Ugly Dumpling, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: several employee drinks were left out on prep surfaces and next to clean utensils; dishes stored clean still have dried food debris on them; temperature of beef and noodles in hot holding unit is too low; temperatures of noodles and greens in walk-in cooler, flat noodles in prep unit and ugly butter out on counter are elevated; several wiping buckets are stored on the ground or prep surfaces; temperature of baby ribs is elevated because of improper cooling methods (stored in a covered, deep container); several tong handles are touching food on expo line; several plastic and metal containers are stacked while still wet; clean utensil is in a bucket with excess water; observed dried food debris and grime on floor under expo line; observed grease buildup on hood vents.
Las Palmas Mexican Restaurant, 8415 West Broad Street – The following violations were reported during a routine inspection: hand sink near back door is blocked with a jug of dish soap; hand sink near back door and staff bathroom are missing paper towels; hand sink near dish machine doesn’t have hand soap; raw oysters in walk-in cooler do not have tags; didn’t observe last date served on shellstock tags; raw shell eggs stored over prepped lettuce in walk-in cooler; food debris found on slicer; temperatures of pico de gallo, shredded cheese, lettuce, milk, green salsa, red salsas, raw chicken and pork belly in refrigerated units are elevated; salsas in walk-in cooler are past their expiration dates; ice buckets are not inverted to air dry; gasket is torn and racks are rusted in juice reach-in cooler; observed grease buildup on sides of fryer and on grill hood; there are cracked floor tiles near dishwashing area.
PITSA, 11321-B Nuckols Road – The following violations were reported during a routine inspection: person in charge is not performing food safety duties (many issues with temperature control, improper thawing and improper cooling); hand washing doesn’t appear to be a priority as blocked hand sink is “not used” and the other hand sink doesn’t have hand soap; person in charge couldn’t provide written procedures for cleaning up vomit or diarrhea; an employee touched pickled vegetable salad with bare hands; there is heavy food splatter in microwaves; observed residue inside ice machine; raw shell eggs were left out on counter; observed improper temperatures for fresh garlic in butter, two pans of rice and raw chicken; raw chicken is thawing in bowl of stagnant water in three-compartment sink; there are no date marks on prepared TCS foods; oven cleaner and two spray bottles of cleaning chemicals are stored next to or above food; gravy made this morning is sitting out on the counter; pickled vegetable salad prepped today is out of the refrigerator (employee states it has to stay out one hour to ferment); one reach-in refrigerator isn’t working (there is an active work order); observed heavy ice buildup in refrigerated units in prep area; heavy food debris found on non-food contact surfaces, such as shelving, throughout kitchen; observed food splatter on walls throughout kitchen.

The Place at Innsbrook, 4036-C Cox Road – The following violations were reported during a routine inspection: dishwasher isn’t reaching the required final surface temperature; container of salsa in walk-in cooler is past its expiration date; gasket liner is torn on standing dessert refrigerator; walk-in cooler wire racks are peeling; top prep shelf storing entrée plates is covered in a layer of black debris; there is a direct plumbing connection into the sewage line under the three-compartment sink; dumpster area is dirty and surrounded by loose trash and debris; ceiling vents above the prep tables are covered in a thin layer of dust.
Zen Café, 9041 West Broad Street – The following violations were reported during a routine inspection: observed buildup on reusable containers in back kitchen; time isn’t marked on boba and did not observe a time control policy; boba scoop is stored in ambient temperature water; make table cutting board is scratched; gasket is torn on make table across from two-door freezer; there is a leak at three-compartment sink faucet.
Pupuseria Ena, 9115 Quioccasin Road – The following violations were reported during a routine inspection: spray bottle of degreaser under three-compartment sink is unlabeled; shelf above prep unit holding take-out containers is dirty; gasket liner is torn on large standing freezer; fryer baskets have broken metal pieces.
Portabella’s Restaurant, 2627 New Market Road – The following violations were reported during a routine inspection: food debris found on can opener; pasta made yesterday, lasagna and pasta rolls are not date marked; in-use knives with handles are on top of food; baked bread is wrapped in non-food grade black plastic bags; some ceiling tiles in the kitchen are stained.

The Dominion Club, 6000 Dominion Club Drive – The following violations were reported during a routine inspection: bar hand sink is missing a handwash sign; food splatter found in microwave; temperature of chili in steam table is too low because of improper reheating procedures; temperatures of chicken and sausage in altosham are too low; temperatures of hummus and squeeze bottle of heavy cream and sugar mixture (used for key lime pie) in make table are elevated; make table isn’t maintaining foods at the proper temperature; tomato basil soup and marinara are past their expiration dates; observed an inconsistent date marking system (some foods are marked with prep date and some foods are marked with discard date); menu is missing disclosure for foods that may be served raw or undercooked; three chemical spray bottles in kitchen and bar are unlabeled; observed many foods vacuum packaged including raw meat, raw shrimp, salmon, whole branzino fish and greens, but the HACCP plan couldn’t be provided and no one is familiar with what is in the plan; vacuum packaged fish, both commercial and in-house packaging, was found thawed in the walk-in (tuna, mahi and salmon); three employees are wearing bracelets while preparing food; a couple of food service employees need beard guards; seal inside the ice machine is peeling away with a black residue; there is ice buildup in outdoor freezer (there is an active work order); observed dust on ceiling and wall near pass-thru window; observed mold on the wall and caulking in dish pit.
Domoishi / Alohahola, 1080 Virginia Center Parkway – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; employee’s snack/drink is stored on counter next to food prep and sneeze guard; raw chicken stored over ready-to-eat seaweed salad and sushi ginger in walk-in cooler; raw eggs stored over onions and celery; mold and residue are forming on main ice machine; heavy dried on food debris found on two slicers and can opener; observed pre-cooked chicken wings sitting on a shelf by the fryer with no time control policy and elevated temperature; untreated raw eggs are sitting out by grill line; can of Raid is stored in kitchen; multiple spray bottles with yellow liquid are unlabeled; observed pans of chicken wings thawing on shelf by walk-in cooler with plastic wrap covering; containers of corn and lima beans are thawing at ambient temperature; thermometer is inaccurate; multiple bulk containers of white ingredients are unlabeled; observed small flies throughout kitchen and front counter area; multiple food items are stored directly on walk-in cooler and walk-in freezer floors; sauce cups are stored inside reused raw chicken boxes; two employees in contact with food are wearing bracelets; cardboard is being used as a shelf liner; original stainless steel sticker is still on a shelf; microwave interior is warped and rusted; walk-in cooler racks are heavily rusted; person in charge couldn’t provide sanitizer test strips; observed heavy dried food debris on gaskets of every unit; dust and food debris found on shelves above food prep areas; hose with spray nozzle is attached to mop sink faucet without a backflow prevention device installed; lid was left open on grease bin outside of facility.
Moe’s Southwest Grill, 10839 West Broad Street – The following violations were reported during a routine inspection: several food items (rice on serving line, diced chicken in bottom warmer, rice in warmer) were found at improper hot holding temperatures; employee working with food isn’t wearing a beard guard; wire racks in walk-in cooler are peeling; y-connector on mop sink faucet doesn’t have a dual check valve installed; mop isn’t hung or inverted to air dry.

Buckhead’s Chop House & Restaurant, 8510 Patterson Avenue – The following violations were reported during a routine inspection: employee is prepping garnishes with bare hands; observed buildup on soda nozzle at server station; pickled veggies are past their expiration date; ceiling tiles on cook line and in storage area are dirty and stained; observed grease on hood filters and suppression pipes.

Subway, 9621 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; two slicers stored “clean” at three-compartment sink still have dried food debris on them; both walk-in gaskets are torn; racks are rusted and dirty in both walk-in units; air curtains in walk-in cooler are broken; walk-in cooler floor is dirty.

Bocata Arepa Bar, 10170 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; black beans and bulk soap container found in hand sinks; raw shell eggs are stored over bread and ready-to-eat sauce in reach-in and lowboy units; garlic cilantro sauce and other items are not date marked; menu doesn’t contain a consumer advisory for runny egg; bulk containers of flour, sugar and salt are unlabeled; observed a live roach crawling on clean dishes in three-compartment sink.
Corner Bakery Café, 11000 West Broad Street – The following violations were reported during a routine inspection: shelf storing take-out containers for catering is covered in a layer of grease buildup; gasket liners are torn on second prep unit on line and two lowboy units.
Maple Street Biscuit Company, 11657 West Broad Street – The following violations were reported during a routine inspection: temperature of grits on hot holding line is too low; top prep shelf storing take-out containers is covered in a thin layer of dust buildup; two mops are not hung or inverted to air dry.
Plaza Azteca Mexican Restaurant, 12020 West Broad Street – The following violations were reported during a routine inspection: raw chicken and ground beef in portioned bags are stored above ready-to-eat produce in first prep unit on the line; cooked seafood in walk-in cooler is past its expiration date; menu doesn’t contain an asterisk linking Chori steak to the consumer advisory statement; spray bottle of Windex is unlabeled; bulk bins of salt and flour are unlabeled; prep shelf storing take-out containers is covered in a layer of debris buildup; prep unit at the end of the line on the right side of the kitchen is leaking and pooling water at the bottom; drying shelf next to dishwasher is covered in a layer of debris buildup; dumpster area is dirty and surrounded by loose trash and debris; mop in utilities area isn’t hung or inverted to air dry.
Perlas Pizza, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: spray bottle containing oven cleaner is unlabeled; raw chicken is stored in a non-food grade grocery bag in back lowboy on prep line; employee cooking in the kitchen isn’t wearing a beard guard; cardboard is being used as a liner on wire racks storing dry ingredients and take-out containers; gasket liner on walk-in cooler door is torn; walk-in wire racks are rusty; dumpster area is dirty and surrounded by loose trash and debris.
