Sistar Seafood, 4844 S. Laburnum Avenue – The following violations were reported during a routine inspection: observed boxes of seafood improperly stored on the floor of the walk-in and freezer; observed heavily scratched cutting boards.

Casa Italiana, 8801 Three Chopt Road – The following violations were reported during a routine inspection: observed uninverted single service to-go trays on rack next to the walk-in closet; found heavily scratched cutting boards on shelf in dish area and on cook line; observed debris on fryer baskets on cook line. JULY 9

Casa Grande Restaurant, 10120 Brook Road – The following violations were reported during a routine inspection: no hand soap available at front handsink; observed tomato and cheese on line at elevated temperatures; observed multiple prepared foods in the walk-in closet with no date marks; found adoboda past its expiration date in walk-in closet; fan grates above the reach-in need cleaning.

Gino’s Ristorante Pizzeria, 10118 Brook Road – The following violations were reported during a routine inspection: found can opener blade in need of cleaning; observed broken handle and floor in disrepair at walk-in closet.

JJ’s Grille, 10298 Staples Mill Road – The following violations were reported during a routine inspection: observed old build-up on can opener blade; observed black beans, roasted corn, tomatoes and cheeses at elevated temperatures; found chili and pork in the walk-in past their expiration dates; gaskets in low boy across from the stove are torn; prep unit drawers across from the cook line freezer are not holding foods at the proper temperature; wall behind the dish machine is dirty.

Bryan Park Bar and Grill, 5516 Lakeside Avenue – The following violations were reported during a routine inspection: observed raw shell eggs stored improperly; found cooked noodles past their expiration date; no chlorine test strips were present.

Nuevo Mexico Restaurante, 10282 Brook Road – The following violation was reported during a follow-up inspection: consumer advisory needed for steak a la Mexicana if it can be ordered medium or rare.

Subway Sandwiches & Salads, 9621 W Broad Street – No violations reported during a follow-up inspection.

JULY 10

Plaza Azteca Mexican Restaurant, 10099 Brook Road – The following violations were reported during a routine inspection: observed food debris in front handsink; hand towels needed at front handsink; observed front handsink slow to drain; no handwash signage in all three restrooms; observed tomatoes on the guac cart at an elevated temperature; observed drain flies in the bar area and live cockroaches in the kitchen; observed walk-in freezer in need of defrost and a broken thermostat on warmer; found low grout and damaged tilework throughout the kitchen; hood system is missing a grease catcher.

Cheesecake Factory, 11800 W. Broad Street – The following violations were reported during a routine inspection: person in charge was unable to provide parasite control documentation from salmon supplier; observed mold on the top interior surface of the ice machine; prepared potatoes in the walk-in refrigerator were not cooled properly; tamale patties, tamale filling, raw beef and raw chicken in the refrigerated drawers on the cook line were at elevated temperatures; pans of prepared mac and cheese bite mix in the walk-in refrigerator were not cooled properly; observed chipped floor tiles on the cook line; observed worn grout at mop sink and throughout the kitchen; observed mold in the caulking in dish area.

McDonald’s, 4708 Williamsburg Road – The following violations were reported during a routine inspection: observed a build-up of pink and black mildew on shield of ice machine and on black plate of smoothie machine; lettuce and ham in the prep unit by the dish area were at elevated temperatures; did not observe proper written time on items being held on time and not temperature (lettuce, tomatoes and cheese); observed wet wiping cloths on prep surfaces; found build-up of food debris and grime on gaskets throughout the facility and racks in the walk-in.

Burger King, 3720 Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed dried milk product on and inside the blender container; observed build-up of food and debris throughout the walk-in, kitchen and inside reach-in coolers and freezers; observed food splatter on walls and dust build-up on vents and ceiling tiles; food debris and dirt found on floors and under/behind equipment.

JULY 13

Zaxby’s, 11350 Nuckols Road – The following violation was reported during a routine inspection: observed some resides on the exteriors of equipment.

Chen’s Chinese Restaurant, 11361 Nuckols Road – The following violations were reported during a routine inspection: carton observed in handsink (handsinks are only for handwashing); raw egg yolks and raw beef skewers in low boy and raw chicken in the walk-in were stored incorrectly; observed crumbs and debris on plates stored in the warmer and on dishes stored along shelves in the kitchen; observed build-up in tea nozzles at the server station, on a can opener blade and in the soda gun at front counter; water that is hot held to hold rice scoops was at an improper temperature; observed rice noodles left out past an hour after prepping; all raw meats in the meat prep top and beaten eggs were found at elevated temperatures; dumplings were stored incorrectly in the prep unit; fried chicken pieces and white rice are kept on a time control at the cook line but no marked times were observed; observed chicken left sitting in the prep area after having been cut and wrapped in plastic; noodles in the low boy on the cook line were improperly cooled; observed boxes of food incorrectly stored on the walk-in freezer floor; observed bins of fresh prepped raw meats and veggies incorrectly stored on the kitchen floor; observed meat make table unit on the cook line at an improper temperature; cutting boards found with deep scratches and stains; racks throughout the kitchen, exteriors of equipment and inside the microwave need cleaning; observed dry spills in the bottom of the cooler at front bar; floor and pipes under the cook line and ceiling tiles need cleaning; hood vent system is dripping with grease icicles and needs cleaning.
