Les Crepes, 1308 Gaskins Road – The following violations were reported during a follow-up inspection: there is 0 ppm chlorine in bar and verdant lady dish machines and dish machine in dish room; black buildup found on plastic panel in ice machine; temperature of milk in bar fridge is elevated; green cutting board is heavily scratched; floor tiles are cracked in staff bathroom; there is a hole in the wall in staff bathroom; floors in kitchen need re-sealing; walk-in cooler vent is dusty.

JULY 30

Dominion Club, 6000 Dominion Club Drive – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t locate the employee health policy; facility doesn't have written procedures for cleaning up vomit or diarrhea; observed an employee drinking from an open can behind the bar; pink slime and black buildup found inside ice machine; there is buildup inside soda gun nozzles at the bar; crab cake, artichokes, vodka sauce and salsa are past their expiration dates; smoked salmon isn’t date marked; pesto dated from April was not relabeled after being removed from freezer two days ago; bulk containers of flour and sugar are unlabeled; observed small flies at bar and dish pit; ice cubes in reach-in are not covered and it appears debris/crumbs have fallen onto the ice; employee with long hair is handling food without wearing hair restraint; single-service containers are being used as scoops for flour and sugar; flat spatulas are damaged and chipped; cutting boards are heavily scratched; facility couldn’t provide chlorine test strips; observed grime on wall in dish pit; caulking at sink is soiled.

Rise Biscuits & Donuts, 11561 West Broad Street – The following violations were reported during a routine inspection: fryer baskets are in disrepair; several areas/items are dirty (bottom shelf of make table, top shelf of prep table, walk-in refrigerator floor and racks, drying racks at three-compartment sink, ceiling vents, and dry ingredients shelf); dumpster area is surrounded by loose trash and debris; back hoods are dusty.

Peking Restaurant, 8904-F West Broad Street – The following violations were reported during a routine inspection: employee used prep sink instead of hand sink to wash hands after handling raw chicken; employee failed to change gloves and wash hands after handling dirty dishes and before handling clean dishes from the dishwasher; hand soap dispenser in employee bathroom is broken and empty; observed buildup on can opener blade; black mold found inside ice machine; temperature of fried chicken cooked this morning in walk-in cooler is elevated; temperatures of raw chicken and cooked shrimp in reach-in prep unit are elevated; no date marks found on egg rolls, pasta or chicken in walk-in cooler; fried shrimp and fried chicken are being held without the proper time control policy; observed improper thawing methods for shrimp and scallops; raw chicken is stored directly on freezer floor; cutting boards in prep area are scratched; racks in walk-in fridge and freezer are rusty; observed food debris and buildup on cook line equipment; there is dust buildup on make table and walk-in fan guards; observed cleaning errors on the walls; concrete floor is not sealed and easily cleanable; observed buildup of food debris and grease under cook line and near grease traps.

Little Caesars, 4007 Mechanicsville Turnpike – The following violations were reported during a follow-up inspection: facility doesn’t have a person in charge with food protection manager certification; food debris found on can opener blade; clean dishes in dish area are stored on dirty, dusty and greasy storage racks; observed dried food debris, grime, grease, dust and mold on several items (speed racks throughout kitchen, walk-in cooler storage racks, wire racks in dish area, and rubber door gaskets of Pepsi beverage cooler); receptacles/waste handling units for refuse, recyclables and/or returnables kept outside the establishment are not kept covered with tight-fitting lids or doors; observed grime buildup around three-compartment sink drain.

Domino’s Pizza, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: temperature of shredded cheese in lowboy unit is elevated; air temperature of this unit is elevated; walk-in cooler fans are dusty.

Canaima Venezuelan Food, 2755 Hungary Spring Road – The following violations were reported during a routine inspection: hand sink doesn’t have hand soap; pasta paste and picante sauce are not date marked; single cutting boards are heavily scratched and stained; grease trap is leaking; wall coving is peeling from the wall.

Little Caesars, 5101 South Laburnum Avenue – The following violations were reported during a routine inspection: multiple open employee drink containers found in prep areas and in walk-in; Crazy Sauce in individual cups is being held without time being tracked; employees are handling food without beard and hair restraints; single-service to-go containers are stored on a dirty rack; pizza assembling machine is out of order; observed heavy buildup of food debris and grime on surfaces of all wire storage racks, dunnage carts, portable fans, walk-in cooler fan grates, cooler air curtain, pallets in coolers, pizza oven and inside prep unit; walls and ceiling tiles in kitchen area are stained; there is a buildup of food debris in drain under three-compartment sink; floors in walk-in fridge and freezer are uncleanable and uneven; mop isn’t hung to air dry.

Red Lobster, 4415 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sinks at bar, expo beverage area and dishwashing area don’t have paper towels; tomato dicer has food debris and buildup; temperatures of tomatoes and caesar salad in salad prep unit are elevated; sanitizer level is too high at bar three-compartment sink; raw fish in walk-in fridge was thawed while still in reduced oxygen packaging; glasses in bar area are stacked while still wet; slicers and cutting blades are damaged; multiple cutting boards in kitchen prep area and bar area are scratched and stained; observed black mold on walk-in fan blades; cables, monitors and above the oven are dusty; observed grease buildup on hood; there are cracked floor tiles and areas of low grout in walk-in fridge.

Qpot Hot Pot & BBQ, 11740 West Broad Street – The following violations were reported during a follow-up inspection: cold-holding TCS food items in display case, self-service case and buffet line (noodles, quail eggs, melon, ribs, pork belly and cabbage) are consistently not holding at the proper temperature; there is 0 ppm sanitizer in the dishwasher; sanitizer level in three-compartment sink is too low; three metal containers holding powder ingredients on the line are unlabeled; there is no sneeze guard covering food items in the self-serving display cooler; can of Redbull found in ice machine; mops are not being hung to air dry.

Nuevo Mexico Restaurante, 10282 Staples Mill Road – The following violations were reported during a routine inspection: temperatures of chicken, carnitas and salsas in walk-in cooler are elevated; pickled veggies and salsa in reach-in cooler are past their expiration dates; menu doesn’t contain asterisk linking ribeye steak that can be ordered undercooked or raw to the consumer advisory statement; in-use knife is stored in dirty crevice between prep unit; observed copper plumbing nozzle on frozen drink machine (not food grade or corrosion resistant); metal is bending and breaking on fryer baskets; spatulas are in disrepair; walk-in cooler isn’t maintaining foods at the proper temperature; observed buildup of dust and grease residue on surfaces of all wire storage racks and hood suppression nozzles; a bulb is out in the hood system.

Cava Grill Short Pump, 11780 West Broad Street – The following violations were reported during a routine inspection: dishwasher testing at 0 ppm sanitizer; temperature of falafel on front line is too low; chopped, leafy greens held on time control in the display cooler are past their expiration time; reach-in unit holding raw meats has a torn gasket; racks above three-compartment sink are rusty; inside of dish machine and inside of reach-in units are dirty; mop sink faucet doesn’t have a dual check valve installed; wall next to three-compartment sink is peeling; floor, wall and drains around three-compartment sink are dirty.

Umi Sushi Restaurant, 11645 West Broad Street – The following violations were reported during a routine inspection: tofu and cream cheese are not marked with the date they were opened; menu doesn’t contain asterisk linking specialty rolls to the consumer advisory; bleach and other cleaning supplies are stored near food; facility couldn’t provide a thermometer; white powders on cook line and on bottom storage shelf are unlabeled; take-out container shelf is dirty; napkins and other absorbant materials are lining shelves in kitchen and in bottom of the beer chest cooler behind bar; freezer chest lid has a torn gasket.

McDonald's, 607 East Laburnum Avenue – The following violations were reported during a routine inspection: observed small amount of mold inside ice machine; observed dark buildup of debris around ice chute of drive-thru drink machine; multiple flies found in kitchen because staff had back door open while receiving a truck order; there is a small leak in pipes under three-compartment sink.

KFC, 3800 Mechanicsville Turnpike – The following violations were reported during a routine inspection: facility doesn’t have sanitizer; mold found on base near dispenser spouts on front of house soda machine; observed one dead roach and evidence of pests in dry storage area; racks throughout kitchen and in middle walk-in cooler are rusty; middle of walk-in cooler isn’t holding foods at the proper temperature; observed food debris and grease buildup on sides of holding cabinets and shelves under prep tables; sides of deep fryers are greasy; several items/areas are dirty (prep sink floor drains and nearby wall, wall and floor behind oven, ceiling vents, interior and exterior of tall holding cabinet, walk-in cooler and freezer doors, floor and ceiling of left walk-in, and speed racks in walk-in cooler); pipes and cords behind hot line, hood vents and fire suppression nozzles are covered with dust and flour; there is a leak underneath hot line hand sink; there is missing floor tile in front of three-compartment sink; floor coving tile inside walk-in cooler is in disrepair; observed dried food debris on wall behind breading tables; wall behind three-compartment sink is moldy.
