Wendy’s, 4026 Glenside Drive – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy, written verification that staff has been trained on employee health, or written procedures for cleaning up vomit and diarrhea; person in charge couldn’t name the “big six” reportable foodborne illnesses and the associated symptoms; open employee drink found on make table near food and single-service items; an employee at the grill cracked raw eggs and then dipped his hands into the sanitizer bucket used to store cloths that wipe food contact surfaces; temperature of chili in front warmer is too low; time isn’t being tracked for container of sliced cheese at make unit; observed infestation of small flies under beverage cabinet in dining area and around drain tubes and pipes; an employee is handling exposed food while wearing a watch; employees are working with exposed food while not wearing proper hair restraints; observed food debris and dust buildup on surfaces of wire storage racks and fan system in walk-in cooler, monitors, cables, touchpads, and inside wrapper holders and biscuit warmer; there is a leak at chemical dispenser faucet at three-compartment sink; light fixture in outdoor walk-in cooler isn’t functioning correctly; light fixture above chili warmer on front counter is hanging from the ceiling and the bulb is out.
China Panda, 8970 Quioccasin Road – The following violations were reported during a routine inspection: an employee came in through the back door of the kitchen and went straight to cooking equipment on the stove without washing hands first; raw pork in stainless steel bowl stored above ready-to-eat sauces in walk-in refrigerator; old food debris is covering the interior, racks and bottom of both reach-in refrigeration units; no date marks found on prepared noodles, egg rolls or dumplings in reach-in refrigeration units; stainless steel containers sitting next to the stove have three different white, powdery substances in them but no labels; shelf storing takeout containers is covered in heavy dust buildup; both cutting boards on the prep units are scored and stained; both doors of left most lowboy have torn gaskets; walk-in refrigerator gasket and standing refrigerator door gasket are torn; fryer baskets are broken; stove is in disrepair (siding is melting and has a heavy amount of burnt-on grease); walk-in refrigerator vent is dusty; walk-in refrigerator racks are covered in old food debris; ingredient racks in back of facility are covered in dust and old food debris; drying racks above three-compartment sink are covered in heavy layer of dust; freezer floor is uneven and cracked; ceiling tiles throughout kitchen are missing; kitchen floor has cracks in front of stove and across from walk-in freezer; mop isn’t hung to air dry.
Taco Bell, 1330 North Laburnum Avenue – The following violations were reported during a routine inspection: observed mold on drive-thru soda machine (ice dispenser and drain spouts); cutting boards are scored and stained; observed grease residue on side of reach-in freezer; make table door gaskets are dirty and moldy; reach-in freezer door gaskets and dry storage racks behind assembly line are dirty; outdoor dumpster door was left open; observed dried food/drink stains on walls behind assembly line and in drive-thru area.

Starbucks Coffee Company, 8631 Quioccasin Road – The following violations were reported during a routine inspection: equipment found in hand sink in back area; food debris found in hand sink in front; temperatures of whipped cream and milk are elevated; rack in lowboy under bean grinder is rusty; lowboy under syrup bottles has a torn gasket; bottom of reach-in cooler storing milk is dirty.
Tropical Smoothie Café, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: plastic bins are dirty, stained and scratched; plastic bins throughout kitchen are cracked; tortilla holders, blender bottoms and can opener/base are dirty; temperatures of kale salad and yogurt bowl in prep unit are elevated; observed imcorrect sanitizer concentration in bucket with wiping cloths; ice bucket is not inverted to dry; cutting boards are scratched and stained; storage racks are rusty throughout; facility couldn’t provide quat sanitizer test strips; storage racks, floor fan and area under prep sink are dirty; dripping juices from prep table are spilling onto lowboy cooler which has standing water and mold on it; mop isn’t hung to air dry; wall corners are not smooth and easily cleanable; floors throughout the kitchen and floors in walk-in cooler and freezer are dirty; wall behind fruit prep table and air vents throughout the kitchen (including in walk-in cooler) are dirty.

Red Robin America’s Gourmet Burgers & Spirits, 10067 Brook Road – The following violation was reported during a routine inspection: observed buildup in drains and under equipment on front service line and at bar.
Hawk’s BBQ & Seafood, 8191-B Brook Road – The following violations were reported during a routine inspection: observed mold and mildew buildup in ice machine; multiple containers on clean dish rack are still dirty with flour and grease; small flies found in back kitchen; single-service container is stored uninverted; observed heavy buildup of dirt and grease on sides of equipment, racks and all surfaces; there is dirty standing water in units; there is dirt and debris under equipment and on walls, floors and ceilings; observed heavy buildup on and around mop and three-compartment sink; there is grease and dust on hood and hood filters.
JS Lakeside Tavern, 5406 Lakeside Avenue – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; hand sink is blocked and has dishes in it; mold found inside ice machine; temperatures of half-and-half and creamer in bar area mini fridge are elevated; facility doesn’t have a food temperature measuring device; a container of white powder in kitchen is unlabeled; front door was left open; observed several flies inside restaurant; cutting boards in kitchen are stained and scored; lowboy cooler is in disrepair; hood filters are greasy; observed grease residue on sides of cooking equipment and on lowboy cooler; walk-in cooler fan guards are dusty; bar area keg cooler door gaskets are dirty; there is damaged kitchen floor coving under three-compartment sink and in front of walk-in cooler; dried food stains found on wall under three-compartment sink; there are food stains and grease residue on hot line walls, particularly back corner near deep fryer.

Applebee’s Neighborhood Grill & Bar, 9601 West Broad Street – The following violations were reported during a routine inspection: temperature of fries is too low; temperature of lettuce in unit is elevated; air temperature of this unit is elevated too; plastic bin storing lettuce is cracked; seals are loose on lowboy units on cook line; there is a broken top panel inside microwave above ice cream freezer; observed buildup on wire racks in lowboy units along cook line; floor tiles on cook line and in walk-in cooler are broken; there is low grouting throughout the kitchen.

IHOP, 4840 South Laburnum Avenue – The following violations were reported during a routine inspection: employee in prep area rubbed nose and failed to wash hands afterwards; temperature of hash browns on the hot line is too low; there are no thermometers in lowboy or reach-in units on prep line; wire racks in dry storage room are rusty; cutting board on make table is heavily scored; observed buildup of food debris and mold on green wire storage racks in walk-in; there is food debris buildup on waffle iron; there is a steady leak at the AVB on the mop sink; mop sink with hose attached held at pressure without a dual check valve installed; multiple ceiling tiles in seating area and prep area are soiled and stained; AC intake is dusty.

Lakeside Barbecue LLC, 6920-E Lakeside Avenue – The following violations were reported during a routine inspection: didn’t observe a ServeSafe certificate; hand sink doesn’t have hand soap or paper towels; no date marks found on barbecue, chicken or cheese; sanitizer bottles are unlabeled; interiors of units need cleaning.
Melito’s, 8815 Three Chopt Road – The following violations were reported during a routine inspection: a drink without a lid found stored on prep shelf above meat slicer; employee handled phone and failed to wash hands and change gloves before continuing food prep; an employee is handling food while wearing a bracelet; shelf liner on dry storage racks is cracked; there is rust on the walk-in freezer floor; single cutting boards are scratched and stained; the crates used for clean lids is dirty; observed clean bins with label residue remaining; floors in bar area are dirty; mop isn’t hung to air dry.
Tacos El Guero, 8026 West Broad Street – The following violations were reported during a routine inspection: employee left mobile unit and didn’t wash hands or change gloves upon returning; employee is handling bread with bare hands; temperatures of two small pans of refried beans and a small pan of rice are too low; cooked beef held for more than 24 hours in prep unit isn’t date marked; personal medicines and sanitizer tabs are stored on a shelf with spices; cooked chicken, beef and cactus are stored in non-food grade grocery store bags; prep unit cutting boards are heavily scratched and pitted.
