Canaima Venezuelan Food, 2755 Hungary Spring Road – The following violations were reported during a routine inspection: single-use items found in hand sink; handle is missing on reach-in cooler; gaskets on reach-in cooler are heavily torn; observed heavy grease buildup on hoods of cook line.

Boychik’s Deli, 4024-B Cox Road – The following violations were reported during a routine inspection: dishwasher is dispensing 0 ppm of chloride sanitizer; temperature of grits in warmer is too low; both gasket liners are torn on lowboy doors on prep line and on lowboy at the end of the cook line; gasket liner is torn on right-side door of standing reach-in two-door freezer; fryer baskets have broken metal pieces; long cutting boards on prep line are scored and stained; walk-in cooler vents are dusty; dumpster area is surrounded by loose trash and debris; ceiling vents throughout kitchen are covered in dust; ceiling panels in kitchen above walk-in cooler are peeling.
Subway, 73 South Airport Drive – The following violations were reported during a routine inspection: employee is wearing a bracelet; observed black mold on walk-in cooler and freezer door gaskets; outdoor dumpster doors were left open; observed grease residue on ceiling and walls above bread oven; ceiling tile in kitchen is stained.

Con Salsa Venezuelan Cuisine LLC, 3016-A Mountain Road – The following violations were reported during a routine inspection: dishes found in hand sink; hand sink doesn’t have paper towels; temperatures of pans of pulled pork and pans of black beans prepared yesterday in reach-in cooler are elevated; steam table is not holding food at the proper temperature; spray bottle with pink liquid by front counter is unlabeled; juice bottle doesn’t have a warning label on it; observed heavy food residue and dried food debris on gaskets of lowboy cooler.
Chick-fil-A—Short Pump Town Center, 11800 West Broad Street – The following violations were reported during a routine inspection: used hand towel found in hand sink on prep line; dry ingredient bulk bin container is unlabeled; there are two y-connectors attached to mop sink faucet and chemical pump dispenser without the appropriate backflow prevention device installed.
Greek Cuisine, 11214 Patterson Avenue – The following violations were reported during a routine inspection: person in charge doesn’t have a food protection manager certificate; several food items in reach-in coolers (marinara, pasta, meatball soup, baked macaroni, meatballs and lasagna) have exceeded the six-hour cooling time; dolmas, soups, marinara and baked lasagna are not date marked; large single cutting board is heavily scratched and stained black; gaskets are torn on lowboy cooler near oven; facility couldn’t provide test strips for sanitizer used at three-compartment sink.

Acacia Midtown, 2363 Roux Street – The following violations were reported during a routine inspection: temperature of double-panned cole slaw in reach-in cooler top is elevated; roasted red peppers and tuna in reach-in cooler are past their expiration dates; ice scoop is stored with handle touching ice; observed dried food debris and grease residue on sides of cooking equipment; door gaskets on reach-in freezer are dirty; walk-in cooler fan guards are dusty; outdoor dumpster door was left open.
Chick-fil-A—Willow Lawn, 4920 West Broad Street – The following violations were reported during a routine inspection: observed dust, food debris and powder on clean dish drying rack and walk-in cooler wire racks; grease residue found on sides of chicken deep fryers; outdoor dumpster lid was left open.
New Chinese Chef, 403-A North Ridge Road – The following violations were reported during a routine inspection: sanitizer concentration at three-compartment sink is too low; temperature of fried rice in large container made yesterday in walk-in is elevated; no date marks found on chicken, noodles or egg rolls; small containers of white substances are unlabeled; observed debris on racks in units, walk-in and throughout kitchen; observed grease on cooking equipment; floors and wall are dirty and in disrepair.

The Brass Tap, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels; raw foods on speed rack in walk-in cooler are not stored in hierarchical cooking order; observed syrup stains inside soda gun nozzles; dried food debris found inside microwave; food is stored directly on walk-in freezer floor; kitchen employee is wearing a watch; to-go boxes at the bar are not stored inverted; observed grease buildup on sides of hot line cooking equipment; y-valve at mop sink doesn’t have the proper backflow prevention device installed; there are mildew and dried food debris on walls behind dish machine.
Starbucks Coffee Company, 12241 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t locate the employee health policy; no handwash signage is posted at hand sinks in food prep areas.
Chuy’s, 11229 West Broad Street – The following violations were reported during a routine inspection: hand sink in server wait area doesn’t have paper towels; strainer and used gloves found in hand sink on prep line; observed food residue inside blender and on lid; the facility’s HACCP logs are incomplete and haven’t been completed for several days; metal wires are bent on fryer baskets; there are broken and cracked plastic containers and lids; wooden lodge boards are in disrepair; observed dust and grease buildup on wire racks on cook line, portable fan, sprayer at tilt kettle and tortilla press; there are areas of low grout on floors throughout the kitchen.
Sheetz, 7035 West Broad Street – The following violation was reported during a routine inspection: quat sanitizer concentration at three-compartment sink is too low.
Huang’s Express, Inc., 5462 West Broad Street – The following violations were reported during a routine inspection: dried food debris found inside microwave; can of spray pesticide is stored in the kitchen; powdery, white substances on hot line are unlabeled; boxes of food stored directly on walk-in cooler floor; staff are not wearing hair restraints while handling food; to-go containers and paper plates are not stored inverted; observed grease buildup on sides of hot line cooking equipment; front and back of house reach-in coolers have dirty/moldy door gaskets; walk-in cooler door gaskets are dirty/moldy; three-compartment sink is dirty; outdoor dumpster lid was left open; there are dried food stains on walls throughout kitchen; there are stains on ceiling tile in dining area and kitchen.
Frida’s Mexican Restaurant, 910 East Parham Road – The following violations were reported during a routine inspection: there is black residue on soda nozzle; observed residue buildup on ice machine; there is heavy food buildup and debris on gaskets throughout kitchen; paint is chipping on reach-in cooler racks; there is heavy grease buildup and grease is dripping from hoods of grill onto service plates.
