Ford’s Garage, 11275 West Broad Street – The following violations were reported during a routine inspection: temperature of brisket in hot holding unit is too low; employee working with exposed food is not wearing a hair restraint for beard; in-use knife stored in crevice of prep units; equipment is being stacked while still wet; employee is cloth drying equipment out of the dish machine.
Zorba’s Greek-Italian Cuisine, 9068 West Broad Street – The following violations were reported during a routine inspection: ice and food debris found in hand sinks; temperatures of feta and hummus in prep unit and tomato sauce in walk-in cooler are elevated; grape leaves (dolmades) in walk-in cooler are past their expiration date; fish in walk-in cooler is thawing in reduced oxygen packaging; racks in walk-in cooler and dish area are rusted; observed grime buildup on exteriors of stove and grills and on fans in walk-in cooler; hood filters are bent and in disrepair.
Chuck E. Cheese, 9030 West Broad Street – The following violation was reported during a routine inspection: temperature of shredded cheese at the salad bar is elevated.
The Big Scoop, 200 Towne Center West Boulevard – The following violations were reported during a routine inspection: first front counter hand sink doesn’t have paper towels; there is 0 ppm sanitizer in dishwasher; gasket liner is torn on reach-in refrigeration unit in side storage room; there is a direct plumbing connection under the prep sink in the back of the kitchen; drain under three-compartment sink is covered in a dark buildup.
Five Guys Burgers & Fries, 1601 Willow Lawn Drive – The following violation was reported during a routine inspection: observed food debris and grease buildup at floor drains and coving underneath three-compartment sink and prep sink.

Tarrants West, 11129 Three Chopt Road – The following violations were reported during a routine inspection: employee drink found on cutting board on cook line; an employee wiped face with gloved hand before continuing to touch ready-to-eat food; bottle of bleach and ice found in hand sinks; dish area hand sink is blocked by mop bucket and black cart; hot water at hand sink next to cook line doesn’t reach the required temperature; bins of grits and polenta in dry storage are uncovered; there is 0 ppm chlorine in kitchen and bar dish machines; knives stored clean still have dried food debris on them; temperature of rice in warmer is too low; temperature of chicken salad in salad unit is elevated; date marks on two containers of two-thousand island dressing (made in house with raw egg) are incorrect; sliced hunks of meat in dry storage walk-in cooler are not date marked; there is no consumer advisory for salmon on to-go menu, burger on beer specials menu, or caesar salad, burger or salmon on catering menu; ice scoop handle is touching ice in the ice machine; fryer baskets are bent; meat case racks in walk-in cooler are rusted; soda machine in wait area is broken and leaking; white cutting boards are scratched and stained; observed dust on shelves and looped cords at cook line and on ceiling vents; salad dressing racks in walk-in cooler and floor under main kitchen dish machine are dirty; ceiling panel above grill is bowed; exterior door trim of walk-in freezer is broken; there is low grouting in dish area.
Toast New American Gastropub, 7007 Three Chopt Road – The following violations were reported during a routine inspection: facility couldn’t provide proof of employee health policy; employee walked outside to walk-in cooler and returned to cook line without washing hands and changing gloves; shell eggs stored above fries in walk-in cooler; observed mildew on backspash of ice machine; food debris found on dicer, tongs and fry cutter; temperature of two pans of queso on speed rack are elevated (they had been cooling for three hours); temperatures of portioned turkey, sliced tomatoes and raw salmon in prep tables are elevated; containers of pickled vegetables are past their expiration dates; menu is missing disclosure that eggs may be served raw/undercooked; spray bottles containing blue liquid are unlabeled; cutting boards at all prep units and a single cutting board are scratched and stained; fryer basket wires are bent and broken; there is standing water in beer fridge; several items/areas in kitchen are dirty (shelf under grill, behind cook line, in between grill and range, and rack by front prep unit); receptacle lid is damaged and unable to close; floor tiles in front of fryer are damaged; light bulb is out in hood system.
La Cabaña, 4086 Crockett Street – The following violation was reported during a routine inspection: small cutting boards are scratched and stained.
Potro’s Mexican Restaurant, 9074 Staples Mill Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; oyster tags are not marked with the date the last shell stock was sold; previously used orange juice containers are being reused to store piña colada and margarita mix.
Buttermilk and Honey, 12246 West Broad Street – The following violations were reported during a routine inspection: sanitizer bucket found in hand sink in back of kitchen and this hand sink doesn’t have hand soap; quat sanitizer level in three-compartment sink tested below the required concentration; sauce squeeze bottles on prep line are unlabeled; handle on bottom drawer of the warmer has broken off completely and is missing; fryer baskets have broken metal pieces; walk-in cooler racks and drying racks are rusty; long cutting board on make table is heavily scored and stained; dry ingredients rack is covered with sticky debris; dumpster area is dirty and surrounded by loose trash and debris; ceiling vents in kitchen are covered in dust.

Golden Corral Restaurant, 4050 Gaskins Road – The following violations were reported during a routine inspection: employee reached into soiled water to unclog sink and then handled clean dishes without washing hands; hand sink in back kitchen doesn’t have hand soap; sink and surface sanitizer isn’t reaching acceptable levels; dicer and tongs are stored with food residue still on them; temperatures of fried chicken in hot holding warmer and burgers on buffet line are too low; temperatures of shredded cheese, pepperoni, pizza sauce and cabbage are elevated; there are unclear procedures for foods placed on time control (some foods are not labeled and the start time for some foods is incorrect); meatloaf and chicken previously placed on time control were pulled to the walk-in cooler for later use; dairy allergy pills and hand sanitizer are stored above prep table where clean silverware is wrapped; there is no working thermometer for outdoor walk-in cooler; fly trap above hand sink in back kitchen is full; several metal and plastic containers are stacked while still wet; food contact equipment is stored in a container collecting water; fryer baskets are in disrepair; there is a gap in hood vents throughout the kitchen; mop sink is missing handles; racks in reach-in freezer are rusty; ice machine is broken; there is ice buildup in bottom of pizza prep unit; observed dust and food residue on wire racks, fan guards, buffet signage and around equipment; dump sink in dishwashing room is clogged; pre-wash faucet is broken and leaking; water line in wait station is leaking; there is a leak at prep sink in bakery area; spray hose under pressure at pre-wash station doesn’t have a dual check valve installed; both dumpster bins were left open; mop is not hanging to air dry; there is cleaning needed throughout kitchen, near fryers, and under equipment; blood found in walk-in cooler; observed food residue in floor drains and on walls and ceilings; observed low grout and cracked tiles throughout kitchen; FRP is peeling on walls; lights are out in both hood areas.
Starbucks, 7000 Forest Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; cups in dispenser are overfilled; bottom shelf of dry storage racks next to back door is dirty.
Foxtail Coffee Co., 2240 Old Brick Road – No violations reported during a routine inspection.
Baskin-Robbins, 5510 West Broad Street – The following violations were reported during a routine inspection: observed several roaches (one living and two dead) in storage cabinets; walk-in freezer door gaskets are torn; observed food debris and crumbs in reach-in coolers and take-home ice cream coolers; cabinet doors and shelves throughout kitchen are in need of cleaning; white reach-in cooler and walk-in freezer gaskets and candy topping shelf are dirty; dried food debris found on walk-in freezer racks; sound system above food prep area, shelf under register and ceiling vents over food prep area are dusty; there isn’t a lid on the trash can in employee restroom; observed food debris on floors underneath counters and equipment and along piping throughout kitchen; there are food stains/splatters on fan guards and the backs of take-home ice cream coolers; ceiling tile in dining area is stained.

Papa John’s Pizza, 551 East Laburnum Avenue – The following violations were reported during a routine inspection: dried food debris found on can opener blade; temperatures of pepperoni and sausage in walk-in cooler are elevated; staff with long hair have hair hanging out of the hair restraint; box liners are not stored inverted to prevent contamination; walk-in cooler door isn’t closing properly; large white cutting board on wheeled cart is heavily scored and stained; observed dust buildup on fan guards and piping behind fans in walk-in cooler, back of convection oven, coils and wires, and behind computer screen in prep area; observed slight buildup on racks in dry storage area; there are dried food stains on ceiling above prep table, dry storage area ceiling and walls throughout kitchen.
Domino’s Pizza, 550 East Laburnum Avenue – The following violations were reported during a routine inspection: staff couldn’t present food protection manager certificate; hot water is turned off at dish area hand sink because the sink is leaking; pan of tomatoes in walk-in cooler is past its expiration date; food thermometer is not calibrated as necessary to ensure its accuracy; top, sides and back of pizza oven are very dusty; oven hood filters are greasy and dusty; observed dust, flour, food debris and mold on wire storage racks throughout kitchen (dish area, dry storage, walk-in cooler); hand sink next to three-compartment sink is inoperable.
Tip Thai, 8131 Staples Mill Road – The following violations were reported during a routine inspection: chemical dish machine isn’t dispensing sanitizer and test strips indicate 0 ppm bleach; several containers of curry sauce and pre-cooked noodles and rice are not date marked.
Hunan Café, 9117 Staples Mill Road – The following violations were reported during a routine inspection: person in charge couldn’t verify employee health training for staff; person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; several employee drinks found on prep shelves near clean dishes; hand sink doesn’t have paper towels; several containers of egg rolls are not date marked; bulk containers of sugar, salt and flour are unlabeled; scoops are stored in bulk food containers without handles; observed dried food debris and grime on floors under equipment; observed buildup behind grease trap.
