Sportsman’s Restaurant & Lounge, 3306 Williamsburg Road – The following violations were reported during a routine inspection: person in charge does not have a “serve safe” certificate; observed person in charge eating in the kitchen; paper towels and soap are missing from handsink in the bar area; raw meat incorrectly stored next to ketchup and raw meat incorrectly stored next to potatoes and over open wine in the reach-in cooler; sanitizer in three-compartment sink is over 200 ppm; observed utensils stored between three-compartment sink and wall; plates stored on top shelf are uncovered; observed build-up and food debris on upper part of ice machine, around the frying equipment and on sides of refrigerator doors and gaskets; kitchen handsink faucet is moldy; observed dust accumulation on fan in kitchen and under cooking equipment; light in the kitchen is not working; observed broken floor tiles in side room.

YaYa’s Cook Book, 11674 W. Broad Street – The following violations were reported during a routine inspection: kitchen handsink is missing paper towels; soap dispenser at handsink doesn’t work; no parasite destruction documentation available for sushi; tags are not available for fresh mussels; raw tuna incorrectly thawing above avocadoes; raw chicken and raw beef incorrectly stored together in walk-in; sanitizer not detected at bar dish machine; temperature of rice in microwave is not hot enough; temperatures of red curry sauce and panang sauce are elevated because they were not adequately cooled; elevated food temperatures detected in several units (sticky rice in white homestyle fridge, sushi station crabstick in lowboy, cream cheese and seared tuna in case, raw beef and dumplings in fridge in soup area, fried salmon and shrimp on counter); no dates found on any prepared foods; consumer advisory on menu is incomplete (foods served raw are not marked); chemical spray bottles incorrectly stored next to food, clean equipment and utensils; improper cooling methods observed; thawed tuna found in reduced-oxygen packaging; containers of spices, sugar and vinegar are unlabeled; containers of food incorrectly stored on the floor and uncovered in walk-in units; utensils incorrectly stored in room water temperature; ice scoop handle is in contact with ice water; observed cloth covering handles of homestyle fridge and freezer; shelving throughout the kitchen is soiled with food debris and dust; mops are not hung to dry; ceiling tiles above cookline and wall on other side of cookline are soiled/in poor repair/discolored; hood filters are greasy.

Sheetz, 7035 W. Broad Street – The following violations were reported during a routine inspection: temperature of sliced cheeses in prep top unit on front service line are elevated; cutting boards on front service line are heavily scratched; observed debris on racks and shelves in front service line, on sides of fryers on cookline and on top dish machine in back kitchen; observed low grouting on floors in kitchen.

Hungry Dragon, 3422 Lauderdale Drive – The following violations were reported during a follow-up inspection: slicer has food debris in areas; interior of two-door low freezer and interior of two-door tall freezer are dirty; lower wall under spray sink and dish area is dirty.

Texas Roadhouse, 10075 Jeb Stuart Parkway – No violations reported during a follow-up inspection.

Habanero Mexican Grill, 9101 Quioccasin Road – The following violations were reported during a routine inspection: temperatures of cheese, pork and shredded beef in unit on cookline are elevated; multiple salsa cups in reach-in unit do not have date marks; refried beans prepped one hour prior are not cooling properly; salts and sugar containers on rack in kitchen are unlabeled; observed wet nesting of containers on rack at three-compartment sink; reach-in freezer in back kitchen has a torn gasket; cutting boards are heavily scratched; observed grease/debris under fryers and grills on cookline and under hood filters on cookline; observed debris on utensils stored clean in water heater room; observed cracked floor tiles, chipping paint on walls and low grouting on floor next to cookline; ceiling tiles are not easily cleanable; observed grease on walls behind cookline and stains on walls in the dish room.

Panera Bread, 1370 N. Parham Road – The following violations were reported during a routine inspection: quaternary ammonium sanitizer level is at 0 ppm at three-compartment sink in back kitchen; observed food debris greater than one day old on ladles, equipment and scoops hanging next to chemical dish machine, as well as on rack next to ice machine and above three-compartment sink; temperatures of chicken, cheese, lettuce and leafy greens in prep top unit and refrigerated drawers on front service line are elevated; did not observe a written time on skim milk and half-and-half that were left out at room temperature at drink station; forks on rack next to dish machine are stored uninverted; cutting boards on front service line are heavily scratched; observed debris on racks and shelves throughout, on top of dish machine, in utensil containers on front service line, in ice chute at soda dispenser, and in between and under equipment on front service line; observed low grouting on floors throughout kitchen; trash and debris found under equipment throughout the facility and under soda dispensers in front dining area; fan guards in both walk-in coolers are dusty and ceiling in front service line is dusty; observed stains on walls throughout; observed debris on floor under walk-in freezer.

Mariscos El Barco, 6409 Rigsby Road – The following violations were reported during a routine inspection: an employee’s bare hand came into contact with food in the prep area and an employee touched cucumber in prep area in kitchen with bare hand; did not observe proper sanitizing of cleaned multi-use equipment and utensils; manager had multiple shell stock tags that were not dated or in chronological order; manager did not keep shell stock tags with specific bushels of shell stock in walk-in cooler and freezer chest; facility could not provide a certified food protection manager certificate during inspection; manager was unaware of the “big six” foodborne illnesses and the associated symptoms; manager does not know how to properly clean up a possible vomiting or diarrheal event; facility could not provide a parasite destruction letter for fish used in ceviche; raw shrimp incorrectly stored over garlic bread and raw eggs incorrectly stored next to milk in walk-in cooler; observed food debris on knives on wall and utensils hanging above prep table in prep area, as well as on top of plates stored clean on table next to sandwich unit; temperature of whipping cream on shelf on cookline is elevated; menu does not have asterisks on all foods that are served raw/undercooked; Pledge chemical sprays incorrectly stored next to sweet and sour mix and crushed peppers in dry storage room; multiple powders on shelf on cookline are unlabeled; multiple foods incorrectly stored on the floor in outdoor storage room and on the floor in front of walk-in cooler; ice scoop incorrectly stored on top of chest freezer in front of walk-in cooler; forks stored uninverted at cash register in kitchen; observed wet nesting of metal pans on racks above three-compartment sink in main kitchen; single use trays stored uninverted on shelf in main kitchen; cutting boards in kitchen are heavily scratched; low boy unit on cookline has torn gasket; racks in walk-in cooler are dusty; observed broken metal skimmer hanging above three-compartment sink; observed stone morcahetes on shelf on cookline (only use plastic); observed debris on racks in walk-in cooler, racks throughout kitchen, on hood filters on cookline and on sides of equipment on cookline; observed grease under stove top on cookline; observed cracked and missing floor tiles in kitchen, especially in prep area.
