Fogo de Chao, 11221 West Broad Street – The following violations were reported during a routine inspection: bartender cut limes for customer drinks with bare hands; employee restroom doesn’t have paper towels; temperature of three pans of polenta made the previous day is elevated; temperatures of roasted broccoli, cauliflower and chicken salad in make table are elevated; thawed salmon in prep unit is still in reduced oxygen packaging; metal wires on fryer baskets are bending and breaking.

Dunkin Donuts, 7300 Staples Mill Road – The following violation was reported during a routine inspection: sanitizer buckets are stored on prep surfaces and next to single-service utensils.

Chipotle Mexican Grill, 8955 Staples Mill Road – The following violations were reported during a routine inspection: Hobart dish machine isn’t reaching the required temperature to sanitize; dish machine doesn’t have an irreversible registering thermometer; two light bulbs are out above the grill; observed dust buildup on light shield above expo line.

McDonald’s, 8975 Staples Mill Road – The following violations were reported during a routine inspection: there is ice buildup on the bottom of the fry reach-in freezer; cement pad under water heater isn’t sealed.

Brick Road Coffee Co., 8055 Shrader Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; pink buildup found inside ice machine; observed dust buildup on ceiling vents; there is residue around floor drain.

Top’s China, 68 South Airport Drive – The following violations were reported during a routine inspection: men’s restroom doesn’t have paper towels; foods are stored uncovered in reach-in cooler and freezer; dried food debris found on can opener blade; facility doesn’t have a food thermometer; white powdery substances on cook line are unlabeled; facility is serving imitation scallops but the menu doesn’t state they are imitation scallops; food is stored directly on walk-in freezer floor; did not observe staff wearing hair restraints; cooks are not wearing aprons and their clothes have food stains, powder and residue on them; knife is stored in gap between prep table and reach-in cooler; cutting boards are heavily stained and scored; door gaskets are torn on reach-in coolers; a reach-in cooler is in disrepair; wire storage racks throughout the kitchen and can opener base are rusty; storage shelf liners and reach-in cooler door gaskets are dirty; observed grease buildup on sides of hot line equipment, in hood system, and on filters; there are stained and missing ceiling tiles; there are food stains on kitchen walls; there is grease pooling on floor underneath deep fryers.

America’s Best Wings, 4931 Nine Mile Road – The following violations were reported during a routine inspection: hand sink in kitchen is blocked by a bowl of cut green peppers; dried food debris found on can opener blade; there is mold forming in ice chute of soda dispenser; breader and batter for frying are not marked with discard times; food is stored directly on walk-in cooler and freezer floors; there is grease buildup on sides of hot line cooking equipment; shelves underneath prep tables are dirty; walk-in cooler fan guards are dusty; hand sink in kitchen is separating from the wall (screws are loose); floor tiles underneath deep fryers are cracked and raised; observed stained and dusty ceiling tile in kitchen; there is grease residue dripping from hood system and filters.

Mission BBQ Richmond, 5440 Glenside Drive – The following violations were reported during a routine inspection: there is heavy staining inside several large food containers; observed grease buildup on fryer baskets; there are old sticker labels and residue on metal/plastic food containers; there is dust buildup on ice machine vent; observed food and residue on floor drains and underneath and behind kitchen equipment.

Dave & Buster’s, 4001 Brownstone Boulevard – The following violations were reported during a routine inspection: temperatures of fire roasted salsa and bagged barbacoa in prep unit are elevated; thawed salmon is still in reduced oxygen packaging; a food prep employee is not wearing hair and beard restraints; there are bent metal wires on fryer baskets; observed buildup of dust and grease residue on wire racks, pizza oven, ovens, suppression nozzles, shelves on cook line, and rolling carts in coolers; there are old labels on clean containers; concrete flooring under prep units is not sealed and easily cleanable.

Ninja Hibachi, 10162 West Broad Street – The following violations were reported during a routine inspection: container of noodles cooling in the walk-in cooler is covered; raw shrimp is thawing in a bucket of water in prep sink; food throughout kitchen is stored in non-food grade plastic to-go bags; observed buildup on front hand sink; observed grease and food debris on sides of grill and fryer, storage shelves, pipes and squeeze bottles; food debris and grime found on walls and underneath kitchen equipment; vents are dusty.

Einstein Brothers Bagels, 3320 Pump Road – The following violations were reported during a routine inspection: temperature of top portion of plain cream cheese is elevated; pans on racks near three-compartment sink are stacked while still wet; observed dried food debris and dust on wire storage racks, fan grate in walk-in coolers, toaster and inside prep units; there is dust buildup on ceiling vents and panels.

The Tin Pan, 8982 Quioccasin Road – The following violations were reported during a routine inspection: cut tomatoes, cooked meats and cut leafy greens in prep units are not date marked; tape attached to ticket line and wall is peeling and is no longer smooth and easily cleanable; cutting boards on prep tables across from cook line are scratched and stained; free cutting boards are heavily scratched and chipping; racks throughout kitchen are rusty; floor fan and ice machine filter are dusty; observed grease buildup on sides of fryer, grill, and along the wall by the fryer; heat lamps have grease and dust build up on them; sides of prep units across from fryers are dirty; there are leaks at three-compartment sink faucet, pipe under three-compartment sink, and faucet of sink by dish machine; there are food splatters on walls by cook line; debris buildup found along baseboards under equipment; grease buildup found on floors and lines behind cook line; observed dust on airvents by walk-in cooler and in front of kitchen by glass storage and spice shelf; floor boards in dry storage rooms are missing in places and the floor is no longer smooth and easily cleanable; drain cover near prep tables on cook line is dented and disconnected.

Papa John’s Pizza, 9853 Brook Road – The following violations were reported during a routine inspection: person in charge couldn’t provide food service manager certification; there isn’t a handwash sign posted at hand sink; employee handling food isn’t wearing a hair restraint; dishes in dish pit are stacked while still wet; there is a torn gasket on left-hand door of reach-in cooler; mop sink is leaking because it doesn’t have a dual check valve installed.

The Honey Baked Ham Co., 200 Towne Center West Boulevard – The following violations were reported during a routine inspection: stainless steel bowl found in hand sink; specks of dark debris found on top of interior shield in ice machine; temperatures of sliced ham, tomatoes and slaw in prep unit are elevated; package of Hawaiian rolls stored directly on walk-in freezer floor; drying rack is rusted; vents in walk-in cooler are dusty; dumpster area is surrounded by loose trash and debris; mops are not hung or inverted to air dry; two light shields in kitchen are in disrepair.

Popeye’s, 8044 West Broad Street – The following violations were reported during a routine inspection: cold holding unit behind front counter isn’t working (person in charge states it is not currently being used); observed food residue and grime inside reach-in units and doors; cords and monitors are dusty; ice machine exterior is dirty; observed grease buildup under fryers; there is grime on floor behind ice machine and ovens; water is pooling in several areas throughout kitchen.

Kabuto Japanese House of Steaks, 8052 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide food protection manager certification; raw shell eggs stored above ready-to-eat foods; several knives and kitchen utensils are stored dirty in prep drawer; temperature of three containers of fried rice cooked yesterday in walk-in cooler is elevated; fried rice, tempura batter and cooked noodles are not date marked; menu is missing a consumer advisory for steaks; tuna is thawing while in reduced oxygen packaging; small cutting boards throughout the kitchen have deep scratches and stains; cardboard and aluminum is being used to line shelves in kitchen and wooden post holding hand sink in washing area; observed food debris and grime on service carts, hand sinks, food storage bins, storage racks/shelves, and floors and walls underneath kitchen equipment; grease buildup found on hood vents; walk-in cooler and freezer floors need cleaning; floor tiles in staff bathroom are cracked; floor tiles in dry storage room are deteriorated; there is low grouting in dish room.

Panda Express, 9100 Staples Mill Road – No violations reported during a routine inspection.

Franco’s Italian Restaurant, 9010 Staples Mill Road – The following violations were reported during a routine inspection: the certified food protection manager certificate has expired; several utensils and pans are stored with dried food debris on them; temperature of a large container of cooked pasta prepared yesterday in walk-in cooler is elevated; temperatures of ham and mozzarella in pizza prep unit are elevated; two large containers of cooked pasta made today are cooling while covered; fryer baskets are heavily bent; several areas/items are dirty (underneath pizza oven, dish machine, three-compartment sink, cook line and walls behind equipment).

Marco’s Pizza, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certification; observed heavy splitting on gaskets at multiple units; cutting boards have heavy stains and scratches; observed dust buildup on top and side of oven; observed grease buildup on oven hoods.
