A look at how some Henrico restaurants fared on their most recent health department inspections.

AUG. 27

Live Crawfish & Seafood, 9020 W. Broad Street – The following violations were reported during a routine inspection: observed an employee’s open bottle of soda in the prep unit and an employee’s open drink and bowl of rice on the prep top; all three handsinks in the kitchen area are missing soap; observed plastic container and a piece of wood on top of the handsink next to cookline; observed garlic and oil, sausage and crawfish in reach-in at elevated temperatures; observed potatoes cooling on the floor uncovered.

Wok N Roll, 7514 W. Broad Street – The following violations were reported during a routine inspection: observed debris on utensils stored in cambros throughout the kitchen; front handsink was blocked by a storage cart; observed soiled cardboard lining shelves throughout the kitchen instead of a durable cleanable material; non-food contact surfaces (cookline equipment exteriors, inside of refrigeration units, gaskets, racks, etc.) have reside buildup; walk-in fan is dusty; the walk-in freezer’s drain pipe from condenser is leaking; three-compartment sink’s floor drain has food debris in it; observed grease dripping on the right side of the hood.

Capital Ale House, 4024-A Cox Road – The following violations were reported during a routine inspection: shellstock tags are not being labeled with date the product was last served; observed cheese and portioned turkey in bags in the top prep unit at elevated temperatures because they were stacked too high; observed about 10 live roaches in dish, break room and beverage station areas; gaskets of salad unit are torn; walk-in freezer door does not shut tightly so there is ice buildup; fryer basket is damaged; racks of salad prep unit are rusty; observed moldy caulking in dish area; grout has eroded and water is pooling at main bar; observed a hole in the wall near cabinet; cover for return air intake is missing from the ceiling; observed soiled floor drains, especially at the bar; black mold/mildew found on tiles and below bar refrigerator due to heavy condensation; observed water/food debris/mildew in gaskets of prep refrigerators on the cookline; ceiling around vent is dusty.

World of Beer, 11600 W. Broad Street – No violations reported during a routine inspection.

AUG. 31

Tip Thai, 8131 Staples Mill Road – The following violations were reported during a routine inspection: chemical dish washer is not sanitizing because the sanitizer dispensing tube is broken; observed rice scoops in standing water; observed a piece of unsealed wood attached to the dish rack; cutting boards at the prep units and on the dish rack have deep scratches; metal racks in the walk-in cooler are rusty; observed a leak under the dish area handsink; ceiling tiles in the kitchen are discolored and the dish area wall has chipped/cracking paint; observed dirty oil behind the fryer.

MOD Pizza, 8671 Quioccasin Road – The following violations were reported during a routine inspection: observed pink mold on back top portion of ice chute in the dining area; observed debris greater than one day old on utensils and food contact surfaces in container on clean rack in the back kitchen; observed spinach, lettuce, tomatoes, ham and mozzarella on the line and in low boy unit have elevated temperatures; spinach should be stored in a deep metal pan; observed salt and sugar containers without a label on rack near prep table; observed torn gasket in low boy unit on the line and cracks in plastic lids in container on clean rack in the dish area; debris found on the outside of multiple equipment, on a shelf under the prep table, on shelves in front of house, and on top the dish machine; observed dust on fan guards in walk-in cooler; debris found on floor of walk-in freezer and on the wall and floor under the three-compartment sink; debris found on floor under equipment in front of house.

Chipotle Mexican Grill, 8675 Quioccasin Road – The following violations were reported during a routine inspection: observed multiple small flies on onions in the prep sink; cutting board on clean rack next to three-compartment sink is heavily sratched; observed mop in the back of the facility not hung to dry; observed dust on fan guards in walk-in cooler.

Subway, 9105 Quioccasin Road – The following violations were reported during a routine inspection: observed veggie patty and cheese with elevated temperatures and not stored properly; observed multiple dead roaches under the reach-in in back kitchen and under a table in back of facility; quaternary ammonium and chlorine sanitizer levels in sanibuckets are incorrect; found wet nesting of metal pans above the three-compartment sink in back of the facility; rack where single-service items are stored next to the reach-in freezer are dusty; observed debris on the bottom of the reach-in freezer and in floor corners of the dry storage room.

SEPT. 1

Chop’t Creative Salad, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: observed an employee placing lids for the salad mixer on top of the handsink (handsink is for handwashing only); final rinse temperature for high temperature dish machine was slightly elevated; observed debris on utensils and food contact surfaces that were hanging above the three-compartment sink; observed chicken in temporary holding refrigeration unit, sweet potatoes at three-compartment sink and falafel on the line had elevated temperatures; observed high pH levels for veggie wash at prep sink next to the walk-in cooler; observed wet nesting of metal pans in the prep area; single-service lids are not being stored inverted; proper cooling methods were not used for coleslaw, artichoke and salads; observed torn gasket on low boy unit on the front line; observed chipped corners of plastic lexan pan in front display case; debris found on floors and walls throughout the facility; fan guards in walk-in cooler are dusty; handsink in prep area is slow to drain and there is a leak under the three-compartment sink; many areas in the kitchen need cleaning including racks in walk-in cooler and prep areas, shelves throughout the facility, outside of equipment in the prep area, front display case, top of dish machine, vent above the dish machine and in between gaskets on front low boy unit.

Red Robin America’s Gourmet Burgers & Spirits, 11784 W. Broad Street – The following violations were reported during a routine inspection: raw burgers in the bottom of refrigerator unit on the cook line had an elevated temperature; observed grease build-up on fryer wheels and fryer baskets, and food debris and ice build up in several freezer and refrigerator units; observed mold/food debris on refrigerator gaskets and carbon build up on the outside of egg pans; observed some areas of low grout on the floor and cracked/missing tiles on line, by handsink across from burger refrigerator and inside the beer walk-in; lights in the hood system don’t work; freezer door doesn’t shut due to ice build up; observed debris on floor drains in the dish room and at bar; observed dust on ceiling vents.
