New Dragon City, 2220 John Rolfe Parkway – The following violations were reported during a routine inspection: used breading found at room temperature; Gen Tso’s chicken and cooked chicken inside the walk-in do not have date marks; mopsink is dirty; interior of reach-in freezer is soiled.
Subway Sandwiches & Salads, 3326 Pump Road – The following violation was reported during a follow-up inspection: observed build-up on freezer floor.
Papa John’s Pizza, 4744 S. Laburnum Avenue – The following violations were reported during a routine inspection: handwash signage is missing at handsink by the dish machine and bathroom handsink; observed debris and sauce splatter on floors throughout; observed cracked flooring tiles throughout, especially near dish machine and walk-in.

International House of Pancakes, 9820 W. Broad Street – The following violations were reported during a routine inspection: observed an employee handle raw steak and then wash hands without soap, place on new gloves and handle clean plates; soap is missing at handsink on the cookline; observed a spoon and debris in handsink by dish machine and a container of ketchup blocking and leaking into servers’ handsink; observed debris on ladles over three-compartment sink and butter dried on brushes; temperatures of ham, turkey and hash browns are elevated; temperature of cheese sauce is elevated after sitting out in back kitchen 1.5 hours after preparation; found torn gaskets or gaskets coming off units throughout the facility; observed water at bottom of low boy across from the cookline and dirty water sitting in units under grill; fan is leaking onto product in reach-in; metal plates are missing on fryers and oven; observed build-up of grease and/or old food on fryers, sides of fryers, under fryer, on prep tables, and on side of grill on cookline; observed ice cream splatter on racks for clean cups; observed low grouting or cracked floor tiles throughout the facility; ceiling fans are dusty; drains in server area and dish area need to be cleaned; observed food debris build-up under cookline and dish area.
China Wok, 11530-C W. Broad Street – The following violations were reported during a routine inspection: employee drinks and clothes are stored next to food on dry storage shelf; plastic containers found in handsink (handsink is for handwashing only); kitchen handsink is missing soap; cabbage incorrectly stored next to raw meats in refrigerator on the line; different types of raw meat and prepared dumplings incorrectly stored together on a speed rack in the walk-in freezer; observed food splatter inside microwave; plastic container with wontons is heavily soiled with greasy food residue; fried chicken prepared earlier incorrectly cooling by sitting out on the counter; garlic in oil is out on cart by woks with no attempt to keep cold; egg rolls and tofu are not date marked; observed in-use knife incorrectly stored in crevice between refrigerator and counter; cutting boards at the refrigerator units on the line are heavily scratched and discolored; observed grease and food residue on shelving throughout the kitchen.
Honest Restaurant, 8900-A W. Broad Street – The following violations were reported during a routine inspection: no paper towels found at any handsink in the facility; multiple prepped items in low boys are not covered; observed 0 ppm of sanitizer when testing dish machine; temperatures of potato masala and rice are elevated; multiple prepped items (rice, noodles, etc.) in prep units are not date marked; spices on both cooklines are unlabeled; bulk containers of flour and sugar on dry storage shelf are unlabeled; observed small flies by little ice machine in the serving area; multiple wet wiping cloths found on prep surfaces; observed rice scoop touching rice and multiple plastic cups used for scooping touching food; observed grease and debris build-up under cookline in back kitchen.

Subway at Henrico Doctors’ Hospital, 1602 Skipwith Road – The following violations were reported during a routine inspection: temperature of meatballs and marinara in steam table on front line is not hot enough; temperatures of sliced and shredded cheeses on front line are elevated; cutting boards on front line are scratched; observed debris on shelves on front line, under convection oven, and on racks and fan guard in the walk-in cooler; ceiling tiles in the kitchen are not easily cleanable.
