MARCH 21

Surrounding Counties, 7019 Three Chopt Road – The following violations were reported during a routine inspection: temperatures of milk, oat milk, almond milk and half-and-half in both small beverage refrigerators are elevated; there are no thermometers inside small refrigerators used for beverages.

Capriccio Pizza, 9127-A West Broad Street – The following violations were reported during a follow-up inspection: open staff coffee cup found on food prep table; raw ground beef and bacon stored above cooked pasta and bread in reach-in cooler; temperatures of two containers of pasta and a large bucket of tomato sauce made yesterday are elevated because of improper cooling methods; temperatures of shredded cheese, tomato sauce and salami in walk-in cooler and temperatures of cheese slices and shredded cheese in reach-in cooler are elevated; there are no date marks on tomato sauce, cheese or meatballs.

Fat Boy’s Bar + Grill, 2 West Williamsburg Road – The following violations were reported during a routine inspection: dried food debris found on some strainers and utensils; observed pink residue inside ice machine; squeeze bottles of water and oil and bulk bins are unlabeled; a metal bowl without handles is stored in bulk corn flour bin; in-use utensils are stored with handles touching food; observed grease and food debris between cook line equipment and on fryer and grill; there is flour all over the outside of bulk bins; dried food found inside all cold holding units; fryer baskets are encrusted with grease; observed food debris and grime behind cook line and under storage shelves; ceiling tiles are stained.

9 Mile Grill, 309 West Nine Mile Road – The following violations were reported during a routine inspection: employee garnished a beverage with a lemon wedge bare handed; raw bacon stored over produce in walk-in cooler; pink mold found inside ice machine; dried food debris found on can opener blade and inside microwave; temperatures of cheese and tomato in reach-in cooler are elevated; pesticide is stored in the kitchen; spray cleaner bottles of Windex are unlabeled; facility doesn’t have a food thermometer on site; observed dead pests in trap in corner of dry storage area; food is stored directly on walk-in cooler and freezer floors; spoon is stored in ambient temperature water between uses; cutting board is scored and stained; several items/areas are dirty (reach-in cooler door exteriors and door gaskets, storage shelves in kitchen, wall behind hot line, hand sink in women’s restroom); observed grease buildup on sides of hot line equipment; walk-in cooler wire racks are dirty and moldy; walk-in cooler fan guards are dusty and moldy; there are food stains on walls throughout kitchen; observed food debris, trash and grease residue on floor under hot line; hand sink in kitchen has black residue on it.

Bella Luna, 1212 Concord Avenue – The following violations were reported during a routine inspection: a broken reach-in freezer door is being held shut by heavy boxes; there is ice buildup inside reach-in freezer and mold growing on the door frame; observed heavy grease buildup on side of fryer; there are cracked floor tiles throughout the kitchen; there is an accumulation of unused items in the kitchen, including dishes with heavy dust buildup.

Chicken Fiesta, 8211 Brook Road – The following violations were reported during a routine inspection: neither hand sink has handwash signage posted; hand sink by dish room doesn’t have paper towels; raw marinating chicken is stored above ready-to-eat chicken in walk-in cooler; dried food debris and residue found on and inside the microwave; residue and plastic pieces found on can opener; can of Raid is stored in dry storage; multiple food items are stored directly on walk-in cooler floor; cardboard is being used as a shelf liner on prep table in kitchen; reach-in unit has torn gaskets; there is rusted and chipping paint on racks in dry storage and walk-in cooler; observed heavy dust buildup on ceiling tiles throughout kitchen.

Starbucks Coffee Company, 11800 West Broad Street – The following violation was reported during a routine inspection: hand sink in back of kitchen doesn’t have hand soap.

Jersey Mike’s Sub Shop, 11444 West Broad Street – The following violations were reported during a routine inspection: large container of tuna fish salad in walk-in cooler isn’t date marked; box of bread is stored directly on walk-in freezer floor; gasket liners are torn on lowboy in back kitchen and walk-in cooler door; wire racks in walk-in cooler are peeling; vent in walk-in cooler is covered in a layer of debris; ingredient shelf is covered in a layer of grease and debris; drying rack is covered in a layer of slimy debris; there is a direct plumbing connection under three-compartment sink leading into the sewage line; dumpster area is dirty and surrounded by loose trash and debris; light shield is broken above door leading into back kitchen.

Sheetz, 8711 Quioccasin Road – No violations reported during a routine inspection.

Pizza Hut, 9020-B Quioccasin Road – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; observed dried sauce left from last night on pizza ring hanging on make line.

Papa John’s Pizza, 3067 Lauderdale Drive – The following violations were reported during a routine inspection: temperatures of wings, philly steaks and cheese sticks under make table are elevated; temperatures of pepperoni, sausage, ham, cheese sticks and tomatoes in reach-in unit are elevated; bulk container of flour is unlabeled; gasket liners are torn on middle-right door of make table and reach-in unit; there is a direct plumbing connection underneath three-compartment sink that goes into the sewage line; mop by utility sink isn’t hung or inverted to air dry.

Jersey Mike’s Sub Shop, 7000 Forest Avenue – The following violations were reported during a routine inspection: no handwash signage is posted at either hand wash sink; hand sanitizer, equipment spray cleaner and employee hat and apron are stored next to open ready-to-eat lettuce on prep table; shelf liner on front cook line is broken; there are heavy stains and scratches on hanging cutting boards; observed heavy grease residue on side of fryer and splashes on reach-in cooler.

Panda Express, 4501 South Laburnum Avenue – The following violation was reported during a routine inspection: hand sink doesn’t have paper towels.

Christian’s Pizza, 7003-A Three Chopt Road – The following violations were reported during a routine inspection: there isn’t water or water pressure at front sink; observed mildew buildup on shield of ice machine; time isn’t being tracked for pizza sold by the slice; wet wiping cloths are sitting out on prep tables; sanitizer buckets are slightly high in concentration; there are torn gaskets and a missing kick plate on reach-in; light shields are missing on lights in kitchen; racks inside units, gaskets, hand sinks and cooking equipment need cleaning; observed debris on floors, walls and ceilings throughout kitchen; there is mud on the floor of the store room.

Tavern at Triangle Park, 7110-F Patterson Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; observed open drinks on prep table, lowboy and storage shelf; temperature of rice in hot holding unit is too low; temperatures of pasta, chicken and shrimp in lowboy are elevated; temperatures of tomatoes, deli meat and lettuce in units are elevated because they are overstacked; multiple spray bottles are unlabeled; medicine is stored on prep shelf with to-go containers and other dishes; oysters are stored in plastic cups in lowboy; food is stored directly on walk-in floor; items are uncovered in the walk-in; knives are stored in between prep table; cutting boards throughout are heavily stained; racks in units and in storage, floor under units and wall along cook line need cleaning; observed heavy grease deposits on cook line equipment; observed grime buildup on outside of units.

The Local at Forest & Ridge, 403-B North Ridge Road – The following violations were reported during a routine inspection: metal wires on fryer baskets are bent and breaking; cement floor strip in front of dish machine isn’t sealed; shield is missing from light fixture above dish machine.

Jimmy John’s, 4420 South Laburnum Avenue – The following violation was reported during a routine inspection: back hand sink doesn’t have hand soap.

Subway, 3135 Mechanicsville Turnpike – The following violations were reported during a follow-up inspection: dried food debris found inside microwave and on reach-in cooler door gaskets; observed mold in ice chute of soda machine; pest droppings found on non-food contact surfaces, underneath soda machine, and behind oven and proofer; employee is wearing a bracelet; there is a leak in plumbing underneath three-compartment sink; vent above oven is dusty; there are dried food stains on walls throughout kitchen.

Ay Mama, 9809 West Broad Street – The following violations were reported during a routine inspection: chlorine concentration at dish machine is 0 ppm; temperatures of salsa and queso in reach-in cooler are elevated; air temperature of this unit is elevated; menu doesn’t contain a consumer advisory statement for steaks that can be cooked to order; ground beef is thawing in prep sink without cold running water; employees are wearing bracelets; make table cooler gasket is torn; make table cooler gaskets are dirty; ice machine is dusty; y-connector at mop sink doesn’t have the proper backflow prevention device installed.

Yellow Umbrella Provisions, 5001 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: employee beverage doesn’t have a lid; food debris found on meat slicer blade and base; temperatures of Swiss cheese, turkey and cheddar cheese in reach-in coolers are elevated because the items are double panned; a consumer advisory isn’t posted for raw sushi items for retail sale; temperature of carnitas in top portion of reach-in cooler is elevated because of improper cooling methods (double panned in top portion); bulk container of sugar is unlabeled; employee personal bag and food are stored in reach-in cooler amongst food items.
