Hardee’s, 3201 E. Parham Road – The following violations were reported during a routine inspection: metal racks in the walk-in freezer nearest to the wall are excessively rusted; walk-in freezer floor under the condenser is excessively rusted; non-food contact surfaces throughout the facility are dirty and covered with grease (shelving, equipment interiors and bottoms, inside refrigeration and freezer units, gaskets and pipes); floors, walls and ceiling throughout the kitchen are heavily soiled; observed heavy dust and dirt on ceilings; walls are stained; observed large amounts of grease, food debris and trash on the floors under equipment; grouting in walk-in freezer is worn.

Courtyard by Marriott, 10077 Brook Road – The following violations were reported during a routine inspection: front handsink is missing hand towels; observed liquid egg past its expiration date in the walk-in cooler; observed a hose attachment below flood line at the mopsink with no backflow device.

Pho Thien Phat, 6403 Rigsby Road – The following violations were reported during a routine inspection: men’s restroom is missing handwash signage; observed debris in the tea nozzle in front service area; multiple utensils and equipment stored on clean rack in the kitchen are soiled; temperature of rice reheating in the rice warmer is not hot enough; temperature of sprouts in multiple reach-in units in back of the facility is elevated; temperatures of noodles, broth and sprouts in the reach-in unit next to the prep sink are elevated; multiple broths in the reach-in and low boy units were taken out to thaw and not dated; temperatures of beef, pork, marinated beef, sausage, garlic and oil, chicken and tofu in low boy unit and prep top on the cook line are elevated because they were cooled incorrectly; temperatures of egg roll in reach-in unit and tapioca in the display case are elevated because they were cooled incorrectly; observed one live roach on cook line near wok; observed multiple foods stored on the floor in the walk-in freezer and onions stored on the floor in the room with reach-in units; observed multiple to-go trays stored uninverted on front service area and back kitchen; multiple refrigeration units have torn gaskets; racks in the dish area are rusty; observed a gap greater than one inch on hood filters on the cook line; cutting boards are heavily stained; observed debris greater than one day old on fryer baskets on the cook line; debris found on racks in the dish area, in drawer next to the prep sink, under stove/cook line, on pipes of the hood system, outside of wok on the cook line and on top the dish machine; observed slight debris on walls and floor of the cook line; observed low grouting and cracked floor tiles throughout the kitchen.

Red Robin America’s Gourmet Burgers & Spirits, 10067 Brook Road – The following violations were reported during a routine inspection: observed beer cooler in the bar in disrepair; observed leak from the men’s bathroom handsink and a leak from the dish machine pipe; observed low grouting throughout the kitchen, especially in the dish machine area; floors under the cook line and under the dish machine need cleaning; floor drains need cleaning.

Target, 10100 Brook Road – The following violations were reported during a routine inspection: observed pink mold on the ice machine shield; observed a leak from bottom of the dish machine.

Buckhead’s Chop House & Restaurant, 8510 Patterson Avenue – The following violations were reported during a routine inspection: canned crab meat stored incorrectly next to raw tuna and calamari in the walk-in cooler; observed debris greater than one day old in mixing bowls on rack next to the back door; observed debris on can opener blade in the prep area; temperatures of half-and-half in the low boy unit and raw steaks in reach-in unit are elevated; observed ROP tuna incorrectly thawing in vacuum-packed bag; observed utensils in rack at the three-compartment sink stored uninverted; multiple units have torn gaskets; racks in reach-in units on the cook line are rusty; observed door to reach-in unit and low boy freezer unit not sealed completely, allowing cold air to escape; cutting boards are stained and heavily scratched; wooden shelves in the dry storage room and ceiling tiles in back of the facility are not easily cleanable; handsink on cook line and handsink in men’s restroom are slow to drain; faucet at the prep sink is leaking; receptacle in employee’s and women’s restrooms are missing lids; observed heavy stains and debris on wall behind the dish machine; observed dust on a ceiling vent in the dish area; observed debris on bar area floor; multiple ceiling tiles above the reach-in unit on the cook line are cracked; multiple floor tiles in the kitchen are cracked; observed low grouting throughout the kitchen.

Crossings Golf Club, 800 Virginia Center Parkway – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed multiple pieces of equipment in disrepair; observed low grout in the kitchen, especially in the three-compartment sink area.

Cielito Lindo, 6003 W. Broad Street – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed raw eggs stored incorrectly next to salsa in low boy unit and raw ground beef stored incorrectly next to cheese and over pineapple juice in the back reach-in unit; observed food debris inside of grinder and blender stored clean on prep table in the dish room; observed debris on cheese grater stored clean on equipment rack next to the reach-in unit; observed 0 ppm quaternary ammonium at three-compartment sink; temperature of horchata in reach-in unit is elevated because proper cooling methods were not used; temperatures of refried beans in the oven and cheese in the low boy unit are elevated; observed beans and cheese, pork and cheese and veggie cheese out at room temperature at the end of the prep sink; incorrect prep dates found on coleslaw, lettuce and salsa in reach-in unit in the dish area; observed items not being held on a time control policy (beef soup on stove and refried beans and chicken on the hot line); steramine (chemical) tablets were stored incorrectly next to condiments on a shelf in the dish area; temperature of tomatoes stored in prep top unit on the cook line is elevated; cutting boards are stained and heavily scratched; multiple units have torn gaskets; racks next to reach-in unit in the dish area are rusty; found gaps greater than one inch on hood filters on cook line; observed debris on outside of pans and outside of grinder and blenders on prep table in the dish area; observed grease on hood system under filters and under fryers; observed debris under oven doors on the cook line and in the dish area (racks, can opener holder, shelves and fan); kitchen handsink is slow to drain; several ceiling tiles in the employee restrooms are cracked; observed chipping paint on kitchen wall and cracked floor tiles in the dining area; walls above and below two-compartment sink are stained; observed slight debris on cook line floor.
