Bryan Park Bar & Grill, 5516 Lakeside Avenue – The following violations were reported during a routine inspection: tuna in reach-in fridge is thawing while in reduced oxygen packaging; there are torn gaskets on reach-in refrigerator doors; observed grease and food debris buildup between fryers and grill top and on fan grates within kitchen.

Sandano’s, 11389-B Nuckols Road – The following violations were reported during a routine inspection: food manager certification expired in 2023; black residue found inside ice machine in and around ice chute; a whole cooked chicken is in a covered pan on counter (wasn’t cooled properly); multiple TCS foods are out of refrigeration and their temperatures were elevated (peppered ham, deli meat, swiss cheese, chicken); cooked sausage is sitting out on the counter (isn’t being cooled properly); observed a live German roach inside the walk-in used for dry storage; two-door reach in, panini press, oven and other equipment are unused/broken and should be removed from the facility; racks inside multiple units are rusty; observed grime in mop sink.

McDonald’s, 11311 Nuckols Road – The following violations were reported during a routine inspection: employee filling bags of fries has hands/fingers in touch with the fries; temperature of cream in dispenser is elevated; burritos are past their expiration date; stickers on lettuce and tomato had a use-by date of Dec. 26 (appears that these foods were not marked with a four-hour discard time today); y-connector at mop sink doesn’t have a proper backflow prevention device installed.

Crustacean Boil N’ Grill, 8902-C West Broad Street – The following violations were reported during a routine inspection: observed black and pink mold on interior of ice machine; wooden pallets are being used to store food in walk-in fridge and freezer; wire storage racks in walk-in and dry storage are rusty; fryer baskets have bent corners; observed mildew buildup on walk-in fridge storage racks.

Wendy’s, 10177 Brook Road – The following violations were reported during a routine inspection: soiled cardboard found next to fryers on cook line; observed dust buildup on fan grates in walk-in fridge; there is grease buildup on side of fryers; there is missing and broken coving underneath three-compartment sink, next to walk-in fridge and underneath hand sink next to ice machine; observed dust buildup on AC vents above dry storage and three-compartment sink; observed heavy grease buildup at floor drain of three-compartment sink.

Bubbles and Bites, 6329 Mallory Drive – The following violations were reported during a routine inspection: hand sink in kitchen doesn’t have hand soap; observed several foods throughout the kitchen without lids or bags not sealed (cheese, olives in bucket, quinoa in bin, bag of apricots, container of figs, bag of peanuts); there is a dent on can seal of pineapple; single-use articles are not stored in a way to protect them from contamination; walk-in cooler racks and dish rack are rusty; both ovens are dirty with food build up on the bottom; drip pan of grill is full; mop sink with y-connector valve doesn’t have the proper backflow prevention device installed; bathroom door isn’t self-closing.

Amanecer Latino IV Inc., 9301 Quioccasin Road – The following violations were reported during a follow-up inspection: raw chicken is stored above hot dogs in grill drawers; salsa cups are being used as scoops for salsa; gaskets of prep unit are torn; racks in prep unit are rusted; prep unit cutting board is scratched and stained yellow; several items are dirty (prep unit gaskets, shelves under prep tables, crates and reach-in freezer vents); wall is torn and floor is cracked at three-compartment sink.

Tropical Smoothie Café, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: plastic bins are scratched and stained; storage racks throughout are rusty; mop isn’t hung to air dry; wall behind fruit prep table is dirty; wall corners are not smooth and easily cleanable.

Cooper’s Hawk Winery & Restaurant, 11792 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; three containers of white powdery spices in back of kitchen are unlabeled; gasket is torn on lowboy freezer on cook line; fryer baskets have broken metal pieces; outdoor dumpster area is dirty and surrounded by loose trash and debris; mops are not hung to air dry.

Dave’s Hot Chicken, 11525 West Broad Street – The following violation was reported during a routine inspection: boxes of chicken are stored directly on walk-in freezer floor.

Pho Thien Phat 2, 9031 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate; person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; an employee is eating with gloved hands and continued to handle food without washing hands; observed employees not wash hands when entering the kitchen or after eating; employee washed hands in prep sink instead of hand sink; kitchen hand sink basin is blocked with a bucket; observed an employee wash a bowl in hand sink, shrimp thawing in hand sink and food debris in hand sink (hand sinks are for handwashing only); staff bathroom doesn’t have hand soap; hand sink near stove and staff bathroom don’t have paper towels; raw beef is stored over sliced green onions and fried noodles in walk-in cooler; employee failed to sanitize bowl and cutting board after washing; microwave and ice machine are dirty; several cooking utensils have dried food debris on them and the container they’re stored in is dirty; observed elevated temperatures in noodles, brisket, sprouts, pork belly, sliced meat, imitation crab, tofu, fishball and lettuce; spackling and cans of paint are stored next to jelly mix and syrup; Hot Shot ant and roach spray found in kitchen; open first aid kit and hydrogen peroxide are stored over food on prep counter shelf; meat is cooling in covered, deep metal pans; raw shrimp is thawing in standing water; salt and sugar on cart on cook line are unlabeled; two sticky fly traps are stored over food prep and service areas; food is stored directly on walk-in freezer floor; single-service containers are being used as scoops in bulk rice bins; forks are not stored inverted; gaskets are torn in prep units and walk-in cooler; paint is chipping on storage rack near walk-in freezer; two-compartment sink is out of order; reach-in cooler racks are chipped and rusty; dirty cardboard is being used as shelf liners in walk-in cooler; unsealed wooden pallets are being used in storage room and walk-in cooler; banh mi bread is stored in a non-food grade white garbage bag; dish machine is dirty; several items/areas need cleaning (inside both prep units, fire extinguisher, exteriors of trash bins, racks in walk-in cooler and floor fan); there is carbon buildup and food debris on stove tops and in drip trays; hood and suppression system pipes are greasy; there is plastic wrapping on microwave; hand sink near stove is slow to drain; there are no covered trash cans in staff bathroom or women’s bathroom; facility has many items and equipment either not in use or not necessary to keep; there is a hole in walk-in cooler door; walk-in freezer door is bent and there is heavy ice buildup inside; floors throughout kitchen are cracked; floors in dish area, walk-in cooler and under cooking equipment are dirty; ceiling panels near hallway are stained black; ceiling vents in kitchen and staff bathroom, grease trap and walls near hallway are dirty.

Sandston Moose Family Center, 4505 Oakleys Lane – The following violations were reported during a routine inspection: food protection manager certificate is expired; prepared chicken salad and cooked hamburgers in reach-in fridge are not date marked.
