Euphoria Bar & Lounge, LLC, 4360 South Laburnum Avenue – The following violations were reported during a routine inspection: hot water valve on hand sink next to fryer is broken; there is low flow on cold water valve on hand sink next to three-compartment sink; the hand wash station next to fryer is being used to dispose of food products (french fries and sauces found in sink basin); utensils are not stored inverted to prevent contamination; observed buildup of full trash cans behind building that need to be taken to refuse recepticle; covering of air vent is falling off the ceiling; there is buildup of old food debris on walk-in floor; observed dust and grease buildup on walls and vents next to hood; light bulbs are out underneath hood system; tube bulb lights are out in kitchen prep area.

Cities Bar & Grill, 8004 West Broad Street – The following violations were reported during a follow-up inspection: hand sink outside bathroom doesn’t have paper towels; there is 0 ppm sanitizer in dish machine; middle soda nozzle in bar is dirty; menu doesn’t contain a consumer advisory next to temperature-sensitive items; spray bottle with light red liquid in the bar area is unlabeled; second prep unit has a broken gasket; vents in walk-in refrigerator are dusty.

Honest Restaurant, 8900-A West Broad Street – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; no handwash signage is posted at front counter hand sink or middle prep area hand sink; there is no hot water in men’s or women’s restrooms; there is 0 ppm sanitizer in dish machine; bulk containers of salt and sugar are unlabeled; tubs of ready-to-eat food are stored directly on kitchen floor; ice cream scoop is stored in a container of ambient temperature standing water; dishes are stacked and/or stored while still wet; to-go containers are stored uninverted throughout kitchen; cutting boards throughout kitchen are scratched and stained; reach-in coolers in back kitchen have torn rubber door gaskets and there is dried food debris/buildup on them; observed dried food debris and grease buildup on sides of all hot line cooking equipment; walk-in cooler fan guards are dusty; there are dried food stains on walls throughout the kitchen; observed mildew on dish area walls (behind dish machine and three-compartment sink).

Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a routine inspection: facility doesn’t have a hand sink in the front food prep/dispensing areas which is preventing routine handwashing by food workers; unknown solid substances are floating in the sweet tea container; sanitizer concentration in three-compartment sink basin is at 0 ppm; observed grease residue on plastic containers stored clean; temperatures of ham/cheese boats and cupped coleslaw prepared this morning in reach-in coolers are elevated because of improper cooling methods (covered and stacked during cooling process); temperature of philly steak in make unit is elevated; observed several small flies in dry storage area and near hand sink; metal wires on fryer baskets are bent; several plastic containers are scored and not smooth; air curtains on walk-in freezer and cooler doors are torn; observed encrusted grease and food residue inside broiler and warming cabinets above prep unit and on touchscreen pads and wheels of fryers; observed heavy buildup of dried food residue, grease, dust and grime on several surfaces (walk-in cooler wire storage racks and fan system, cup and lid holders, tops of ovens and prep units, microwaves, fryers, monitors, cables, rolling carts, dunnage racks, floor mat inside cooler, cooler doors, under broiler, bottle water storage bin, and handles on all reach-in coolers and freezers); several mops are not hung to air dry; observed heavy buildup of grease, food debris, dust and mold on walls, floors and ceiling throughout kitchen; there is heavy buildup of food and grime inside mop sink.

Subway Sandwiches & Salads, 1216 North Concord Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit or diarrhea; temperatures of tomatoes, tuna and ham in lowboy across from prep line are elevated; this unit isn’t maintaining foods at the proper temperature; there is a gap to the exterior of the building underneath back shop door (no protective barrier present between exterior and interior); prep line cutting boards are heavily scratched and stained.

Topgolf Richmond, 2308 Westwood Avenue – The following violations were reported during a routine inspection: no handwash signage is posted in first floor men’s bathroom; observed mold inside ice machines on second and third floor; can opener blade is rusty; observed flies and fruit flies throughout establishment, particularly in bar areas and near floor drains; wire racks in second floor blast chiller are rusty; reach-in freezer gasket in second floor kitchen is torn; reach-in cooler gaskets in bar prep room are torn; facility couldn’t provide an irreversible registering temperature device; observed black stains, mold and dried food debris on reach-in cooler and freezer door gaskets throughout establishment (bars and kitchens); observed grease buildup on sides of hot line cooking equipment on first and second floors; wire racks in walk-in coolers on first and second floors are moldy; observed dried food debris on second floor tilt skillet; there is grease residue on stainless steel walls behind hot line equipment on first and second floors; food stains found on ceiling tiles in first floor kitchen and bar prep room; floor drains are dirty in bar prep room and first and third floor bar areas; hood filters in second floor kitchen are greasy (above deep fryers and grill).

Calypso’s LLC, 5717 South Laburnum Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit or diarrhea; hand sink next to freezer doesn’t have paper towels; wire racks in rear walk-in fridge are rusty; there is a broken whisk in kitchen prep area; cutting boards in prep area are heavily scratched and stained.

Little Caesars, 6040 Brook Road – The following violations were reported during a routine inspection: no certified food protection manager is present; facility couldn’t provide employee health policy; multiple open employee drinks are stored throughout the walk-in fridge; observed heavy buildup of grease and food debris on containers, sauce bottles and utensils within dry storage racks; pre-staged pizzas are on a timer rack without timer started; multiple sheet pans are dented and soiled with food residue; observed food debris and grease buildup on wire racks in dry storage, rolling rack in walk-in and prep rack in pizza prep area; there is dust buildup on AC vent cover, monitor cables and fan unit next to oven; ceiling tiles and wall next to oven are soiled and stained; mops are not hung to air dry.

Tropical Smoothie, 9972 Brook Road – The following violations were reported during a routine inspection: sandwich prep table cutting boards and cutting boards in dry storage are scratched and stained; observed food debris inside walk-in freezer; smoothie line prep table’s gaskets are sticky to the touch; observed buildup of grime and food debris at coving of walls behind prep units and underneath sinks in the kitchen.

Target, 4521 South Laburnum Avenue – The following violations were reported during a routine inspection: observed food debris and grease buildup on green rolling rack in kitchen prep area; observed heavy dust buildup on hood system above pizza oven.

Starbucks Coffee Company, 4511 South Laburnum Avenue – The following violations were reported during a routine inspection: person in charge couldn’t show instructions or kit for cleaning up vomit or diarrhea; observed buildup of dust on fan grates within walk-in fridge and freezer; bottom plumbing fixture of ice machine is pooling water in the storage area; observed heavy buildup of food debris, grime and waste along coving of the walls beneath three-compartment sink and behind lowboy reach-in units in prep area.

Starbucks Coffee Company, 11332 Nuckols Road – The following violations were reported during a routine inspection: staff reports that their food protection manager certification is expired; bag of straws stored in hand sink by three-compartment sink; this hand sink doesn’t have paper towels; there are syrup/coffee stains on shelves throughout front counter area; coffee residue found in cabinets; there is stainage in dump sinks; reach-in freezer door gaskets are moldy; kitchen floors are pitted and no longer smooth and easily cleanable.

McDonald’s, 11218 West Broad Street – The following violations were reported during a routine inspection: observed an employee washing his hands in the three-compartment sink instead of the hand sink; hand sink in back of kitchen doesn’t have paper towels; several items in the kitchen are in disrepair (fryer basket, reach-in freezer gasket, walk-in freezer door and walk-in refrigerator rack); mop isn’t hung to air dry.

Tuckahoe Moose Lodge, 4366 Springfield Road – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; no handwash signage is posted in women’s or unisex restrooms; shell eggs and liquid eggs stored over ready-to-eat foods in reach-in coolers; dried food debris found inside microwave and on can opener blade; temperatures of sloppy joe meat, hot dog and fried chicken in reach-in cooler near hot line are elevated; several hunks of previously sliced deli meat do not have date marks; observed fruit flies in bar area, particularly near soda gun; wet wiping cloth isn’t stored in a chemical sanitizer in between uses; hood system lights are not working; wire racks in reach-in coolers are rusty; light fixture above prep table doesn’t have a protective shield; cutting boards throughout kitchen are heavily scratched and stained; observed buildup of food debris, dust and grime on surfaces of fryers, grill, oven, inside prep and reach-in coolers, reach-in cooler door gaskets, on portable fans, prep and counters, inside mop sink and wire storage racks; walk-in cooler fan guard is moldy; women’s restroom and unisex restroom don’t have covered trash cans; there are food stains on walls throughout the kitchen; there is grease buildup on hot line floor (underneath deep fryers and grill).
