Taco Bell, 7210 West Broad Street – The following violations were reported during a routine inspection: utensils and fryer baskets are stored on top of grease-stained cardboard; observed a layer of dust buildup above ovens and on single-service items rack.

TGI Friday’s, 4459 South Laburnum Avenue – The following violations were reported during a routine inspection: no handwash signage is posted at bar area hand sink; observed heavy buildup of grease and rust within rear industrial oven; buildup of food debris found inside microwave; temperatures of raw chicken and raw shrimp in tabletop prep unit are elevated; cutting boards on make table are heavily scarred and stained; small cutting boards are heavily scarred; rusty racks found in walk-in fridge, dry storage room and service area; tabletop prep unit has broken hinges; there are torn gaskets on two reach-in fridge units along prep line and on bar reach-in unit; observed food debris and grease buildup on wire racks, wheels of fryers, cables and monitors; there is dust buildup on walk-in cooler fan system; observed a slow leak with standing water present underneath bar prep sink and ice storage; ceiling tiles in dry storage area are damaged; there is low grout on floors; some floor tiles are missed/damaged; observed buildup within floor drains; AC vents and ceiling tiles are dusty; there is grease and food debris on floor under units in kitchen.
Noodles & Company, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: spray bottle with blue liquid inside is unlabeled; spray bottle of cooking oil by the stove is unlabeled; shelf storing take-out containers is covered in thick black debris; dumpster area is surrounded by loose trash and debris.
Bambu Drinks & Desserts, 4040-F Cox Road – The following violations were reported during a routine inspection: three spray bottles with clear liquid inside are unlabled; cardboard is lining the ingredients rack next to the hand sink; drying racks above three-compartment sink are peeling; square cutting board is stained and heavily scored; drain underneath three-compartment sink is covered in black debris.
The Daily Kitchen & Bar, 12201 West Broad Street – The following violations were reported during a routine inspection: automatic hand sink faucet isn’t working properly; food debris found on slicer blade; observed buildup of mold-like substance inside ice machine; high temp dish machine isn’t reaching the required sanitizing surface temperature; temperature of chicken that just came off the grill is too low; temperatures of pickled onions, pico, lettuce, chicken, sliced and cut tomatoes, beet hummus, cut cabbage, half-and-half, milk, raw chicken and raw steak in multiple make tables and other units are elevated; several refrigerator units are not maintaining foods at the proper temperature; no date marks found on pickled onions or roasted garlic; cooked chicken in make table and pickled cabbage in walk-in are past their expiration dates; tuna pieces are thawing in reduced oxygen packaging; utensils throughout the kitchen and bar are stored in ambient temperature standing water; ice paddles are stored directly on racks in walk-in unprotected from contamination; food storage containers are stacked while still wet; food debris found on mixer; observed grease buildup and food debris on floor and equipment at cook line; light is out in the hood; a couple of light shields in the kitchen are broken; employee’s personal care belongings are stored on drainboard at bar.

Chipotle Mexican Grill, 8675 Quioccasin Road – The following violations were reported during a routine inspection: temperature of pan of brown rice is too low; temperature of brisket in small lowboy cooler is elevated; air temperature of this unit is also elevated; red cutting board on prep table is scratched and stained; dish machine isn’t working.
Anna’s Pizza Pasta and Subs, 601 North Airport Drive – The following violations were reported during a routine inspection: staff are washing hands in the three-compartment sink instead of at designated hand sinks; staff handled raw chicken and moved onto other tasks without changing gloves and washing hands; staff also handled raw chicken and washed gloved hands; men’s restroom doesn’t have paper towels; paper towel dispenser in women’s restroom isn’t working; raw shell eggs are stored over deli meats and imitation crab meat is stored amongst raw chicken in cooler unit; black stains/mold found inside ice machine; temperatures of tomatoes, lettuce, turkey and hamburger in reach-in cooler are elevated; this unit needs servicing; facility doesn’t have a food thermometer in the kitchen; double-door reach-in cooler fan guards are dirty and dusty; walk-in freezer door gaskets are moldy and stained black; salad cooler door gaskets are dirty; spaces between hot line equipment have grease residue on them; ceiling tile in front of house area is stained; ceiling vent in kitchen is dusty; there are food stains on wall behind slicer and hot line equipment.
Red Salt, 12221 West Broad Street – The following violations were reported during a routine inspection: employee touched cooked ground beef mixture in walk-in with bare hands to check the temperature; sushi bar hand sink doesn’t have paper towels; raw beef stored over tempura in breading reach-in unit; food debris found on slicer blade; temperature of beef mixture in walk-in is elevated because of improper cooling methods (stored in deep covered container); temperatures of greens in a bowl, blue cheese, corn, tomatoes, raw chicken, buttermilk, raw steak, pasta, spinach dip, shredded cheese, toasted peppers, brussel sprouts in portioned bags, hollandaise and garlic butter in several units are elevated; tempura batter is on a time control policy but time isn’t being tracked; person in charge states that the breading mixture used for raw chicken is on time control but it isn’t on the written policy; two chemical spray bottles at the bar are unlabeled; temperature of forbidden rice prepared an hour ago in lowboy is elevated because of improper cooling methods (stored in covered container); observed small and large flies in kitchen and small flies at bar; boxes of food are stored directly on walk-in floor; containers of ice cream in the chest freezer are stored uncovered; sugar and cherries on the bar are stored uncovered; wet wiping cloths found on cutting boards instead of in a sanitizer solution; knives are stored in crevices between units; utensils at bar are stored in soda water; bowls are being used as scoops in bulk food containers and they are stored in the product; take-out containers in back storage room are unwrapped and uninverted; soiled, damaged aluminum foil is lining storage shelf on end of cook line; cutting boards at bar, sushi bar and in kitchen are heavily scratched; tempura storage rack is soiled with grease and food debris; observed rust on racks in lowboy and storage shelf near expo; fans in the walk-in are dusty; there is a leak from a pipe under the dish machine; observed grease buildup on floor and pipes at the cook line; floor drain under three-compartment sink is soiled; there are damaged coving tiles behind three-compartment sink and damaged floor tiles under the fryer; several lights are out in the hood system.
Mezeh Mediterranean Grill, 11710 West Broad Street – The following violations were reported during a routine inspection: kitchen hand sink doesn’t have soap; temperature of rice is too low; several stainless steel containers of cooked rice are covered with plastic wrap and stacked on top of each other; foil is lining bottom shelf of the prep table; right-side door gasket on lowboy under conveyor oven is torn; warming unit next to cook line is maintaining foods at the proper temperature; ingredients rack above prep sink is covered in old food residue; prep sink in kitchen has a direct plumbing connection; faucet at mop sink doesn’t have a dual check valve installed; dumpster area is surrounded by loose trash and debris.
Mexico Restaurant, 11621 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide the employee health policy; prep sink in kitchen is doubling as a hand sink; spray bottle with cloudy liquid inside is unlabeled; foil lining is being used on the stove, next to the prep unit and under prep tables; plastic liners used on walk-in refrigerator racks are chipped and cracked; drying racks in dishwashing area are rusty; dry ingredients rack above prep sink is covered in food residue; mop sink faucet doesn’t have a dual check valve installed.

Roma’s Italian Restaurant, 325 East Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; the date on which the last shellstock from the container was sold or served was not recorded on the tag/label; raw oysters stored over ready-to-eat foods in walk-in; light is out underneath hood system.
Sistar Seafood, 4844 South Laburnum Avenue – The following violations were reported during a routine inspection: no supervisors or managers have food protection manager certification; raw eggs and shrimp are stored over ready-to-eat items in walk-in refrigerator; multiple skimmers, utensils and bowls are stored “clean” with visible food debris on them in dry storage area; buckets of steamed crabs are stored directly on walk-in floor; cutting board at fish prep station is heavily scored and stained; multiple wire storage racks in front and back of kitchen are rusty; observed food debris and grease buildup on wire storage racks, reach-in gaskets, sliding fridge door hinges, scales and inside prep units; ceiling tiles around fryer are stained; multiple areas in kitchen have cracked floor tiles.
The Cocky Rooster, 12171-D West Broad Street – The following violations were reported during a routine inspection: no handwash signgage is posted at hand sink; temperatures of pepper jack cheese, provolone, pickle slaw and sriracha slaw in the make table are elevated; make table is not maintaining foods at the proper temperature (person in charge discovered ice buildup); mop sink faucet doesn’t have a dual check valve installed; pipes, floor and floor drain in dish area are soiled; wet mop isn’t hung to air dry.
Red Robin America’s Gourmet Burgers & Spirits, 11784 West Broad Street – The following violations were reported during a routine inspection: the employee health policy is not available; raw turkey burgers are stored in the same refrigeration drawer as ready-to-eat ribs; residue buildup found on front panel crevices of ice machine; gasket is torn on pizza refrigeration unit; reach-in unit in the bar is leaking; prep unit cutting board is stained and scored; dumpster area is surrounded by loose trash and debris.
Chicken Salad Chick, 9631 West Broad Street – The following violations were reported during a routine inspection: temperature of pasta salad in sandwich unit is elevated; foods nearest the bread toaster in sandwich unit have elevated temperatures (per person in charge, lunch rush had just ended and the unit’s lid was lifted for an extended period); observed several small flies throughout the kitchen.
Popeyes Louisiana Kitchen, 8121 Brook Road – The following violations were reported during a routine inspection: temperature of chicken in walk-in is elevated; heavy ice buildup found on walk-in fans; no time stickers found on cheese; observed live roaches on breading station and dead roaches in dish area; flies found in main kitchen; observed debris in units; observed grease on tacks/hot holding units and cooking equipment; mop sink doesn’t have a dual check valve installed; drains are in need of cleaning; observed grease and food debris under fryers and breading stations.
Einstein Brothers Bagels, 1368 Gaskins Road – The following violations were reported during a routine inspection: single-service containers are stored uninverted above dump sink; mop sink doesn’t have a dual check valve installed; ceiling panels in back kitchen are stained; wall behind oven in back kitchen is dusty.
