Lalo’s Pinches Tacos Bar & Grill, 8026 West Broad Street – The following violations were reported during a routine inspection: employee drink stored on cutting board; hand sink nearest to the ice machine doesn’t have paper towels; raw shell eggs stored over ready-to-eat salsa in walk-in cooler; dried food found on can opener blade; raw beef is thawing in ambient temperature standing water; bulk bins of flour and salt are unlabeled; food is stored in non-food grade plastic to-go bags.

Waffle House, 8400 Brook Road – The following violations were reported during a routine inspection: there is 0 ppm bleach in bucket; ice scoop handle is touching ice; observed buildup on gaskets, bottom of units and racks of units.
Chopped Cheese and Pizza LLC, 10209 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t provide employee health policy, written procedures for cleaning up vomit and diarrhea, or verify employee training; raw shell eggs stored over ready-to-eat pizza sauce and cheeses; dried food found on meat slicer blade; several pizza cutters and knives are stored on dirty prep shelves; foil is being used to line prep shelves throughout the kitchen; cooked wings are placed directly into a cardboard box instead of a food safe container; several cutting boards have deep scratches and stains; observed dried food residue inside reach-in freezers and on dry storage shelves.
Sandano’s, 11389-B Nuckols Road – The following violations were reported during a follow-up inspection: person in charge isn’t a certified food protection manager yet; unusued equipment has not yet been removed from facility; temperature of philly cheesesteak beef cooked three hours prior in reach-in is elevated because of improper cooling methods.

Persis Indian Grill, 11331 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels or hand soap; raw marinating chicken stored over ready-to-eat Indian cheese in lowboy; temperature of two containers of rice sitting on the counter is elevated; yogurt and hard-boiled eggs sitting out on the counter are at room temperature; none of the food items stored in the walk-in refrigerator are marked with the dates they were prepared on; no ingredients on the cook line are labled; box of frozen halal chicken is stored directly on walk-in freezer floor; large spoons are stored in a stainless steel container in water that is only 115 degrees; two knives stored in crevice of second make table on prep line; foil lining is being used on cook line, above the prep units, and on bottom of storage tables; cardboard is lining the spice racks in dry storage area; both long cutting boards on prep unit are heavily stained and scored; gasket liners are torn on two lowboy units, walk-in cooler door and walk-in freezer door; fryer baskets have broken metal pieces; racks in walk-in cooler are covered in old, caked-on food debris; observed dust buildup on walk-in cooler vents; drying shelf above three-compartment sink is covered in dark residue; mop isn’t hung or inverted to air dry; there is a hole in the floor in front of the left side of the three-compartment sink; drains under three-compartment sink, prep sink and dishwasher are covered in old, dark debris; observed dust and dark debris on vents above cook line.
Torchy’s Tacos, 11120 West Broad Street – The following violations were reported during a routine inspection: soda nozzle in bar has buildup of debris encrusted along the interior; temperature of fried shrimp in the warmer is too low; y-connector on mop sink faucet doesn’t have the appropriate backflow prevention device installed.
Tropical Smoothie Café, 8207 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; quat sanitizer concentration in three-compartment sink is too low; person in charge states that blenders are sanitized every couple days (should be at least every four hours); temperatures of chia yogurt and shredded cheese in walk-in cooler are elevated; temperatures of chopped spinach and chopped kale in make table are elevated; quat sanitizer solution for wiping cloths is too low; ice bucket is stored upright with water inside; walk-in cooler gasket is torn; walk-in cooler racks are rusted; equipment not in use in storage room (reach-in cooler and microwave) should be removed from the facility.

Casa Grande, 7818 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t provide written procedures for cleaning up vomit or diarrhea; raw steak is stored above cheese in walk-in cooler and raw pork is stored above tres leches cake in walk-in freezer; observed a food employee handle raw meat and then reach for clean dishware; several utensils are stored with dried food debris on them; temperature of carnitas hot holding on grill is too low; can of Raid found near mop sink; raw pork and steak are thawing in prep sink basin without water; bulk container of flour is unlabeled; observed a live roach on the prep window and small flies; in-use wiping cloths are stored in soap and water, not sanitizer; in-use knife stored in crevice between prep unit and wall; to-go containers are stored uninverted; several cutting boards have deep scratches; foil and cardboard are lining shelves throughout the kitchen; raw steak is stored in a non-food grade plastic to-go bag; observed food residue and buildup in floor drains; ceiling vents are dusty; there are cracked tiles and low grout throughout the kitchen.
Al-Modina Restaurant, 10188 West Broad Street – The following violations were reported during a routine inspection: rice and curries stored in reach-in are not date marked; prepped vegetables are stored on the ground in the kitchen; several knives are stored in crevice between sink and wall; single-service spoons and forks are stored uninverted; foil and cardboard are being used to line shelves in the kitchen.
My Empanada, 1421 Blue Jay Lane – The following violations were reported during a routine inspection: observed a food employee wash hands without soap; food employee touched lettuce with bare hands; chlorine sanitizer concentration at three-compartment sink is too high.

Elephant Thai Restaurant, 11676 West Broad Street – The following violations were reported during a follow-up inspection: temperatures of sticky rice and diced chicken are elevated; food items stored directly on walk-in freezer floor; take-out containers are still stored on dirty shelf of prep unit; long cutting boards on prep line are still scored and stained; drying rack above three-compartment sink is still dirty; ceiling vents in kitchen are still dusty.
Jimmy John’s, 11301 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; cutting boards on long prep line are heavily scored and stained; gasket liner is torn on walk-in cooler door; walk-in cooler is leaking from the interior vent; drying racks in back of kitchen are rusty; racks in walk-in cooler are covered in old food debris; walk-in cooler vent is covered in dust; there is a hole in the ceiling in back of kitchen.

Bernie’s Sub & Pizza Shop, 415 North Ridge Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; observed an employee complete items around kitchen and then place gloves on to prepare food without washing hands first; hand sink doesn’t have paper towels; temperatures of pizza sauce and deli meat are elevated; deli meat and other items are past their expiration dates; racks in units are rusty; some equipment and containers are cracked; observed debris in units and on counters; floor is in disrepair; lights are missing protective shields.
New Chinese Chef, 403-A North Ridge Road – The following violations were reported during a routine inspection: sanitizer concentration at three-compartment sink is too low; temperature of fried rice in large container made yesterday is elevated; chicken, noodles and egg rolls are not date marked; small containers of white substances are unlabeled; observed debris on racks in units, walk-in and throughout kitchen; observed grease on cooking equipment; floors and walls are dirty and in disrepair.
