Zaxby’s, 9110 Staples Mill Road – The following violations were reported during a routine inspection: person in charge states the facility doesn’t have an employee health policy; soda and ice found in register hand sink; observed leak at mop sink; fan grate is broken and the condenser unit is filling with ice in the walk-in freezer; observed grease and grime on floor behind fryer where pan drips; there is grime on floor drains.

New Dragon City, 2220 John Rolfe Parkway – The following violations were reported during a routine inspection: mug of coffee without lid found on prep table near food; temperature of large bowl of General Tso chicken placed on prep table is too low; temperatures of bowl of cooked shrimp in prep unit and bowl of cut cabbage on prep table are elevated; containers of noodles, fried chicken and boiled chicken are in covered, deep bowls during the cooling process.

Cheng Du Chinese Restaurant, 9503-C West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; food employee is chewing gum and eating in the kitchen; food employee washed hands in three-compartment sink instead of hand sink; food employee failed to wash hands when entering the kitchen from outside; hand sink near entrance doesn’t have paper towels; dented can found on storage rack; flour is stored in large, uncovered pots in dish area; can opener blade and slicer are dirty; temperature of chopped cabbage in make table is elevated because the pan is overfilled; no date marks found on cooked chicken neck, Peking duck or tofu; butane fuel is stored next to soy sauce packets and single-service cups; hand sanitizer on cook line is unlabeled; white powders on spice carts and large bins of flour are unlabeled; observed live roaches on floor in dish area and mouse droppings in dry storage; fly traps are full; there are dead roaches in hand sink and in light shield near mop sink; food boxes are stored directly on walk-in freezer floor; food employee is wearing a watch; yellow cutting board in prep room and make table cutting board are heavily scratched; cardboard is used throughout the kitchen as shelf liners and on the floor; fryer baskets are bent; walk-in cooler racks are chipped and rusted; observed grease buildup on cook line units, fire suppression nozzles, walk-in cooler racks, dry storage shelf liners, milk crates in walk-in, and spice carts; spray hose under pressure at mop sink without backflow prevention device installed; handle at hand sink in dish area is leaking; dumpster lid was left open; observed holes in wall near exit door; ceiling panels are loose in prep room; there is unused equipment in storage room and items outside near the dumpster; ceiling panels are stained; observed grease and food buildup on walk-in cooler racks, cook line floor and walk-in freezer floor.

Starbucks Coffee Company, 9004 West Broad Street – The following violations were reported during a routine inspection: didn’t observe a food protection manager certificate; person in charge couldn’t provide written procedures for cleaning up vomit or diarrhea; hand sink in dish area doesn’t have hand soap or paper towels; no handwash signage is posted in bathroom; temperature of whipped cream out on the counter is elevated; quat sanitizer level is too low for wiping cloths; lowboy in drive-thru has a torn gasket; lightshield near ice machine is cracked.

Bartizan Events, 4035 Whittall Way – The following violations were reported during a routine inspection: handle of ice scoop is in contact with ice at the bar; observed dust buildup around and on ceiling vents in kitchen; there is food debris on floor at bar.

Outback Steakhouse, 7917 West Broad Street – The following violations were reported during a routine inspection: dish area hand sink doesn’t have paper towels; soda machine nozzle and soda gun nozzle are dirty; temperatures of prime rib strips and raw chicken in refrigerated drawers are elevated; chemical spray bottle at mop sink is unlabeled; hand sanitizer and CO2 cartridges are stored on spice shelf; spray bottle of olive oil is unlabeled; wet ice bucket is not inverted to air dry; dish machine is broken (it broke over the weekend and was being worked on during inspection); y-connector and spray hose under pressure at mop sink without backflow prevention device installed; bar hand sink is leaking; staff bathroom doesn’t have a covered trash can; floors at bar, ceiling vents and walk-in cooler fan guard are dirty; observed low grout at three-compartment sink; floor tiles and lightshield in dish machine area are cracked.

Bowlero Richmond, 6540 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels; food debris found in both hand sinks in kitchen; dried food debris found inside microwave; observed black mold inside ice machine; there are dried soda stains inside ice chest under soda dispenser; temperatures of cheese and hot dogs in reach-in cooler are elevated; several spray cleaner bottles are unlabled; containers of white powdery substances are unlabeled; there is grease buildup on sides of cooking equipment; reach-in freezer door gaskets are moldy and stained black; observed dried food debris, crumbs and food stains on top of hot line hoods; there are dried food stains on walls throughout the kitchen; observed leak in pipes under three-compartment sink and at facuet.

Panera Bread, 9960 Brook Road – The following violations were reported during a routine inspection: there is dust and residue buildup on wire racks by dish three-compartment sink; mop isn’t hung or inverted to air dry; observed heavy buildup and food debris on floors underneath equipment on cashier line and prep line.

New Top’s China 1, 7111 Staples Mill Road – The following violations were reported during a routine inspection: scoop used for rice is stored in ambient temperature water; non-food grade plastic to-go bags are being used for food storage; cardboard is lining shelves throughout kitchen and walk-ins; observed food debris and grime on storage racks and reach-in units; ceiling tiles are missing and in disrepair.

Amanecer Latino Market, 8410 Staples Mill Road – The following violations were reported during a routine inspection: food protection manager certificate is expired; food found inside hand sink; scoops without handles are stored in food containers throughout kitchen; knives are stored in crevices between prep equipment; foil is being used to line prep shelves; unealed wood is used in walk-in cooler and kitchen; some cutting boards are heavily scratched; exterior of grill and fryer and gaskets on reach-in units are dirty; wall in storage area is in disrepair; floor is lifted in walk-in freezer; there is ice buildup on condenser in walk-in freezer; floors and walls are dirty in walk-in cooler, walk-in freezer and storage areas.

Stella’s Grocery, 5001 Huguenot Road – The following violation was reported during a routine inspection: observed dust on metal ceiling vents and ceiling tile near ventilation.

Noodles & Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: rack by sauce line is rusty; observed food splatter on panel beside stove and on sides of grill; mops are not hung or inverted to air dry; several items/areas are dirty (floor under cook line, under racks and tables, dusty air vents, wall behind dish machine, debris in mop sink); there is cracked tile by drain under hot holding table.

El Paraiso Restaurante, 4034 Crockett Street – The following violation was reported during a routine inspection: foil is being used to line prep shelves.

Noorani Kabab House, 2757 Hungary Spring Road – The following violations were reported during a routine inspection: raw eggs are stored next to cabbage and raw chicken is stored over cooked chicken and shrimp; there is 0 ppm of sanitizer at dish machine; temperatures of tomatoes, chicken and curry in main prep table are elevated; main prep table isn’t working properly; cooked lamb, chicken and rice are not date marked; racks are rusty; there is a torn gasket on lowboy door; hot holding unit is in disrepair; fan broken on lowboy unit; there is a hole in the wall by cook line and walk-in; coving tiles are pulling off the wall under hand sink; a light shield is missing.

International House of Pancakes, 9820 West Broad Street – The following violations were reported during a routine inspection: observed pink buildup on ice machine panel; temperature of links in pan on grill is too low; temperatures of crepes in make table and hash browns in cooler are elevated; two bottles of spray oil are unlabeled; food staff wearing dangling bracelets; observed buildup on stand mixer and between grill and egg station; there is a leak at the prep sink; y-connector at mop sink doesn’t have a dual check valve installed.

Café Y Sabor Bakery and Restaurant, 7807 West Broad Street – The following violations were reported during a routine inspection: open can drink found on prep; hand sink at front doesn’t have hot water; raw eggs are stored over peppers and raw meat is stored over cooked meat in reach-in; there is not enough bleach in three-compartment sink; temperatures of chicken, salad and milk are elevated; containers of white substances and other spices on dry storage rack are unlabeled; food employee is wearing bracelets; there are torn gaskets on multiple units throughout the kitchen.

Roy’s Big Burger, 5200 Lakeside Avenue – The following violations were reported during a routine inspection: dried food debris found inside microwave; ice cream machine tray is dirty; observed grease residue buildup on wire storage rack by hand sink; floor fan is dusty; hood exterior above fryers and burger grill hoods are greasy; outdoor dumpster lids were left open; there are dried food stains on walls throughout kitchen; walls near ventilation are dusty; observed grease residue and stains on walls above deep fryers; floor tile in back of kitchen is cracked.

Thai Top Ten, 9986 Brook Road – The following violations were reported during a routine inspection: open drinks found on prep table; there is buildup on the slicer; temperatures of noodles, shrimp/spring roll and soybean paste are elevated; buckets of white substance and sauces are unlabeled; knives are stored in between prep units; scoops are stored in water; racks are rusted; there are torn gaskets on units; door doesn’t close properly on lowboy unit; observed grease buildup on and around woks, under cook line, on hood and on lights under hood; observed buildup on racks and gaskets of units.

Sumo Sushi Express, 1090 Virginia Center Parkway – The following violation was reported during a routine inspection: person in charge couldn’t provide employee health policy or the written policy for time control.

Bellacino’s Pizza and Grinders, 8026-D West Broad Street – The following violations were reported during a routine inspection: some cutting boards have scratches and stains; observed dust and grime on wire storage racks in kitchen; floors under three-compartment sink and storage racks are dirty; there are broken floor tiles around dish machine, ice machine and in the walk-in cooler.
