Domino’s Pizza, 70 South Airport Drive – The following violations were reported during a routine inspection: temperatures of cheese, ham, pasta and tomatoes are elevated; shredded pizza cheese has incorrect expiration date written on it; observed fruit flies throughout the kitchen; roaches found in men’s restroom; piping along wall behind three-compartment sink is dirty and moldy.

The Place at Innsbrook, 4036-C Cox Road – The following violations were reported during a routine inspection: certain food items in the facility are cooked non-continuously with no written procedures in place; spray bottle containing red liquid chemical is unlabeled; several items/areas are dirty (interior of storage cabinet under cooking line, interior of hot holding box, interior of mini oven, shelves holding dry goods, interior of standing refrigerator, rack in reach-in refrigerator, vents in walk-in refrigerator, walk-in fridge and freezer floors, wall behind vent in walk-in refrigerator, bottom of prep shelf holding clean cooking equipment, top shelf of cook line, drain under dishwasher, kitchen fan, cook line floors, and ceiling vents throughout kitchen); leak observed in bar in front of kitchen; two hood lights do not have a protective shield cover.

Pinch’s Soul Food & Catering, 3708 Meadowbridge Road – The following violations were reported during a routine inspection: raw shell eggs stored over ready-to-eat foods in reach-in cooler; to-go boxes, single-use plates and aluminum pans are not stored inverted; observed stained and damaged ceiling tiles in kitchen and front of house.

Firehouse Subs, 9069 Staples Mill Road – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit/diarrhea; food debris found on deli slicer; temperatures of swiss cheese, American cheese and peppers in au jus in make table are elevated; freshly-made sauteed onions are cooling in make table instead of reach-in fridge or freezer; single-service items are not stored inverted.

JULY 19

Poke Sushi Bowl, 11628 West Broad Street – The following violations were reported during a routine inspection: pots and utensils are stored in hand sink; eggs stored over ready-to-eat foods in reach-in refrigerator; medicine bottle found near food items on storage rack; shrimp is thawing in standing 80-degree temperature water; food items in reach-in refrigerator are being stored in grocery bags instead of food-grade bags or containers; take-out containers are not stored inverted; shelf above three-compartment sink is rusty; cardboard is lining food storage shelves; sanitizer test strips are expired; food storage racks are dusty; observed trash and loose debris surrounding dumpster area; mop isn’t hung to air dry.

Krispy Kreme Doughnuts, 4906 West Broad Street – The following violations were reported during a routine inspection: personal beverage that is uncovered found on doughnut conveyer belt; hand sink in doughnut prep area doesn’t have paper towels; dish area hand sink doesn’t have hand soap; front counter hand sink doesn't have hand soap because the dispenser isn’t working; drive-thru reach-in cooler fan guard is dusty; reach-in cooler doors have dried food residue and stains on them; doors and lids were left open on outdoor dumpsters; ceiling and walls by ventilation above doughnut conveyer belt and ceiling vent in dish area are dusty; ceiling tile in back of house is stained and dusty; floor under three-compartment sink has heavy grease and food debris buildup.

JULY 22

The Melting Pot, 9704 Gayton Road – The following violations were reported during a routine inspection: black buildup found on plastic panel in ice machine; temperatures of shredded cheese and gorgonzola crumbles in lowboy unit are elevated; temperature of potatoes in prep unit are elevated; plastic bins next to dish machine are stacked while still wet; ice machine door is cracked; wire racks in reach-in freezer, lowboy unit across from reach-in freezer and walk-in cooler 2 are rusted; cutting boards on storage rack are heavily scratched; observed white buildup on shelf in walk-in cooler 1; there is dust buildup on plastic sheet holding fondue pots; floor tiles outside walk-in units are cracked.

The Beach House, 4040-B Cox Road – The following violations were reported during a follow-up inspection: food residue found on slicer blade; temperature of tomatoes in prep unit 2 is elevated; there is a direct connection at the prep sink; several ceiling tiles are either removed or pushed aside.

Regal Cinemas—Short Pump, 11650 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate; facility couldn’t provide written procedures for cleaning up vomit/diarrhea; paper towel dispenser is empty at hand sink next to three-compartment sink; spray bottles containing blue chemical are unlabeled; gaskets are torn on standing white freezer in back of facility and reach-in refrigerator behind concession counter; sanitizer test strips are expired; dumpster area is surrounded by loose trash and debris.

Starbucks inside Kroger, 11895 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit/diarrhea; gasket is torn on reach-in refrigerator under oven.

America’s Best Wings, 7536 West Broad Street – The following violations were reported during a routine inspection: observed an employee rinse off a knife after sanitizing it; temperature of noodles prepared yesterday in the walk-in is elevated; temperature of previously cooked wings in the reach-in are elevated; pasta is stored in a tightly-covered deep container in the walk-in during the cooling process; raw chicken is stored directly on the walk-in fridge floor; cardboard is being used to cover two broken fryers; two cutting boards are scratched and discolored; two fryers on the cook line are not working properly; observed grease buildup beneath the hood of the fryers; observed a hose with spray nozzle under pressure at mop sink; observed grease buildup and waste around grease receptacles.

Yen Ching, 9031-9035 West Broad Street – The following violations were reported during a follow-up inspection: person in charge couldn’t provide food protection manager certificate; temperatures of noodles and sprouts in prep unit are elevated; long white cutting board at prep unit is heavily scratched; observed leak at bar hand sink; there are cracked floor tiles near fryers and back door.

Teppanyaki Grill & Buffet, 9043 West Broad Street – No violations reported during a follow-up inspection.

Chili’s Grill & Bar, 9950 Brook Road – The following violations were reported during a routine inspection: employees’ personal drinks are stored on and above prep unit; observed buildup of food debris, dust, mold and rust on several surfaces (wire racks in walk-in and dry storage, liquor racks, beer lines, microwaves, fan blades, point-of-sale touch pads on cook line, and hood filters); observed buildup of grease and waste under flat top grill, hood and between make tables; proper backflow prevention device isn’t installed on hose with nozzle attached under pressure; buildup of food debris, dust, mold and trash found on floors, walls and ceilings; there is trash and food debris on floor drains in kitchen and bar area.

Kabab House, 3411 Old Parham Road – No violations were reported during a follow-up inspection.
