Subway Sandwiches & Salads, 4014 Glenside Drive – The following violations were reported during a routine inspection: observed an insect control device improperly installed above the back prep table where food is prepared; floor under the back reach-in unit was littered with food debris; walls in the back kitchen have splatter/staining near the dish area and prep table.

Saint Mary’s Catholic School, 9501 Gayton Road – The following violations were reported during a routine inspection: observed uncovered pizza slice boxes; observed a crack in tile wall.

Lanay Café, 2810 N. Parham Road – The following violations were reported during a routine inspection: observed raw eggs stored improperly over tortilla in reach-in unit; debris found in mixing bowl stored clean; observed clean equipment stored on top of rusty side of three-compartment sink in dish area; chicken salad, tuna salad, raw eggs and raw burgers in the kitchen’s low boy unit have elevated temperatures; observed heavily scratched cutting boards on front line; observed torn gasket in low boy unit; air temperature of low boy unit was elevated; observed debris/stains on shelves under the handsink, on slicer, in between equipment on the cook line, outside the microwave and on toaster oven next to the handsink; debris also found outside and under the grill and fryers, on hood filters, floors under equipment, pipes on the cook line, under coffee machine in the dining area and outside of equipment on a clean rack in the dish area; observed leak at the faucet at three-compartment sink; observed stains on walls in the dish area and behind the sandwich unit and slicer; observed chipping paint on walls throughout the kitchen; observed low grouting on floors in the dish area.

Pin Kan Thai Restaurant, 3402 Pump Road – The following violations were reported during a routine inspection: no chlorine is being dispensed into the dishmachine; can opener is dirty; observed tofu overfilled in the make table; cooked tofu and pork have elevated temperatures and were not cooled properly; observed non-food grade materials being used for oil and fried foods; cart outside the walk-in, onion shelf, hood filters and exterior of chest freezers need cleaning; floor along the cook line wall is greasy.

Phoenix Garden Vegetarian Restaurant, 7103 Brook Road – The following violations were reported during a routine inspection: the facility does not have sanitizing strips; surface and handle of reach-in unit are soiled; observed hose attachment under continuous pressure at mopsink with an inadequate backflow device; observed paint peeling from the ceiling in the walk-in freezer.

SB’s Lakeside Loveshack, 6935-37 Lakeside Avenue – The following violations were reported during a routine inspection: observed pink mold on ice machine shield; cheese in the prep unit has an elevated temperature; observed working containers of flour and sugar without labels.

Tony’s Italian Restaurant, 3409 Cox Road – No violations reported during a follow-up inspection.

China Star, 3423 Cox Road – The following violations were reported during a follow-up inspection: facility does not have a certified food protection manager; large wok is not smooth or cleanable; observed debris inside the GE freezer; pipes under the cook line and floor drain under the cook line have black grease build-up; pipes to the three-compartment sink have build-up.

Lebanese Bakery LLC, 6215 W. Broad Street – The following violations were reported during a routine inspection: observed raw ground beef mixture stored incorrectly over grape leaves in the reach-in unit; grape leaves were not cooled properly and had an elevated temperature; observed multiple small flies on wall in the dish area; cutting boards in the back kitchen and dish area are stained; observed torn gasket and rusty racks in the reach-in unit in back kitchen; observed debris on racks used to cook pita chips, behind skewer machine, outside of grinder and fryer on the prep table and inside the oven on the cook line; ceiling above the mopsink has a hole.

Ci Ci’s Pizza, 7516 W. Broad Street – The following violation was reported during a follow-up inspection: buffet line is kept on a four-hour time control, but times are not marked.

Chadar Thai LLC, 8030 W. Broad Street – The following violations were reported during a follow-up inspection: soda nozzles at the bar are dirty; observed dead roaches on the bar floor; observed a bag of potatoes and a bin of onions on a rack that is less than six inches off the walk-in floor; rice scoops are stored in standing water; observed soiled cardboard on a walk-in shelf; exterior of storage units and gaskets/interiors of other refrigeration units along the pass-thru line need cleaning; some surfaces are not smooth and easily cleanable; wall behind the water heater and corners of the bar floor are dirty; light shields are missing over the back storage line.

Shawarma Bistro, 12422 Gayton Road – The following violations were reported during a routine inspection: raw chicken stored incorrectly over raw beef and raw ground beef stored incorrectly over lamb (store according to cooking temps); yeast was unlabeled; observed a cardboard box being used to store wrapped utensils instead of a cleanable container.
