Peter Chang, 11408 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; sink in back of kitchen is blocked by a table holding a large rice cooker; sauces in middle lowboy are uncovered while drops of condensation were present on the top rim of the unit; raw chicken stored over lettuce in lowboy #2; ice machine in back corner of the facility and soda nozzle in bar are both covered in black/brown dried-on debris; plastic bag of breaded shrimp in walk-in cooler isn’t date marked; breading, oil and dry ingredients along the cook line are unlabeled; buckets of sauce are stored directly on walk-in freezer floor; lettuce is stored in a non-food grade grocery bag in middle lowboy on prep line; cleaver is stored in the crevice between two prep units; top prep shelf holding dry ingredients and take-out containers is covered in a thick layer of dark buildup; long cutting boards on prep line are heavily stained and scored; gasket liners on right-side door of first and middle lowboys on prep line are torn; gasket liner on standing reach-in freezer door is torn; the main light in walk-in cooler is not functioning, leaving the cooler dim and hard to inspect; fryer baskets have broken metal pieces; ingredients rack and bottoms of all prep tables are rusty; vents in walk-in cooler are dusty; drying shelf above three-compartment sink is covered in a layer of caked on buildup; stove is covered in old food debris and grease; hood filters are covered in a a heavy buildup of grease.
More Than Sushi, 9199 Staples Mill Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit or diarrhea; knives, a funnel and other cooking utensils were stored dirty; temperatures of seaweed, bean sprouts, and pork and vegetable dumplings cold holding in prep unit are elevated; cardboard is lining prep table shelves; observed buildup of grease, food and grime on floors and walls in the kitchen (especially behind equipment) and on hood filters; observed grease and dried food residue on surfaces of grill, fryers, blenders, tea containers, prep unit/table shelves, wire storage racks, sink handles, and inside prep units and freezer.
Taco Bell, 11242 Patterson Avenue – The following violations were reported during a routine inspection: oil is dripping from racks storing clean equipment above three-compartment sink basins; dry racks near three-compartment sink basin are dusty.
Burma Sushi & Salad To Go, 11216 Patterson Avenue – The following violations were reported during a routine inspection: menu doesn’t contain an asterisk next to foods served raw and/or undercooked linking them to the consumer advisory statement; single and sushi bar cutting boards are heavily scratched and stained; two reach-in units in back room are not working properly (person in charge states he is repairing them).

Oyster’s Pearl Café, 3001 East Parham Road – The following violations were reported during a routine inspection: mac and cheese and green beans stored with raw chicken; raw chicken stored over raw fish in lowboy; there is 0 ppm sanitizer at three-compartment sink; no date marks found on chili, mac and cheese, green beans or potatoes; scoops found in water; observed grease and dirt on hood filters.
McDonald's, 1778 North Parham Road – The following violations were reported during a routine inspection: temperature of sausage burritos in breakfast reach-in cooler is elevated; this unit’s ambient temperature is also elevated; walk-in cooler fans are dusty; mops are not hung to air dry.
McDonald’s, 10000 Brook Road – The following violations were reported during a routine inspection: fryer basket has bent wires; walk-in cooler fans are dusty; walk-in freezer floor is dirty; mop isn’t hung to air dry.
Arby’s, 5205 Brook Road – The following violations were reported during a routine inspection: dried food debris found inside microwave and on reach-in cooler door gaskets throughout kitchen; observed food debris and grease residue on sides of oven and deep fryers; walk-in cooler and freezer door gaskets and storage racks in kitchen and walk-in cooler are dirty; wet mop isn’t hung to air dry; observed food stains on ceiling above three-compartment sink, walls behind/underneath three-compartment sink and mop sink, and on walls behind roast beef warmer unit; there is grease and food debris on floors and walls behind/underneath deep fryers.
Dunkin Donuts, 2300 Hungary Road – The following violations were reported during a routine inspection: stirrers are stored in water; observed low grouting in front and back kitchen.

Farm and Oak, 3601-A Cox Road – The following violations were reported during a routine inspection: staff drink found on prep counter; bar hand sink is blocked with glasses; raw shrimp stored over cooked pork belly in drawers under grill; there is 0 ppm chlorine at bar dish machine; one blender and several ladles stored “clean” have dried food debris on them; roasted vegetables are past their expiration date; wet ice buckets are not stored inverted; observed a leak in walk-in cooler; there isn’t a backflow preventer installed at mop sink with y-attachment.
Chipotle Mexican Grill, 4930 West Broad Street – The following violations were reported during a routine inspection: top of dish machine has food debris on it; there are food stains and debris on walk-in cooler storage racks; there are food stains on ceiling and walls in kitchen; ceiling tile near ventilation is dusty.
Wong’s Tacos, 201 Maltby Boulevard – The following violations were reported during a routine inspection: person in charge couldn’t locate written procedures for cleaning up vomit or diarrhea; observed an employee drink from an open cup of water on the cook line and store cup next to clean plates on shelf; uncovered raw tuna stored over uncovered churros in lowboy unit; dried food debris found on dicers; observed black mold-like substance in and directly around ice chute inside ice machine; temperature of portioned bags of brussel sprouts is elevated; several containers of flour for breading shrimp and other foods are not held under refrigeration; discussions with employee and person in charge revealed that the foods are not handled consistently to follow time/temperature requirements; pickled onions are past their expiration date; observed old date stickers for other foods on containers of prepared foods; brussel sprouts and pork are missing accurate date labels; consumer advisory on menu is missing the proper disclosure for foods that may be served raw/undercooked; shrimp is thawing in standing water in the prep sink; small individual cutting boards are heavily scratched; cutting boards at make tables are starting to become worn; metal fryer baskets are damaged and bent; metal skimmer has a metal piece poking out; mop sink faucet needs a dual check valve installed; a drain line is missing at the soda gun holder allowing drippage from soda gun onto ice below.

Jimmy John’s, 7005 Three Chopt Road – The following violations were reported during a routine inspection: there is 0 ppm quat sanitizer at three-compartment sink; pink residue buildup found on Dr. Pepper soda nozzle; cutting boards at prep table are scratched and stained; mop isn’t hung to air dry.
Sea By Thai Restaurant, 2452 Old Brick Road – The following violations were reported during a routine inspection: dates when last mussels from a batch are sold/served are not being recorded on tags; in-use knives are stored in dirty crevices of equipment at bar; spoons are not stored inverted in containers above prep units; single cutting boards are heavily stained and scored; observed buildup of food debris and dust on surfaces of wire storage racks, rolling carts, cables, sprayer at sink and inside reach-in coolers; paint is peeling on walls and on cement floors; floors are no longer smooth and easily cleanable; observed duct tape around ceiling light fixture.
