A look at how some Henrico restaurants fared on their most recent health department inspections.

JULY 20

Pho 1 Grill, 201 Towne Center West Boulevard, #701 – The following violations were reported during a routine inspection: beef, eggrolls and tofu in prep refrigerator were found at elevated temperatures because the door had not been tightly shut overnight; observed rusty racks in the walk-in cooler.

Applebee’s Neighborhood Grill & Bar, 4306 S. Laburnum Avenue – The following violations were reported during a routine inspection: handsink by the dish machine was missing paper towels; observed a heavy build-up of grease deposits in chip warming bin; cheese ravioli, lettuce and prime rib in walk-in found at elevated temperatures; observed multiple flies throughout restaurant and dead flies on hanging traps; observed torn gaskets on units and drawers; prep units across from cook line have heavy ice build-up causing large amounts of water to leak onto the floor; observed build-up of grime on gaskets and handles of units; observed grease build-up on equipment and on racks above prep units along the cook line; build-up of trash and old food under equipment is clogging drains in the kitchen; observed build-up of grease on floor and along wall of cook line; observed food splatter on walls and dust on ceiling tiles; low grouting found throughout the back kitchen, particularly in the dish area and cook line; cracked tiles and missing floor tiles observed in the walk-in; observed missing coving tiles throughout the facility; ceiling tiles are peeling away above the three-compartment sink.

JULY 21

Chianti Italian Restaurant & Pizzeria, 1304 Gaskins Road – No violations reported during a follow-up inspection.

Anant Cuisine of India, 4348 Pouncey Tract Road – The following violations were reported during a routine inspection: observed torn gaskets in lowboy refrigerator on the cook line; observed rusty racks in the walk-in; spray hose by dish machine hangs below flood rim level of the sink; found caulking moldy and in poor repair at mopsink.

Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a routine inspection: sausage and tomatoes in prep unit found at elevated temperatures; no clear time control policy is in place for food items on the prep unit; observed many drain flies and one dead cockroach in the kitchen; front prep unit is not working properly; cold water is unavailable at three-compartment sink due to a leak in hot water supply valve; floors throughout the facility need cleaning, especially under cook line and in the mopsink area; observed loose and broken tiles in the entryway; bulbs in the food prep area are unprotected; hood vent system needs cleaning.

Market Café, 10950 Nuckols Road – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; no paper towels are available at back dry storage area handsink; raw eggs stored incorrectly in the walk-in; observed black moldy build-up inside the ice machine; biryani and curries were cooled incorrectly; unsalted butter and cut tomato in the make table were found at elevated temperatures; pickled peppers in the walk-in did not have a date mark; observed a bowl of Borax on a low shelf at the prep area (this is a prohibited tracking powder and may not be present in the establishment); observed three roaches of varying sizes at the dish area; a scraper/cleaning tool used for washing dishes was not stored correctly; lights in the walk-in freezer are burned out; front make table isn’t working properly; observed debris on metal racks throughout the kitchen; observed a leak at the air vents in the kitchen; dish area wall and walk-in freezer floor need cleaning.

Tropical Smoothie, 9972 Brook Road – The following violations were reported during a routine inspection: observed a heavy ice build-up in walk-in freezer; floor drains need cleaning.

JULY 22

Pulp On Lakeside LLC, 5411 Lakeside Avenue – No violations reported during a routine inspection.

Eat 33, 6901 Staples Mill Road – No violations reported during a follow-up inspection.

Dugout Bar & Grille, 1510 Mountain Road – The following violations were reported during a routine inspection: cheese and tomato in the prep unit were found at elevated temperatures; no consumer advisory for burgers is on the menu; observed leak from dump sink drain; no concrete pad is under the dumpster; observed hood filters in need of cleaning.

JULY 23

Rapid Pizza, 1592 Mountain Road – The following violations were reported during a routine inspection: observed marinara being reheated incorrectly; observed sliced tomato and chopped romaine without date marks.

Casa Grande, 3532 Pump Road – The following violations were reported during a follow-up inspection: observed rust on lower shelf of the work table; handsink faucet is deteriorating and can no longer be cleaned.

JULY 24

Wendy’s Old Fashioned Hamburgers, 9116 W. Broad Street – The following violations were reported during a routine inspection: sausage, lettuce and tomatoes in prep top by the drive-thru were found at elevated temperatures; observed sanitizer buckets on the cook line at 100 ppm of quat sanitzer.

O’Charley’s Restaurant, 9927 Mayland Drive – No violations reported during a follow-up inspection.

Mellow Mushroom, 12171 W. Broad Street – No violations reported during a follow-up inspection.
