Five Guys Burgers and Fries, 4450-A South Laburnum Avenue – The following violation was reported during a routine inspection: inside gaskets of the make table have food debris and mold on them.
Firehouse Subs, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sink at far end of prep line doesn’t have hand soap; temperature of freshly-made sauteed onions cooling in make table is elevated; there are torn gaskets on prep line three-door reach-in and reach-in fridge next to hand sink; observed rust on drying rack next to three-compartment sink and on dry storage racks near prep line; observed heavy buildup of food debris inside prep units, on gaskets of three-door reach-in and on the inside roller of the toaster; there is trash and food debris present on front prep line and beneath equipment and prep sinks.
Tokyo Sushi Japanese, 10274 Staples Mill Road – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate, the facility’s employee health policy, or the facility’s written procedures for cleaning up vomit and diarrhea; tags for shelled scallops are stored without the date last sold; sushi rice on a time control policy is not labeled with start or discard time; lunch and dinner menus are missing the consumer advisory disclosure indicating which foods are served raw; thawed salmon is still in its reduced oxygen packaging; salt, sugar and oil are unlabled; sanitizer bucket for wiping cloths is registering 0 ppm chlorine; cardboard is lining several prep shelves throughout kitchen; cutting board at sushi station has deep scratches and scores; raw chicken is stored in plastic single-service bags instead of food-grade bags; observed dust and food residue inside bottom of reach-in freezer and on wire racks throughout kitchen; a heavy buildup of grease is dripping down from hood vents.

Crafted, 4900 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: there isn’t a certified food protection manager on staff; bar area hand sink and men’s restroom are missing handwash signgage; bar area hand sink is being used as a dump sink; observed mold and pink slime inside ice machine; dried food debris found on can opener blade and inside microwave; foods in two reach-in coolers (chicken, shaved beef, butternut squash, fry batter and chicken wings) are not holding at the required temperature; multiple spray cleaner bottles throughout kitchen are unlabeled; tuna and salmon are thawing in reduced oxygen packaging; facility doesn’t have a working food thermometer on site; bulk containers of rice, flour and sugar are unlabeled; cutting boards are heavily scored and stained; reach-in cooler on hot line is in disrepair; two lights are out in hood system; reach-in cooler and freezer door gaskets throughout restaurant are dirty; observed dried food debris on walk-in cooler wire racks; vent hood filters are dirty and greasy; there are dried food stains on kitchen ceiling; walk-in cooler ceiling is dusty.
Jimmy John’s Gourmet Sandwiches, 9018 Staples Mill Road – The following violations were reported during a routine inspection: observed an employee enter the kitchen from outside and start food prep without washing their hands; knife is stored underneath the tea dispenser; observed grime buildup at the hand sink; there is dust buildup on fan system in walk-in cooler; blue-ish liquid found in bottom of prep unit.
SB’s Lakeside Loveshack, 6935-37 Lakeside Avenue – The following violations were reported during a routine inspection: temperature of grits made two hours ago is elevated; temperatures of pickled onions, lettuce and tomatoes in prep top are elevated; chicken mixture and roasted garlic are past their expiration dates; large flies found near walk-in and the bar; there is a leak in the walk-in’s ceiling; there is a hole in the wall behind cook line three-compartment sink and in dish area; observed staining on ceiling tiles and wall under prep sink.
Family Secrets Restaurant, 7103 Brook Road – The following violations were reported during a routine inspection: observed mildew buildup on ice machine; there is 0 ppm sanitizer at dish machine; temperature of mac and cheese prepared two hours ago in walk-in is elevated because of improper cooling methods (stored in tightly covered container); noodles, greens and liver gravy are past their expiration dates; chemical spray bottles are unlabeled; there is a gap around the door; observed live roaches on hot holding units; multiple food items throughout the kitchen are uncovered; fryers and cook line equipment are covered in grease; racks in units, hot holding racks, dish area, mop area, blender container and nozzle container need cleaning; cook line floor and wall are dirty.
The Mantu Market, 7510 West Broad Street – The following violations were reported during a routine inspection: observed staff wipe face with gloved hand multiple times while preparing food but didn’t change gloves or wash hands; dried food debris found on can opener blade and base and inside microwave; several bulk spice containers are either unlabeled or mislabeled; food is stored directly on walk-in cooler and walk-in freezer floors; ice cream scoop stored in room temperature standing water; spoon stored in pan of rice with handle touching food; cutting boards are scored and stained; facility has no chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration; observed dried food debris and grease buildup on sides of hot line equipment, bread making equipment, walk-in cooler wire racks, walk-in cooler speed rack, wire racks over three-compartment sink, walk-in cooler and freezer doors, and on doors/door gaskets in prep units; observed buildup of food debris and grime on floor underneath hot line equipment and walls; ceiling panels and vents are dusty.

Buffalo Wild Wings Grill & Bar, 7801 West Broad Street – The following violations were reported during a routine inspection: three hand sinks don’t have paper towels; hand sink by walk-in doesn’t have hand soap; soda nozzle at bar, inside of units, racks, holding units and gaskets need cleaning; there is 0 ppm sanitizer at bar dish machine; observed small flies at bar; multiple containers are cracked and heavily stained; cutting boards on cook line are stained; mop sink coving tiles are cracked and split; drains and floors throughout need cleaning; observed heavy grease buildup behind cook line; there is grease on hood filters; wet mops are not hung to air dry.

Panera Bread, 4483 South Laburnum Avenue – The following violations were reported during a routine inspection: layer of black mold found on top and sides of ice machine in rear of house; sanitizer buckets along front line are registering 0 ppm sink and surface sanitizer; sanitizer at three-compartment sink is empty; temperatures of romaine lettuce and mixed green lettuce in salad drawer units are elevated; single, small prep and sandwich make table cutting boards are heavily scratched/scarred; observed heavy buildup of rust on wire storage racks above prep sink and on rolling racks on front line; fans above prep line and racks above drink prep area are dusty; observed buildup of food debris near floor drains and underneath prep units; ceiling tiles above ovens are stained and discolored; there are food stains on wall behind prep line.

Trevor’s Grill and Tavern, 4106 Williamsburg Road – The following violations were reported during a routine inspection: open employee drinks found along prep line and on storage racks within kitchen; an employee handled a cell phone and failed to wash hands afterwards; temperatures of raw burgers, chicken breasts, bologna burger and ham in reach-in drawers within make table are elevated; reach-in doors of the make table have torn gaskets; menu doesn’t contain a consumer advisory statement with asterisks next to items that may be served raw and/or undercooked; multiple employees handling food are wearing bracelets and jewelry on hands.
Qdoba, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: two hand sinks don’t have paper towels; wire storage racks in walk-in and in dish storage area are rusty; observed grease buildup on wire storage racks in rear of house next to three-compartment sink.

Popeyes Louisiana Kitchen, 4402 South Laburnum Avenue – The following violations were reported during a routine inspection: large scoop in utensil storage area has food debris and water still in the basin; sanitizer buckets throughout the kitchen are registering 0 ppm quaternary sanitizer; raw chicken wings are thawing in prep sink without water running; fryer basket has bent and broken corners; heavy buildup of food debris found on metal sheet pans; observed flour and dust buildup on racks above breading station; top of hood unit and rolling racks in walk-in are dusty.
Marco’s Pizza, 4450 South Laburnum Avenue – No violations reported during a routine inspection.
Roma Ristorante Italiano, 8330 Staples Mill Road – The following violations were reported during a routine inspection: several packages of opened and prepared foods (cooked pasta, broccoli, deli meats, sauces, soups) are not date marked; menu doesn’t contain an asterisk next to salmon and ahi tuna linking them to the consumer advisory that they may be served raw and/or undercooked; gasket and handle of walk-in freezer are in disrepair which is leading to heavy ice buildup; knife taped with duct tape; cardboard is lining the bottom of several prep shelves; a few small cutting boards have deep scratches; observed food and grease buildup on grills, stoves, fryers, shelves and inside reach-in freezer.
