JUNE 4

Hanabi, 7801 West Broad Street – The following violations were reported during a routine inspection: open employee beverages found in food prep area; facility doesn’t have a food thermometer; bag of onions, carrots and sprouts are stored directly on the floor; single-service containers are not stored protected/inverted; facility couldn’t provide sanitizer test strips.

JUNE 5

Atlas 42, 4032-A Cox Road – The following violations were reported during a routine inspection: hand sink behind bar doesn’t have paper towels; black debris found inside ice machine in kitchen; over a dozen food containers are past their expiration dates; bulk bins are unlabeled; foil found lining lower shelves of equipment; standing reach-in freezer has a broken gasket; small cutting boards have deep scratches; dishwasher isn’t producing the required amount of sanitizer; sanitizer test strips are expired; several items/areas are dirty (walk-in fan vents and wire racks, floors in walk-in, lower shelves holding plates, wire racks holding utensils and bottom of beer coolers); there are broken tiles behind three-compartment sink; walls and floors around dishwasher and cooking equipment are dirty; mops are not hung to air dry.

Marco’s Pizza, 2284 John Rolfe Parkway – The following violations were reported during a routine inspection: cutting board on prep unit is heavily scratched and stained; several lids in-use are cracked; dry storage racks are rusty; observed food debris buildup on prep shelves, inside reach-in cooler and on gaskets, and on racks in walk-in cooler; there is grease buildup on green racks above three-compartment sinks.

Papa John’s Pizza, 6100-D West Broad Street – The following violations were reported during a routine inspection: spray cleaner bottles are unlabeled; cases of bottled water are stored directly on walk-in cooler floor; cooler under pizza make table has torn door gaskets; walk-in wire racks are rusty and have dried food debris on them; walk-in cooler fan guards are dirty and dusty; walk-in cooler door gaskets are dirty; both reach-in cooler door gaskets are dirty; outdoor dumpster door was left open.

Chao Tian Fu Spicy Hot Pot, 9127-C West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures; both kitchen hand sinks are missing paper towels; hand sink next to three-compartment sink doesn’t have hand soap; raw chicken stored above raw vegetables in walk-in cooler; observed an employee washing dishes in the prep sink without sanitizer; temperatures of raw chicken and raw beef at self-serve station are elevated; spray bottle filled with dish soap and water isn’t labeled; allergy medicine stored next to single-service containers and above make table; can of WD40 found on shelf above make table; several sauce bottles on black cart are unlabeled; menu lists “lobster ball” but imitation lobster is being sold; fly traps are placed above prep table and make table; several containers of food are stored directly on walk-in cooler floor; there are foods and sauces in open containers at self-serve station with no sneeze guard or lids; plastic bins are stacked while still wet next to dish machine; blender bottle has cap screwed on while still wet; plastic, single-service containers without handles are being used as scoops in flour and rice bins; single-service containers are stored uninverted above make table; walk-in cooler and lowboy freezer wire racks are rusty; the paper towel dispensers in both bathrooms are not dispensing paper towels; long white cutting board at make table is heavily scratched; observed buildup on interior and exterior of dish machine; several items/areas are dirty (food cart next to wok station, exterior of make table, and exterior of lowboy freezers); observed heavy food buildup on floor under wok station; two blenders that are not being used should be removed from the facility.

JUNE 6

Denny’s, 6598 West Broad Street – The following violations were reported during a routine inspection: no handwash signage is posted in women’s restroom; small amount of mold found inside ice machine; dried food debris found inside beverage station microwave; cutting boards are heavily scratched and stained; sides of hot line equipment have heavy grease buildup; several items/areas are dirty (fridge gaskets throughout kitchen, door frames throughout kitchen, dish drying racks, walls behind hot line, floor drains throughout kitchen, cabinet interiors under beverage station, and walk-in cooler and freezer doors and frames); walk-in cooler fan guards are dusty; there is missing/cracked floor tile in break room; there are food stains on walls in dish room.

JUNE 7

Starbucks inside Publix, 2250 John Rolfe Parkway – The following violation was reported during a routine inspection: sanitizer concentration is too low at three-compartment sink.

Arby’s, 11298 Patterson Avenue – The following violations were reported during a routine inspection: temperature of cooked chicken in warmer on prep line is too low; warmer unit isn’t maintaining foods at the proper temperature; observed small flies in back prep area; prep unit cutting boards are heavily scratched and stained; observed food debris buildup on dry storage racks, on top of oven and inside utensil bins at front service counter; observed grease and food debris on floor behind and between fryers; there is dust buildup on ceiling vents above slicer.

Domino’s Pizza, 11210 Patterson Avenue – The following violation was reported during a routine inspection: temperature of sausage in pizza make table is elevated.

JUNE 10

A Sharper Palate Catering, 5511 Lakeside Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide proof of employee health policy; person in charge couldn’t provide written procedures; several trays of foods (chicken, sliced deli meats and cheeses) in walk-in cooler are tightly wrapped during the cooling process; single cutting boards are heavily scratched and stained; observed grease and food debris buildup on storage racks next to stove and in walk-in cooler; walk-in freezer fan grates are dusty; both restrooms are missing covered trash cans.

Les Crepes, 1308 Gaskins Road – The following violations were reported during a routine inspection: person in charge couldn’t verify staff knowledge of the “big six” reportable foodborne illnesses and the associated symptoms; person in charge couldn’t provide written procedures; strainers found in two hand sinks; there is 0 ppm chlorine in three dish machines; observed buildup on plastic panel in ice machine; food debris found on dishes above prep unit; temperature of milk in bar fridge is elevated; propane fuel stored above prep unit and butane fuel stored with spices; a spray bottle filled with sanitizer is unlabeled; three adult and one baby live roaches found near microwaves; two fly traps are installed over clean dishes and above clean knife rack; ice bucket with water inside is not stored inverted; a fryer basket is heavily bent with missing wire in the corner; several cutting boards are heavily scratched; wire rack between fryer and reach-in freezer is dusty/grimy; large fire extinguisher is greasy; floors throughout the kitchen are dirty, especially under three-compartment sink where there is mold buildup and five dead roaches; hood vents are dusty and greasy; walk-in cooler vent is dusty; floor tiles are cracked in men’s staff bathroom; floors are in need of re-sealing; floor drain found covered with duct tape; mops are not being hung to air dry.

JUNE 11

Ming’s Dynasty, 1200 North Concord Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning vomit/diarrhea; raw eggs stored above cooked noodles in walk-in cooler; pink buildup found inside ice machine; temperatures of egg rolls in prep unit and garlic in oil on cart are elevated; several cutting boards are heavily scratched and stained black/brown; wood on dish rack next to three-compartment sink is unsealed; observed buildup on sides of fryer, warmers, prep units, inside fryer cabinet and on wire racks in dry storage and walk-in cooler; several metal and plastic food containers are not being used and should be removed from the facility; there are cracked floor tiles behind fryer and near three-compartment sink; ceiling panel is torn near back door; there are stains on ceiling panels and walls throughout the kitchen; floor tiles and pipes under three-compartment sink are dirty with buildup of grime.

Papa John’s Pizza, 8903 Patterson Avenue – The following violations were reported during a routine inspection: quat sanitizer concentration is too low in three-compartment sink; cutting board near three-compartment sink is heavily scratched with brown/black staining; there is low grouting throughout the kitchen.
