Mosaic Restaurant, 6229-A River Road – The following violations were reported during a follow-up inspection: bar hand sink doesn’t have hand soap or paper towels; several cans are dented along seams; facility isn’t keeping shell stock tags for 90 days past last shell stock was served; observed pink and brown mildew buildup on backsplash of ice machine; servers are handling exposed food while wearing bracelets; several employees with facial hair are not wearing beard guards; fryer baskets have bent and broken wires; prep table and single cutting boards are scratched and stained; facility doesn’t have quat sanitizer test strips for bar sink; side of prep table is dirty; racks in both walk-in coolers have mildew and food debris buildup on them; walk-in cooler fan vents are dusty; Ecolab dispenser at three-compartment sink is leaking; floors under sinks and equipment at bar are dirty; floor in back storage room isn’t smooth and easily cleanable.
Joey’s Hot Dogs, 4028-H Cox Road – The following violations were reported during a routine inspection: facility couldn’t provide food protection manager certificate, written procedures for cleaning up vomitting and diarrhea, or employee health policy; soda nozzle on prep line has dark specks of debris lining the top of the interior; plastic containers holding spices in dry storage area are unlabeled; drying racks above three-compartment sink are peeling; old food debris is covering bottom of standing three-door freezer in back of kitchen.
Glory Days Grill, 10466 Ridgefield Parkway – The following violations were reported during a routine inspection: no manager or supervisor has a food protection manager certification; hand sink at bar is being used as a dump sink (straws and lemons found in basin); observed pink residue buildup inside ice machine; dish machine at bar isn’t sanitizing; temperatures of boneless wings and chicken breast in bottom section of reach-in cooler are elevated because large sheet pans of wings are blocking airflow to the bottom; employees with long facial hair are handling food and clean equipment without wearing a beard guard; prep and single cutting boards are heavily scratched and stained; metal wires are broken and bent on several fryer baskets; wire storage racks in walk-in cooler and prep areas are heavily rusted; observed dust buildup on wire storage racks; there is broken glass and grime inside keg coolers at bar; spray nozzle attached under pressure at mop sink without proper backflow prevention device installed.
The Wine Whisperer, LLC, 11535 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction documentation from salmon provider; containers of compound butter and bresaola are past their expiration dates; gasket liners on two-door reach-in refrigeration unit are torn; cutting board on prep unit is scored and stained; drying shelf is covered in a thin layer of dark debris; y-connector on mop sink faucet doesn’t have the proper backflow prevention device installed.
Mellow Mushroom, 12171 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t verify that employees have received training on illness reporting responsibilities; three dented cans of food found on storage rack; observed flies landing on uncovered pizza dough; observed multiple small flies around pizza boxes, bar and pizza prep area; food debris found on top of conveyer oven; temperature of beer cheese in hot holding unit is too low; temperatures of portioned chicken, onion and shredded cheese in make table are elevated; make table isn’t maintaining foods at the proper temperature; time isn’t marked for garlic in oil or pizza by the slice and no written time policy is available; multiple chemical spray bottles in storage are unlabeled; knives are stored in between crevices of make table; slicer blade guard is soiled with food debris; food debris found on racks; spray nozzle under pressure at bar three-compartment sink doesn’t have a proper backflow prevention device installed; faucet is leaking at three-compartment sink; y-connector on mop sink faucet doesn’t have the proper backflow prevention device installed; wet mop isn’t hung or inverted to air dry; observed dust and food splatter on ceiling tiles; there is food debris in drain by ice machine; observed standing water on floor of dish pit.

Shula’s, 12042 West Broad Street – The following violations were reported during a routine inspection: hand sink in back of kitchen doesn’t have paper towels; ice strainer found in bar hand sink; marinara sauce in reach-in unit isn’t date marked; menu doesn’t contain disclosure next to house-made Caesar dressing; box of raw meat stored directly on walk-in freezer floor; there are torn gasket liners on first reach-in unit on main prep line, reach-in unit at the end of small prep line, and on walk-in cooler #3 door; drying racks next to dishwashing area are covered in a layer of dust buildup; there is a direct plumbing connection into the sewage line underneath the three-compartment sink instead of an air gap separating the water supply plumbing and the floor rim; vacuum package machine that person in charge states hasn’t been used in two years should be removed from the facility.

Chez Max Restaurant, 10622 Patterson Avenue – The following violations were reported during a routine inspection: raw fish was vacuum-packaged in-house; containers of similar looking spices on cook line are unlabeled; cook with long facial hair isn’t wearing a beard restraint; prep unit cutting boards are heavily scratched and stained; observed food debris inside keg cooler at bar, inside lowboy storage cabinet, under grill and on top of prep table shelves; there is a buildup of dust on ceiling panels and vents; wall corner near dishwash area is damaged.
Henrico Sports and Events Center, 1 All Star Boulevard – The following violation was reported during a routine inspection: observed buildup on shoot of ice machine and soda nozzle at bar.
Chanello’s Pizza, 5239 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of sliced tomato, shredded cheese and lettuce in salad prep unit are elevated; this unit is not maintaining foods at the proper temperature; temperatures of bagged chicken and bagged steak in pizza make table are elevated; observed small flies in employee restroom and near three-compartment sink; there is a large gap at the top of the back door; some in-use containers are cracked and broken; gaskets are torn on pizza make table; pizza pans are in disrepair; pizza make table lids are in disrepair and plastic has been stuffed in to fill gaps; observed heavy buildup of dried food debris, dust and grime on wire storage racks throughout kitchen, walk-in cooler fan system, utensil bins, sauce bottles, pizza oven, electrical cables and cords, floor mixer and inside chest freezers; restroom doesn’t have a covered trash receptacle; observed wall damage and peeling paint throughout facility; observed buildup of flour and grime on floors and walls throughout facility.

Taziki’s Mediterranean Café, 4024-C Cox Road – The following violations were reported during a routine inspection: hand sink in kitchen doesn’t have paper towels; a trash can is blocking the hand sink on the cook line; both the three-compartment sink quat sanitizer and the dishwasher chloride sanitizer tested at 0 ppm; refrigeration drawers under grill aren’t able to open and close properly without applying force; wire drying rack next to prep tables is peeling; foil liner is being used on top of warming unit at end of cook line; wire racks storing dry ingredients are covered in old food debris; lights in prep area in back of kitchen are missing a shield.
Firehouse Subs, 4028-K Cox Road – The following violations were reported during a follow-up inspection: there is still no food protection manager certified associate; first hand sink on prep line is still missing paper towels; temperature of sliced tomatoes prepared yesterday is elevated.
Caffespresso, 1127 Gaskins Road – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction documentation from cold smoked salmon supplier; observed dried food splash in microwave; provolone and mozzarella cheese are not date marked; person in charge states the cooked eggs and eggplant were incorrectly dated.
Twin Hickory Tavern, 1420 North Parham Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; knives are stored in between prep table and hot holding table; single-service articles are not stored inverted or covered to protect from contamination; fryer basket in dry storage closet has bent wires; equipment that is no longer in use (tortilla maker, blast freezer and chicken rotisserie) should be removed from the facility; several lights are burned out; several lights are flickering in fume hood; mops are not inverted or hung to air dry.
Taqueria Y Antojitos El Chido, 7568 West Broad Street – The following violations were reported during a routine inspection: temperatures of pico and tomatoes in prep unit are elevated; food is being stored in non-food grade plastic to-go bags.
