The Answer Brewpub Co., 6008 West Broad Street – The following violations were reported during a routine inspection: there isn’t a person in charge with a valid food protection manager certification; raw hamburgers stored on top of ready-to-eat pot stickers and raw chicken stored over raw beef in walk-in cooler; dried food debris found in microwave; black mold found inside ice machine; observed food debris/animal fat on meat slicer; several spray cleaner bottles in kitchen are unlabeled; salmon is thawing in reduced oxygen packaging; door gaskets are torn on reach-in freezer and three-door reach-in cooler; wire racks in three-door reach-in cooler are rusty and dirty and the interior is dirty; rubber door gaskets on refrigeration units throughout the kitchen have dried food debris and mold on them; observed buildup of grease residue on sides of hot line equipment; dried food debris found in top sections of lowboy coolers; outdoor dumpster doors and lids were left open; observed grime on floor under hot line equipment; there are food stains on wall behind hot line and on wall behind prep sink.

Silver Diner, 10890 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or proof that staff are trained on the “Big 6” illnesses and associated symptoms; employees are chewing gum and eating food in the dishwash area; employee handled cellphone and didn’t wash hands before continuing food prep; pink/black residue buildup found inside ice machine; temperature of sausage links on service line is too low; temperatures of meatloaf slices, pancake batter and raw eggs are elevated; can of Rust-oleum stored near clean equipment; employees working in food service area are not wearing proper hair restraints; employees are wearing watches while handling food; several wet wiping cloths found on prep surfaces instead of being stored in sanitizer buckets; several utensil handles are stored in contact with food; in-use utensils at the grill are stored in ambient temperature standing water; prep and single cutting boards are heavily scratched and stained; observed dried food debris and grease on storage racks, under grills and inside toaster; old label residue found on containers; proper backflow prevention device is not installed on mop sink with a spray nozzle under pressure; there are leaks on hot water handle at spray sink and chemical dispenser at mop sink; observed areas of low grout on floor throughout kitchen; there is broken tile coving near mop sink and walk-in cooler.

Hyderabad Biryani House, 4040-H Cox Road – The following violations were reported during a routine inspection: front hand sink doesn’t have hand soap; there is 0 ppm chloride sanitizer in dishwasher; cooked rice in walk-in refrigerator isn’t date marked; facility doesn’t have a food thermometer; two bulk containers storing white powdery substances are unlabeled; prepared food items in containers are stored directly on walk-in refrigerator floor; utensils next to stove are stored in ambient temperature standing water; take-out containers are stored uninverted above prep unit; gasket is torn on reach-in prep unit left-side door; fryer baskets are in disrepair with broken metal pieces; foil lining is being used under prep tables throughout the kitchen; prep unit cutting board is heavily scored and stained; racks in walk-in cooler are covered in heavy dark debris; vents in walk-in are dusty; ingredients shelf and drying rack above three-compartment sink are dirty; wall behind three-compartment sink is covered in mildew; mops are not being hung to air dry.

Dragon Treats Dessert Café, 1420 North Parham Road – No violations reported during a routine inspection.

Chipotle Mexican Grill, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: dried food debris found on “clean” containers on dish rack; temperature of chicken in hot holding unit is too low; observed small flies by grill, mop sink and ice machine; there is 0 ppm sanitizer in sanibuckets; bowls and lids are not stored inverted; chip bags are stored directly on the floor; observed food and other debris in units and on racks on the serving line; observed buildup on cooking equipment; wall by to-go serving line and three-compartment sink needs cleaning; there is food and debris buildup on floors and under equipment.

Bubba’s Bar & Grill, 4000 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have a written employee health policy or written procedures for cleaning up vomit or diarrhea; hand sink in prep area doesn’t have hand soap; observed a presence of dead North American cockroaches and multiple full bait traps within the kitchen; prep and single cutting board are heavily scratched; observed food and grease buildup on backsplash behind fryers and underneath grill and fryers; there are missing ceiling panels throughout kitchen and storage areas; there are holes in the wall; observed damaged and missing coving and broken floor tiles under three-compartment sink and prep table.

Chop’t Creative Salad, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: metal container found in hand sink; temperature of prepped romaine greens in walk-in is elevated because of improper cooling methods (stored in covered, deep containers and stacked); dry storage rack in dish room and wheels on rolling storage rack and prep table are rusty.

Breaker’s Sports Grille, 9127-V West Broad Street – The following violations were reported during a routine inspection: nonsmoking bar hand sink doesn’t have hand soap; ice found in hand sink at nonsmoking bar; kitchen hand sink doesn’t have paper towels; dish machine has 0 ppm chlorine; dirty containers stored on rack with clean dishes; sliced turkey, sliced ham, pasta salad, potato salad and cooked potatoes are past their expiration dates; menu is missing consumer advisory statement for classic burger, bacon burger and egg platters; ice scoop handle is touching ice at nonsmoking bar; ice scoop is stored on a dirty surface of ice machine; there is unsealed bare wood under steamer and under three-compartment sink in kitchen; beverage tubing is in contact with ice at the waitress station; can opener handle is loose; prep unit lid is unable to stay open; sides of cook line equipment and floors of refrigeration units at bar are dirty; microwave is dusty; observed encrusted food debris on pizza pans; ceiling panels and walls near dish machine and three-compartment sink are dirty; ceiling vents in kitchen and floor under three-compartment sink at smoking bar are dirty.

McDonald's, 7527 Staples Mill Road – The following violation was reported during a routine inspection: corners are bent on three fryer baskets.

Kung Fu Tea, 10827 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; front hand sink doesn’t have paper towels; front dump sink has a paper towel dispenser attached in close proximity and a handwash sign above it (if the sink is designated for dumping, all handwashing aids must be removed from area); plastic tarp and plastic chemical bucket found in hand sink in back of kitchen; specks of mold are growing inside ice machine; dark boba in plastic container in reach-in prep unit is not date marked; squeeze bottle of dish soap is unlabeled; facility doesn’t have a food thermometer; plastic liner on ingredient rack is cracked and in disrepair; facility doesn’t have chloride or quat sanitizer test strips; drying shelf above three-compartment sink is covered in thin layer of boba tea residue; mop isn’t hung to air dry.

Chipotle Mexican Grill, 10501 West Broad Street – The following violations were reported during a routine inspection: single-service boat containers are stored uninverted and unprotected at both serving lines; dish machine pipe is broken; red and green cutting boards are heavily scratched.

Burgerworks, 10321 West Broad Street – The following violations were reported during a routine inspection: two employee drinks stored next to warmers and toaster; observed heavy white/yellow buildup on interior panel of ice machine; temperatures of chili and bacon in warmer are too low; menu doesn’t contain a consumer advisory disclosure for fried egg; pans are stacked while still wet.

Waffle House, 3202 East Parham Road – The following violations were reported during a routine inspection: several containers of prepared pancake mix are not date marked; eggs are held on a time control policy but time is not being logged; to-go containers stored with food contact side down on shelves with food residue; observed employees using the same gloves for different tasks (touching raw eggs and then ready-to-eat sandwiches and touching dirty dishes and then clean dishes); coffee makers and juice dispensers are dusty; observed grease and food debris buildup on grills, crevices of oven, wire racks and fan grate in walk-in cooler, and inside reach-in units; observed grease, dust and food residue on floors underneath equipment, the floor drains, and the walk-in cooler.

Charleys Cheesesteak and Wings, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; front hand sink doesn’t have hand soap; spray bottle on chemical shelf is unlabeled; chicken wings cooked this morning are stored in a large, deep container with lid on; several boxes of food are stored directly on walk-in freezer floor; gaskets are torn on reach-in lowboy freezer and both front reach-in prep unit doors; one of the fryer baskets has a broken metal piece; backflow prevention device isn’t installed on mop sink faucet; dumpster area is surrounded by loose trash and debris.
