Rare Olde Times, 10602 Patterson Avenue – The following violations were reported during a routine inspection: employee drink cups are stored on cutting boards; employee handled dirty dishes and failed to wash hands before handling clean dishes; pink residue buildup found inside ice machine; Scottish egg, cauili wings, corn beef and chicken breast are past their expiration dates; back chest freezer door is damaged; metal wires are bending and breaking on fryer baskets; prep cutting board is heavily scratched and stained; observed heavy food and grease buildup on surfaces of broiler, fryers and oven; observed buildup of food debris, grease and dust on surfaces of meat storage shelf, hanging pot rack, grease trap, fan system in cooler, and inside utensil bins; floor behind cook line is greasy; walls are stained under dish wash area.

Original Gino’s Pizza, 10444 Ridgefield Parkway – The following violations were reported during a routine inspection: person in charge’s food protection manager certification is expired; cutting board at make table is scratched and stained; dry storage rack and cleaning supplies rack are rusty; heads of lettuce are stored in a five-gallon non-food grade bucket.

Twin Hickory Tavern, 11351 Nuckols Road – The following violations were reported during a routine inspection: there isn’t a certified food protection manager on staff; facility hasn’t established procedures for cleaning up vomit or diarrhea; no handwash signage is posted at any hand sinks; raw shell eggs stored over cooked pasta and raw chicken stored over raw beef in reach-in coolers; dried food debris found inside microwave; pink slime found inside ice machine; observed mold inside far-left soda gun at bar; observed food residue/debris on immersion blender, vegetable chopper, meat slicer blade and salad spinner; temperature of chicken tenders in bowl next to deep fryer is too low; temperatures of chicken wings in reach-in cooler and sliced cheese in prep table are elevated; there is no written time control policy for onion rings; flour next to deep fryer has no discard time posted; facility doesn’t have a food thermometer on site; bulk containers of flour and rice are unlabeled; observed fruit flies throughout kitchen; fly paper is placed above the beverage station; food is stored directly on walk-in cooler floor; ice cream scoop is stored in ambient temperature standing water; to-go containers in kitchen and on serving line are not stored inverted; plastic hummus containers are being reused in the kitchen; containers are worn to the point of cracking; reach-in cooler door gaskets throughout kitchen are torn; fryer baskets are in disrepair; wire racks in walk-in cooler and dry storage are rusty; cutting boards throughout kitchen are scored; wire racks and shelves above prep units are dusty; observed food and grime buildup on wire racks, fan grate and beer lines in walk-in coolers; observed grease residue on sides of hot line equipment; mop sink and prep sink don’t have dual check valves installed; outdoor dumpster doors were left open; walk-in cooler floor is no longer smooth and easily cleanable and the paint is worn down; observed dried food stains and debris on dish room walls and behind hot line; ceiling tiles near ventilation are dusty.

Domino’s Pizza, 11442 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; reach-in refrigeration racks and drying racks are rusty; ingredients rack is dusty; dumpster area is surrounded by trash and loose debris.

Hardee’s, 3201 East Parham Road – The following violations were reported during a routine inspection: employee drink found on prep table; breading trays and food containers are stored dirty in storage area (there are old sticker labels, food crumbs and grease on them); two buckets of sanitizer solution stored directly on the ground; fryer baskets are bent and broken; metal containers are bent; observed food debris and grease buildup on fryers, ovens, grills, broiler and broiler rack, bread toaster, cables, monitors, hot hold units, prep tables, walk-in cooler racks and fan grates, rolling racks, inside reach-in freezer and inside wrapper holders; gray standing water in the mop sink is unable to drain; there is heavy buildup of food debris, grease and grime on floors and walls throughout the establishment as well as walk-in coolers, mop sink, dry storage, under syrup lines, electrical area, and behind and around equipment; observed heavy dust buildup on ceiling panels and vents above food prep areas.

Texas de Brazil Churrascaria, 11800 West Broad Street – The following violations were reported during a routine inspection: plastic bucket found in bar hand sink; there is 0 ppm sanitizer in bar dishwasher; there is caked on residue along interior of soda nozzle in bar; temperatures of salmon and cole slaw in the cold bar are elevated; menu doesn’t contain an asterisk next to smoked salmon; drying racks in dishwasher area are rusty; two mops are not hung to air dry.

Komma Tea, 11800 West Broad Street – The following violations were reported during a routine inspection: employee health policy was unavailable; facility couldn’t provide written procedures for cleaning up vomit or diarrhea; front hand sink doesn’t have hand soap; facility doesn’t have a food thermometer; bulk bin containing sugar is unlabeled; mop isn’t hung to air dry.

Wok N Roll, 7514 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food manager certification; employee failed to wash hands after re-entering the kitchen from outside; containers of food are stored directly on walk-in cooler and walk-in freezer floors;

Healthy Mixes Herb Spot, 5203 South Laburnum Avenue – No violations reported during a routine inspection.

Maggiano’s Little Italy, 11800 West Broad Street – The following violations were reported during a routine inspection: small bar hand sink doesn’t have hand soap; soiled scoop found in hand sink on cook line; no paper towels are available at hand sinks at main bar, small bar and in dishwashing area; dishwasher isn’t reaching the required temperature in order to sanitize properly; there is 0 ppm chloride at glass dishwasher; temperature of pasta in refrigeration drawer is elevated; gaskets are torn on first refrigeration unit on prep line; plastic serving trays are chipped and in disrepair; light fixture in fourth walk-in refrigerator is broken and off the hinges; three utensils have melted handles; stainless steel containers are dented; cutting boards on prep line are scored; refrigerator vents are dusty in first and fourth walk-in; prep shelf is covered in a thick layer of black debris; drying rack in dishwasher area is covered in old food residue; pot sink faucet is leaking at the base; mop isn’t hung to air dry; ceiling vents and tiles in kitchen are dusty.

Live Crawfish & Seafood, 9020 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; raw scallops stored over boiled eggs and sausage; to-go containers above microwave are stored uninverted; underside of wall shelf above butter warmer is dirty; floor fan is dusty.

Salaam Market LLC, 7103 Staples Mill Road – The following violations were reported during a routine inspection: open sundried tomatoes are not date marked; chemical bottle under mop sink is unlabeled; gaskets are broken at bottom of standing refrigerator; standing freezer floor is chipped; mop sink in back is chipped and broken; there is cardboard and foil lining throughout the kitchen; bottom of display case and ceiling vent above kitchen are dirty; three-compartment sink has a direct plumbing connection; ceiling tiles in kitchen are broken; slicer stored in kitchen is not being used so it should be removed.

Little Szechuan Restaurant, 10234 Staples Mill Road – The following violations were reported during a routine inspection: raw beef stored over tofu in lowboy; black buildup found inside ice machine; temperatures of BBQ pork, sprouts, tofu, raw beef skewers, garlic in oil and fried chicken in various refrigeration units are elevated; boxes of food stored directly on walk-in freezer floor; plastic single-service containers are being used as scoops inside bulk corn starch bin; black to-go containers are stored uninverted above prep unit; observed buildup on walk-in cooler shelving and in between cook line equipment; fan grates in walk-in cooler are dirty; floor tiles in walk-in cooler are broken.

Subway Sandwiches & Salads, 4014 Glenside Drive – The following violation was reported during a routine inspection: observed buildup of dust on fan inside walk-in cooler.

Jersey Mike’s Subs, 8654 Staples Mill Road – The following violations were reported during a routine inspection: stuck on dried lettuce found on slicer; temperatures of ham, Swiss cheese and turkey in display case are elevated; bottles of water, oil and vinegar are in unlabeled bottles; there is 0 ppm quat sanitizer in sanitizer bucket near grill; observed dust buildup on standing fan and mini fan.

Playa Bowls, 11341 West Broad Street – The following violations were reported during a routine inspection: there is 0 ppm quat sanitizer in three-compartment sink; two styrofoam cups of oatmeal in reach-in prep unit are not date marked; squeeze bottle containing brown liquid in reach-in is unlabeled; mop isn’t hung to air dry.

Roberto Italian Restaurant, 5550 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; plastic drink glasses and serving spoon found in hand sink next to drink station; raw eggs stored above ready-to-eat vegetables and prepared lasagna in walk-in refrigerator; sanitizer in dish machine is dispensing 0 ppm bleach; prepared pasta noodles, meat sauce, marinara, lasagna and chicken in walk-in cooler aren’t date marked; temperature of spaghetti in walk-in is elevated because of improper cooling methods (in a tightly covered deep pan); two in-use knives found stored between prep tables; several stacks of to-go trays and containers on cook line and prep shelf are stored uninverted; there are multiple rusty racks in walk-in fridge; ceiling tiles in prep area are cracked; vent filters in the hood system are bent, broken and with an exposed gap between filters.
