Bell Greek, 7011 Three Chopt Road – The following violations were reported during a routine inspection: staff beverage without cover found in prep area; lemon seeds and a utensil found in hand sink near front counter; there is dried food debris inside microwaves; mold found inside ice machine; boxes of pita bread are stored directly on kitchen floor; observed food debris on cooler door gaskets throughout kitchen; walk-in cooler fan guards are dirty/dusty; observed food debris and mold on walk-in cooler wire racks; ceiling tile near ventilation in kitchen is dusty; personal items (jacket, bags, medication, lotion) are stored near/amongst food in kitchen.

Vallarta Mexican Grill Mariscos Bar, 5431 Glenside Drive – The following violations were reported during a routine inspection: tortillas are stored directly inside non-food grade grocery store bags; raw meat is stored over cooked meats and salsas in walk-in cooler; dried food debris found on can opener blade; container of thawed tilapia is still in vacuum sealed packaging; single cutting boards are heavily scratched and stained; floor in dry storage room is no longer smooth and easily cleanable.

Minilandia Ice Cream & Candy, 4062 Crockett Street – The following violations were reported during a routine inspection: food found in hand sink; several spoons are stored with handles in contact with food on service line (ice cream toppings, tajin and cut tomatoes).

Sandston Elementary School, 7 Naglee Avenue – The following violations were reported during a routine inspection: in-use cutting boards are scratched and stained; there is a slow leak at the handsink next to ice machine; observed heavy buildup of dust on AC filter grates located above kitchen entryway.

Highland Springs Elementary School, 600 Pleasant Street – The following violations were reported during a routine inspection: oven tops are dusty; there is mold and dust on dairy walk-in cooler fan guards; there is a leak in pipe under the prep sink.

Seven Pines Elementary School, 301 Beulah Road – The following violations were reported during a routine inspection: salsa and sour cream were placed on line but not included in the posted time control policy; reach-in fridge on front line is broken; observed buildup of dust on fan grates in walk-in refrigerator.

Ratcliffe Elementary School, 2901 Thalen Street – The following violations were reported during a routine inspection: bottle of cleaner in dish room is unlabeled; one light is out in the hood system; multiple lights are out in dry storage, serving line and at prep sink; floor drains throughout kitchen are dirty; ceiling tiles near vents are dusty.

Elko Middle School, 5901 Elko Road – The following violations were reported during a routine inspection: multiple red cutting boards have heavy scaring and discoloration; racks on lower shelf levels in walk-in refrigerator are rusty.

Casa Italiana, 8801 Three Chopt Road – The following violations were reported during a routine inspection: person in charge’s ServSafe certificate is expired; a food handler on the cooking line is chewing gum; the proper shellstock tag is not in the container of mussels in the prep unit; temperatures of large pot of tomato sauce, pan of lasagna and container of beans all prepared yesterday in walk-in cooler are elevated because of improper cooling methods; temperature of fresh garlic in oil is elevated because it is sitting out on prep counter; temperature of top layer of cooked chicken breasts is elevated because they were stacked too high; portioned lasagna in walk-in cooler isn’t date marked; previously frozen Ragu and meat sauces are not being date marked after being removed from the freezer to thaw; menu doesn’t contain a consumer advisory asterisk next to new item Ahi Tuna; tuna is thawing in reduced oxygen packaging in walk-in cooler; squeeze bottles of oils, sun-dried tomato combined with oils, and sauces are unlabeled; a tray holding bagged bread crumbs is stored directly on walk-in cooler floor; a linen napkin is being used to cover cooked spaghetti noodles in prep unit; small plates are not covered or inverted on carts in dining area; a single-use container is being used as a scoop in sauce in walk-in cooler; prep unit lid and bar fridge door are in disrepair (both covered in duct tape); small white bar fridge door gasket is torn; metal wires are bent and broken on fryer baskets; dish machine plastic racks are scratched and stained; prep unit and single cutting boards are heavily scratched and stained; duct tape is being used to hold receipts on prep unit shelf; observed dust accumulation on heat lamp power cord and all wire shelving units; observed grease accumulation on suppression nozzles; ceiling tile above walk-in cooler is out of place; floor tile in bar is chipped.

MOD Pizza, 8671 Quioccasin Road – The following violations were reported during a routine inspection: high temperature dishwasher isn’t reaching the required temperature; temperature of sliced tomatoes in prep unit table is elevated; food employee has long hair that isn’t properly restrained; employee with beard isn’t wearing a beard guard; cutting boards are scratched and stained; dry storage shelving units have rusty shelves; an irreversible registering temperature indicator isn’t provided for the hot water dish machine in operation; air vent above walk-in cooler is dusty; there are areas on floors that are no longer smooth and easily cleanable.

Pinchbeck Elementary School, 1225 Gaskins Road – The following violation was reported during a routine inspection: sanitizer bucket on floor near stand mixer has 0 ppm quat sanitizer.

Ridge Elementary School, 8910 Three Chopt Road – The following violations were reported during a routine inspection: racks are dusty; there is a buildup of dust on walk-in fan grates.

Freeman High School, 8701 Three Chopt Road – The following violations were reported during a routine inspection: two full sanibuckets on front service line are reading 0 ppm sanitizer; single-service items and to-go trays are stored uninverted; observed cleaning errors and buildup at wall juncture underneath three-compartment sink, floor drains by ice machine, and underneath dishwasher; there are three holes and cracks in the coving beneath the three-compartment sink.

Tuckahoe Middle School, 9000 Three Chopt Road – The following violation was reported during a routine inspection: observed buildup of dirt and debris beneath the dishwasher and at the wall coving in both kitchen 1 and 2.

Johnson Elementary School, 5600 Bethlehem Road – The following violations were reported during a routine inspection: reach-in cooler door gaskets throughout the kitchen are stained black; observed dried food debris and food stains on blue wall under serving line; paint is chipping from ceiling throughout kitchen; floor under three-door reach-in cooler is no longer smooth and easily cleanable.

Fair Oaks Elementary School, 201 Jennings Road – The following violations were reported during a routine inspection: temperature of chicken meat for tacos in warmer unit is too low; restroom doesn’t have a trash can with lid; mesh ceiling vents are dusty; walk-in cooler ceiling is dusty and dirty.

Wing Zone, 8310 Staples Mill Road – The following violations were reported during a follow-up inspection: observed an infestation of small flies throughout the kitchen; observed a buildup of food debris and grease on wire racks, breading container, walk-in cooler fan system, fryers, and inside prep unit and reach-in cooler; observed heavy buildup of grease and debris on fryer baskets; observed buildup of food debris and grime on grease trap and floor and walls throughout kitchen; several light bulbs in the kitchen do not have shields.

Wendy’s, 11274 Patterson Avenue – The following violations were reported during a routine inspection: observed an employee handle raw beef and then rinse her hands in the sanitizer bucket; gasket on walk-in freezer door is torn.

Ward Elementary School, 3400 Darbytown Road – The following violations were reported during a routine inspection: prepped feta cheese in storage container in walk-in is past its expiration date; buildup of grime and mold found on inside of ice machine in kitchen; both the recycling and garbage receptacles are uncovered; observed buildup of dust and debris along the wall above reach-in fridge and on air conditioning vents/filters and adjacent ceiling tiles.

Glen Allen High School, 10700 Staples Mill Road – The following violations were reported during a routine inspection: there is dust buildup on top of ovens; milk/food residue found on wire racks in walk-in cooler.
