on front line, in between gaskets on convection oven, in drawers, and on utensils in drawers on cookline; ceiling vent is dusty; observed trash and debris on floors under equipment throughout; walls on cookline are stained.

O.M.O. & Co., 6329 Mallory Drive – The following violations were reported during a routine inspection: raw bacon and sausage incorrectly stored next to raw chicken in reach-in unit; temperatures of raw bacon, broccoli and cooked bacon in reach-in units and refrigerated drawers are elevated; temperature of rice is elevated because it is not cooling properly; observed in-use knives and other utensils stored in standing water on shelf next to convection oven; scoops, tongs and spatulas stored uninverted on shelf and prep tables on cookline and in dry storage room; single-service plates stored uninverted on rack in dry storage room; racks across from three-compartment sink are rusty; cutting boards on shelf near cookline are heavily scratched; observed debris on racks throughout, on shelves next to dish area, on top of dish machine, under grill on cookline, on sides of hood filters, and in utensil containers on cookline; handsink in kitchen is slow to drain.

Tavern at Triangle Park, 7110-F Patterson Avenue – The following violations were reported during a follow-up inspection: temperatures of ground beef, oysters and raw chicken in low boy unit on cookline are elevated; temperatures of shrimp mix, pastrami, cheese, tomatoes and spinach are elevated; racks in dry storage areas and walk-in cooler are rusty; multiple refrigeration/freezer units in back kitchen have torn gaskets; cutting boards in back kitchen are heavily scratched and stained; did not observe quaternary ammonium sanitizer test strips; observed cracked floor tiles and chipping paint/deterioration on walls in back kitchen and bar; ceiling tiles in dry storage area/office are not easily cleanable.

Sparrow’s Pizza, 6313 Rigsby Road – The following violations were reported during a routine inspection: observed debris on racks in kitchen, on lids, on pizza convection oven, and on outside of containers on racks across from three-compartment sink; shelving at front cashier area is dusty; ceiling tiles throughout the facility are not easily cleanable and material around ceiling vent is not easily cleanable.

Hai Ky Mi Gia, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: raw beef incorrectly stored over and next to carrots, and cooked pork incorrectly stored next to raw meats in low boy unit on cookline; raw beef incorrectly stored over carrots in reach-in unit; temperatures of pho broth and conji made the previous night are elevated because they were not cooled properly; temperatures of garlic and oil, raw eggs and bok choi in prep top unit are elevated; sprouts left out at room temperature did not have a written time mark; menu is missing asterisks on items that are served raw/undercooked; temperatures of beef and pork prepped 30 minutes prior are elevated because they were not cooled properly; multiple white powder containers on rack in back kitchen are unlabeled; box of onions, bucket of broth and oil box incorrectly stored on the floor; observed in-use utensils stored in standing water on cookline; to-go containers not stored inverted on cookline; single-service boxes incorrectly stored on floor in dry storage room; cutting boards on prep table are heavily scratched; low boy unit has torn gasket; racks in freezer and walk-in cooler are rusty; observed ice accumulation on pipes and bottom of freezer unit; observed debris on racks throughout, on hood filters and in between equipment on cookline, on back of wok station, on bottoms of reach-in freezer, in containers stored on floor in back kitchen, outside of metal pans, and on equipment on rack in back kitchen; handsink near cookline is slow to drain.

Bella Luna, 1212 Concord Avenue – The following violations were reported during a routine inspection: observed person in charge place raw steak on a grill with gloves and then switch tasks without removing gloves and washing hands; observed pink slime on ice machine; observed debris on peeler and slicer; used chicken breading incorrectly stored at room temperature; items in cooling units did not have date marks (chicken, sliced turkey, goat cheese, ricotta, sliced tomatoes and grilled chicken); menu does not contain a consumer advisory for steak which can be cooked to order; hand sanitizer and deodorant incorrectly stored next to food; wine bottles incorrectly stored on the ground; observed a cloth doily in bar area; gasket on freezer is in poor repair and the door doesn’t close; chemical shelf, legs of three-compartment sink, top of oven, interior of reach-in cooler and exterior of microwave need cleaning; air vent in women’s restroom is dusty; mop incorrectly stored on floor after use.

Subway, 9127 Staples Mill Road – The following violation was reported during a routine inspection: observed three pans of uncovered slice cheeses incorrectly stacked on top of one another in the walk-in.

Visions Child Development Center, 4501-L Williamsburg Road – The following violations were reported during a routine inspection: temperatures of reach-in in main kitchen and black homestyle refrigerator are elevated; inside of units need cleaning; observed build-up of grease under three-compartment sink; kitchen floor is sticky and needs cleaning.

Panera Bread, 1370 N. Parham Road – The following violations were reported during a follow-up inspection: temperatures of lettuce in refrigerated drawers on the line and temperatures of arugula, sliced cheese and chicken in prep top unit on the line are elevated; quaternary ammonium sanitizer level at three-compartment sink is greater than 0.55 ppm; observed multiple uninverted forks on rack next to dish machine; multiple cutting boards on front service line are heavily scratched; air temperature of refrigerated drawers on front line is elevated; observed low grouting throughout kitchen floor.

Azzurro Restaurant, 6221 River Road – The following violations were reported during a follow-up inspection: menu is missing asterisks on items that are served raw/undercooked; boxes of beer and wine incorrectly stored on floor at bar area; multiple cutting boards are heavily scratched; light bulbs are out at hood filters on the cookline; racks in walk-in cooler and dry storage room are rusty; multiple units have torn gaskets; observed holes/deterioration on walls at service station; observed low grouting throughout; cracked floor and wall tiles found in dish room; ceiling tiles in kitchen are not easily cleanable; floor in dry storage room needs to be sealed with paint.
