Plaza Azteca Mexican Restaurant, 10099 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a person in charge with a food protection manager certification; uncovered staff drink found in reach-in cooler next to food; dishes found in hand sinks in dish pit and at bar area; no handwash signage is posted at hand sinks in employee bathroom, on hot line or in dish area; observed mold inside ice machine; temperature of white rice hot holding is too low; chorizo is thawing at room temperature in kitchen sink; fish is being thawed in reduced oxygen packaging; ice scoops are stored in ice bins with handles touching ice; cutting boards throughout kitchen are scratched and stained; tortillas are stored in non-food grade plastic grocery bags; food debris found on reach-in cooler door gaskets and walk-in cooler wire racks; walk-in cooler fan guards are dusty; there isn’t a trash can with lid in the employee restroom; there is low grouting throughout the kitchen; walk-in cooler has missing/cracked floor tiles; a few ceiling tiles are missing.

Casa Grande Restaurant, 10120 Brook Road – The following violations were reported during a routine inspection: multiple items (chips, cinnamon sugar, taco shells) are uncovered in the kitchen and in units; temperature of some rice is too low; temperatures of raw chicken, beef and shrimp in drawers under cook top are elevated; observed a monitoring device in dry storage filled with multiple roaches, dead and alive; single-service containers are not stored inverted to prevent contamination; there are torn gaskets on salsa reach-in, prep unit on line, and grill drawers; gaskets, bar hand sink and walk-in shelves need cleaning; there is low grouting, cracked coving tiles, missing trim and holes in walls.
Chick-fil-A, 10176 Brook Road – The following violations were reported during a routine inspection: there isn’t hot water at hand sink across from dishes; this hand sink is leaking; clean containers are stacked while still wet on dish rack.
MPM Tiki Bar, 2451 Old Brick Road – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit or diarrhea; paper towel dispenser at back hand sink is empty; no handwash signage is posted at bar hand sink; person in charge couldn’t provide parasite destruction documentation from salmon provider; dried food debris found on can opener blade; observed black mildew buildup inside all three ice makers and at ice bins at the bar; temperatures of cut tomatoes and a small bowl of cooked chicken in prep units are elevated; no date marks found on bins of pickled cabbage and sauces; menu doesn’t contain an asterisk linking the disclosure statement to foods that can be served undercooked or raw (tuna, salmon, burgers); raw chicken is thawing at room temperature; bottles of extracts at the bar and sauces are unlabeled; observed small flies throughout kitchen and bar area; a wiping cloth is stored in contact with cooked chicken in prep unit; rice scoop is stored in ambient temperature standing water; in-use knives are stored in dirty crevice of prep units; prep unit cutting boards are heavily scratched and stained; knife on strip is chipped; metal wires are bending and breaking on fryer baskets; walk-in cooler wire storage racks are rusty; one fryer is in disrepair; observed buildup of food debris and grease on surfaces of fryers, storage racks, carts, hoods, bulk flour bins, under grills and inside prep units; faucet at hand sink is leaking; women’s restroom doesn’t have a covered trash receptacle; concrete and wood floors are not sealed (pain chipping); ceiling vents are dusty; there are food debris and grime on floors throughout kitchen and in walk-in cooler and freezer; two bulbs are out in the hood system.
Capital Ale House, 4024-A Cox Road – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit or diarrhea; no paper towels provided at hand sink by small bar; shellstock tag wasn’t included in bin of mussles in the middle lowboy unit; both soda nozzles in the small bar and the ice machine in the kitchen are dirty; temperature of nacho cheese is too low; spray bottle with small amount of reddish liquid under three-compartment sink is unlabeled; two knives are stored in between left prep unit lids; all three long prep unit cutting boards and all small cutting boards in kitchen are stained and deeply scratched; there are torn gaskets on three lowboy units, reach-in freezer under breading station and right walk-in refrigerator; left walk-in refrigerator door is cracked along the interior; ingredient racks are rusty; four fryer baskets are broken; there is black debris in crevices of walk-in refrigerator vents; observed heavy grease buildup on sides of equipment on cook line; storage rack to the left end of the cook line is covered in heavy black debris; drying rack is dirty with caked on food debris; prep shelf holding entrée plates is covered in heavy layer of old dust; two mops are not hung to air dry; there is a hole in the wall on the right side of the hand sink in main bar; floor has cracks in it at the end of the first set of reach-in refrigeration units in main bar; drains under all prep sinks are dirty from heavy use.
Arby’s, 5220 Williamsburg Road – The following violations were reported during a routine inspection: observed a layer of grime/black mold on door of ice machine and behind the hinges; cutting surfaces at make table are heavily scratched and discolored; racks storing meat in walk-in fridge are rusty; there is heavy buildup of rust on wheels of rolling racks; observed buildup of dust on monitors and cables in kitchen prep area and AC vents; there are cracked and missing coving tiles underneath three-compartment sink and rear door.
Taco Bell, 4388 South Laburnum Avenue – The following violations were reported during a routine inspection: soiled cardboard is being utilized under fryer baskets next to fryers; cutting surfaces on prep line are heavily scratched and stained; observed heavy dust buildup on AC vents in kitchen prep area.

Captain D’s Seafood, 7607 Staples Mill Road – The following violations were reported during a routine inspection: salmon was thawed while in reduced oxygen packaging; two ice buckets are stored right side up while still wet; single-service to-go containers are stored uninverted and unprotected from splash at hot holding area; hand sink basin and racks in fish prep reach-in cooler are cracked; there is a torn ceiling panel above soda dispenser at front counter; there are broken wall tiles behind fish prep reach-in coolers, next to fryers, and behind prep sink; floor tiles are cracked while with low grouting throughout the kitchen.
Angela’s Ristorante, 425 North Ridge Road – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; temperatures of pasta, cheese and red sauce in reach-in cooler are elevated (reach-in cooler has a large amount of ice buildup which is blocking cold air circulation); several spray cleaner bottles are unlabeled; there are torn door gaskets on hot line reach-in cooler; wooden handles are worn off on two spatulas; one fryer basket is in need of replacing; reach-in cooler/freezer door gaskets throughout are dirty; observed dried food debris and mold on walk-in cooler wire racks; observed grease buildup on wire racks next to hot line equipment and on sides of hot line equipment, particularly between stoves and deep fryer; there is a leak in pipes under three-compartment sink; cracked floor tile found near hot line in kitchen; there are dried food stains on ceiling tile.
Carini Italian Restaurant, 3718 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; an employee is chewing gum while preparing foods; an employee handled clean dishes after handling soiled dishes without changing gloves; observed black mold buildup on inside of ice machine in rear storage area; there is 0 ppm sanitizer in dishwasher; temperatures of pico and lettuce in reach-in beverage-style fridge are elevated; no date marks found on prepped pasta sauce or meat sauce in walk-in fridge; salmon is thawing while in reduced oxygen packaging; Italian dressing is being sold in mason jars without the proper labeling; items are stored directly on floor in both walk-in freezers; multiple employees are wearing jewelry on wrists while involved in food prep; ice cream scoops and prep knife are stored in water at the prep station; to-go trays and single-service items are not stored inverted on the cook line; there are torn gaskets on reach-in freezer; fryer baskets are broken; there are multiple stained and scratched cutting surfaces along the kitchen cook line and at the salad prep station; facility couldn’t provide chlorine test strips; there is a slow leak on pipe underneath the dishwasher; observed grease and dust buildup on hood system above fryers and ovens.

Devil’s Lettuce, 9050 West Broad Street – The following violations were reported during a routine inspection: sanibuckets holding wiping cloths have 0 ppm sanitizer; temperature of patty in lowboy near fryer is elevated (unit is not maintaining foods at the proper temperature so the facility is has those items on a time control policy).
Chopt Creative Salad Company, 11301 West Broad Street – The following violations were reported during a routine inspection: paper towel dispenser next to two-compartment sink isn’t working properly; temperature plate shows that the dishwasher isn’t reaching the required temperature; shrimp in walk-in refrigerator is one day past its expiration date; lettuce in the display case is on a time control policy but no time stamp was placed on the containers; long cutting boards on the prep line are scratched and heavily stained; drying rack across from three-compartment sink and walk-in refrigerator racks are rusty.
Boil Bay Cajun Seafood & Bar, 8161 Brook Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; dried food debris found inside microwave; mold found inside ice machine; facility is cooking raw animal foods using a non-continuous cooking process but does not have the proper written procedures in place; bottle of peroxide stored on shelf amongst food; spray cleaner bottles are stored near food and single-service items; bulk containers of seasoning, flour, rice and cornmeal are unlabeled; cutting boards in kitchen are heavily scratched and stained; rust found on can opener base and blade; two lights are out in the hood system; reach-in cooler door gaskets throughout kitchen and in bar area are dirty; fan guards in walk-in cooler and on beer keg cooler are dusty and moldy; beer keg cooler keg tubes, ceiling and walls are moldy; there are dried food stains on wall behind hot line equipment; hood filters are greasy.

Hangry Joe’s Hot Chicken, 1501 North Parham Road – The following violations were reported during a routine inspection: person in charge couldn’t provide verification of employee health policy; staff left kitchen to go to the dumpster area and didn’t wash hands or change gloves when re-entering the kitchen; employee beverage found in hand sink; raw porkchops stored next to and above ready-to-eat foods; raw shell eggs stored over cole slaw mix in walk-in cooler; dried food debris found inside microwave and on walk-in cooler wire racks; can of WD-40 is stored among food items; a staff member’s cell phone is stored directly on cutting board in kitchen; jug of olive oil is stored directly on floor; staff is using moist towels and not storing them in a sanitizer solution; ice cream scoop and rice scoop are stored in ambient temperature standing water; to-go containers are stored uninverted; deep fryer basket is in disrepair; cutting boards throughout kitchen are scratched and stained; facility couldn’t provide sanitizer test strips; reach-in cooler door gaskets throughout kitchen are dirty; observed grease residue on sides of deep fryers, especially in spaces between fryers; top shelf of prep table is dusty and dirty; outdoor dumpster lids were left open; ceiling tile near ventilation and wall near electrical panels are dusty; there is grease and food debris on floor underneath deep fryers; there are food stains on wall behind microwave.
Mr. Noodle, 9127-C West Broad Street – The following violations were reported during a routine inspection: strainer and food debris found in hand sink next to prep sink; dried food debris found on slicer; temperatures of cooked bok choy and sprouts inside sandwich unit are elevated; no date marks found on two containers of meat cooked two days ago in walk-in cooler; several pans of white powders on cart next to woks (salt, sugar) are unlabeled; observed four sticky fly traps hanging from ceiling above food prep table; rice scoop is stored in a plastic bin (no water) next to rice cooker; cutting board at sandwich unit is heavily scratched; wet cardboard found under ice machine (person in charge states the ice machine has a leak); observed grease and flour buildup on hood; observed grime buildup on floor at fryer.
