Westwood Club, 6200 W. Club Lane – The following violations were reported during a follow-up inspection: cutting boards on cookline and in prep areas are heavily scratched; racks across from three-compartment sink are rusty; gaps greater than one inch found at hood system in bakery area; multiple refrigeration units have torn gaskets; air temperature of reach-in unit at service station is too high; observed debris on light bulbs, in between equipment on cookline, on shelves, on racks next to three-compartment sink, on panini press, and in utensil/dish containers in prep area and dish room; observed sticker residue on pans in the prep area; fan near cookline is dusty; observed chipping paint on walls and cracked floor tiles throughout the kitchen; missing/deteriorated ceiling tiles found in dry storage and bakery area; observed debris, slight grease and/or stains on floors and walls throughout; fan guards in walk-in cooler are dusty.

Cook Out Store, 1501 Eastridge Road – The following violations were reported during a follow-up inspection: facility does not have a certified food protection manager certificate; observed wet nesting of pans at three-compartment sink; low boy unit has torn gasket; refrigerated drawer in milk shake station does not close all the way; handsink next to the back door is slow to drain; observed low grouting throughout.

Al Modina, 10158 W. Broad Street – The following violations were reported during a routine inspection: observed raw shell eggs sitting out at room temperature; no prepared foods in the restaurant are date marked; spray bottle of bleach water is not labeled; bottom of reach-in is littered with old bits of herbs; observed a variety of cast-off items outside the back door.

Arby’s, 4250 Pouncey Tract Road – The following violations were reported during a routine inspection: temperature of rotisserie chicken is too low; potatoes and sweet potatoes were covered and placed into the refrigerator before properly cooling; metal racks used to store bags of meat are rusty.

E-Loteria Botanas & Ice Cream, 9022 W. Broad Street – The following violations were reported during a routine inspection: raw meat incorrectly stored with cooked chicken and over other ready to eat items; prepped items (tomatoes, lettuce, shrimp and salsa) do not have date marks; observed raw shrimp added to lime juice without a consumer advisory on the menu.

Blaze Pizza, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: temperatures of mozzarella, chicken, sausage and ricotta in prep unit on line are elevated; silicone pizza mats are in poor condition (black in color, carbon build-up).

DeFazio’s Catering, 2601 Tuckernuck Drive – The following violations were reported during a routine inspection: kitchen handsink does not have soap; observed old food build-up on slicer; temperatures of gravy, cheese and beans in the reach-in unit are elevated; observed grease build-up on sides of fryer and along the cookline; observed grime build-up on racks in the walk-in and dry storage racks.

Jack’s Place, 1881 Darbytown Road – The following violations were reported during a routine inspection: observed an employee handle raw chicken, remove gloves, place on new gloves and handled ready-to-eat food without washing hands; milk is past the expiration date; observed build-up of old food on slicer and on potato dicers in the back; nozzles for tea dispenser are dirty; multiple prepped items (chipped beef, lettuce, corn beef and deli meat) do not have date marks; observed food incorrectly stored on the dry storage area floor; observed grease build-up in units, on sides of equipment on the cookline, in fryer baskets, and on hood filters; heavy grime/food splatter found on walls, floors and ceiling; venting system next to prep unit is dusty.
