Arilang Sushi, 2239 Old Brick Road – The following violation was reported during a routine inspection: soiled cardboard is lining shelves in the walk-in cooler.
Taco Bell, 9420 West Broad Street – The following violations were reported during a routine inspection: there is low grouting throughout the kitchen; ceiling vents above the prep line are dirty and dusty.
Hong Kong Kitchen, 2907 Williamsburg Road – The following violations were reported during a routine inspection: rear hand sink at end of prep line is obstructed by old oil containers; time isn’t being tracked for fried chicken held at room temperature on time control policy; containers of sugar, salt and MSG on prep line and in dry storage are unlabeled; single-service to-go containers are not stored inverted on prep shelf; cardboard boxes and soda containers are being reused throughout the kitchen; milk jugs are being utilized as rice scoops; single cutting board in prep area has deep scarring and scoring; fryer basket is broken; wire storage racks in walk-in fridge are rusty; observed food debris and heavy dust buildup on dry storage racks; prep shelves near prep line are greasy; interiors of lowboy fridge units and reach-in unit are dirty; mold is present on gaskets of lowboy units; observed heavy grease buildup and dried food stains on floors and walls behind hot line; there are dried food stains on walls throughout kitchen; observed heavy grease buildup on hood system throughout kitchen.
Long John Silver’s, 2206 East Laburnum Avenue – The following violations were reported during a routine inspection: dried food debris found inside microwaves; wire racks above three-compartment sink are rusted; observed heavy grease buildup on sides of hot line cooking equipment; ceiling tile and vent in kitchen and top of reach-in freezer are dusty; observed debris and flour on wire storage racks; wall behind/underneath three-compartment sink and interior of reach-in cooler behind register are dirty; observed food debris and mold on wire racks in walk-in cooler; observed grease buildup and food debris on floor underneath deep fryers; a ceiling tile is missing in the dining area.

IHOP, 10151 Brook Road – The following violations were reported during a routine inspection: salmon is thawing in reduced oxygen packaging; debris found in bottom of units and drawers; observed grease on sides of cook line equipment; water is pooling around drain in back kitchen; sink in women’s restroom isn’t draining; observed buildup in drains throughout; there is grease on the floor behind fryers; there is black buildup in light by dish machine.
McDonald’s, 5520 West Broad Street – The following violations were reported during a routine inspection: dried food debris found inside microwave; observed grease buildup on sides of burger grills; observed food debris on reach-in cooler door gaskets; there is mold and food debris on wire racks in walk-in cooler; there is a leak at the mop sink behind hot water valve; outdoor dumpster doors were left open; dried beverage stains found on floor and walls behind beverage area; observed dried soda stains/black stains on drive-thru walls; dried food stains found on walls behind/near three-compartment sink; burger grill hood filters are very greasy.
Thai Diner, 8059 West Broad Street – The following violations were reported during a routine inspection: cook wiped nose with forearm and continued cooking without washing hands; dried food debris found on mortar and pestle on cook line shelf; temperatures of fried chicken, cooked broccoli and fried tofu in prep units are elevated; observed a large number of dead flies in traps at kitchen entryway; live mouse found in glue trap in reach-in cooler room; wiping cloths are in a sanibucket with 0 ppm chlorine; in-use cooking utensils are stored on egg cartons; rice scoops are stored in ambient temperature standing water; racks are chipped in reach-in cooler next to ice machine; cardboard is stored throughout kitchen (as storage containers, above heater on ceiling, against walls and on shelves); observed grease buildup on hoods and fire suppression nozzles; pipes at cook line and racks in reach-in coolers are grimy; there are holes in the wall at kitchen entryway.

Starbucks Coffee Company, 3820-A Gaskins Road – The following violations were reported during a routine inspection: temperature of milk sitting out on counter is elevated; observed a leak at the mop sink.

Eggmania, 2610 Tuckernuck Drive – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit or diarrhea; employee entered kitchen from back door and didn’t wash hands; employee failed to sanitize knife at three-compartment sink; microwave is dirty; chlorine concentration at three-compartment sink is too low (person in charge states that no one during current shift knows how to do this); raw shell eggs have been sitting out at room temperature for about an hour; no date marks found on curries, buttermilk or yogurt drinks; menu doesn’t contain a consumer advisory for sunny-side-up eggs; cough medicine found on spice shelf; cans of air freshener found next to soda; rice is cooling doubled panned in prep unit with lid open; plastic containers are stacked while still wet near three-compartment sink; cutting boards are heavily scratched; facility doesn’t have sanitizer test strips; person in charge states that mop water is dumped on the grass next to the dumpster as the mop sink is being used as storage; large amount of food debris found on the ground at dumpsters; side doors of dumpsters were left open; wall behind cook equipment and walk-in cooler floor are dirty.
McDonald's, 11280 Patterson Avenue – The following violations were reported during a routine inspection: no handwash signage posted in women’s restroom; temperatures of lettuce, cheese and creamers in reach-in cooler are elevated; sanibuckets are stored directly on kitchen floor; reach-in cooler and espresso machine are in disrepair; food debris found inside cooler door gaskets throughout kitchen; there is pooling soda/syrup inside cabinets under self-service soda machine; mop sink with y-valve doesn’t have a backflow prevention device installed; outdoor dumpster doors and lids were left open; observed mildew on three-compartment sink wall; wet mop isn’t hung to air dry.
Pho So 1, 9135 West Broad Street – The following violations were reported during a follow-up inspection: person in charge has failed to demonstrate food safety knowledge and ensuring employees follow food regulations (observed improper cooling, no employee health policy, employees are not trained on food safety, and there isn’t a certified food protection manager); employee handled cilantro and sprouts with bare hands; there is no hand sink installed near or in warewashing area; cooked spring rolls are stored in contact with used cardboard boxes in walk-in freezer; menu doesn’t contain a disclosure statement for raw and/or undercooked foods (eye of round and egg); can of Lysol is stored in bin with utensils and lids; observed an infestation of live roaches (adult and baby) throughout the kitchen (crawling in a container of peanuts and behind three-compartment sink); a food employee is wearing a bracelet and watch in the kitchen.

B & L Caribbean Foodz, 100 West Nine Mile Road – The following violations were reported during a routine inspection: temperatures of cheese and butter in white homestyle refrigerator are elevated; kitchen floor is not smooth and easily cleanable; observed food stains on walls next to stove and behind three-compartment sink; mildew and black stains found on seal between three-compartment sink and wall.
Chick-fil-A, 9100 West Broad Street – The following violation was reported during a routine inspection: temperature of chicken wraps made this morning in lowboy are elevated.

Q Barbeque, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: layer of pink mold found inside ice machine; observed rust buildup on wire storage racks in dish room, walk-in and in dry storage; observed heavy dust buildup on fan in prep area and on AC grates within walk-in fridge; mop sink with a hose and spray nozzle under pressure doesn’t have the appropriate backflow prevention device installed; ceiling AC vents and filters are dusty.
Tony’s Italian Restaurant, 3409 Cox Road – The following violations were reported during a routine inspection: staff meal located next to tomatoes being prepped; raw burger patties and eggs stored above cheese and lasagna in walk-in cooler; temperature of diced tomato in pizza unit is elevated; no date marks found on lasagna or shredded cheese in walk-in cooler; bins of sugar and salt are unlabeled; walk-in cooler gasket is torn; paint is peeling on the pizza hood; bread is stored in white non-food grade garbage bags; mop sink is broken; person in charge states that mop water is dumped on the ground near outside dumpsters; dumpster lid and side doors were left open.
El Paso Mexican Restaurant, 3417 Cox Road – The following violations were reported during a routine inspection: cook scratched their head and continued food prep without washing hands; raw chicken is stored over shredded lettuce; ice machine and reach-in cooler fans are dirty; salsas are past their expiration dates; menu doesn’t contain asterisks next to steak dishes that may be cooked to order; several food workers are wearing bracelets and watches; fryer baskets are bent; there are cracks in the steam table used on weekends; reach-in cooler racks and ceiling vents are rusty; walls of reach-in cooler are scratched; there is a leak in ceiling above three-compartment sink.
