Brecotea, 9125-M West Broad Street – The following violations were reported during a routine inspection: pitcher found in hand sink next to three-compartment sink; ice bucket stored on the floor next to ice machine with water sitting in the bottom; plastic containers are stacked while still wet; floor under three-compartment sink and wall behind hand sink at front counter are dirty; light fixtures above front counter and ceiling vents are dusty; mop isn’t hung to air dry.

MAY 30

Quarter Horse, 9018 West Broad Street – The following violations were reported during a routine inspection: food debris found inside blender stored clean; containers of butane fuel are stored above cherry tomatoes and onions; a crate filled with various foods is stored directly on walk-in cooler floor; one white cutting board in the main kitchen is heavily scratched and stained yellow; there is a hole in the wall under bathroom hand sink; floor in walk-in cooler is not smooth and easily cleanable.

Arby’s, 9080 West Broad Street – The following violations were reported during a routine inspection: observed grime buildup on lemonade dispenser nozzle; temperature of cooked wagyu burgers is too low; menu doesn’t contain a consumer advisory for wagyu burgers; long white cutting board on the line is scratched and stained yellow; there is dust buildup on monitors; observed leaks at mop sink faucet, handle at hand sink next to walk-in cooler and at three-compartment sink; ceiling vents are rusty; wall tile in dry storage area is cracked; floor tiles in both walk-in coolers are cracked; several areas are dirty (floor under shelving in dry storage, floor under three-compartment sink and ceiling panels above oven).

Antonio’s Pizza, 63 South Airport Drive – The following violations were reported during a routine inspection: can opener blade has dried food debris; observed one alive and two dead pests in the kitchen; back door of kitchen is in disrepair, isn’t sealed and is potentially allowing pests into the kitchen; multiple food items stored directly on walk-in cooler floor; reach-in cooler door gaskets are torn and stained black; wire racks in walk-in cooler are rusty; hood filters are dusty and greasy; sides of cooking equipment have heavy grease buildup; there is mold inside the pizza make table; door gaskets are dirty; TV in kitchen is dirty and dusted with flour; walk-in cooler ceiling and walls are dusty and dirty; walk-in cooler fan guards are dusty; dining room ceiling tiles are stained/damaged/missing; there are food and grease stains on walls throughout the kitchen and food stains on ceiling tiles in kitchen; walls and ceiling near pizza oven are dusty and stained; various junk and unecessary items are stored in the kitchen.

Wellness Center Tepaz, 5000 Nine Mile Road – The following violations were reported during a routine inspection: observed several dead bugs in corner of main prep area; there is a small leak at three-compartment sink.

“The Original” Ronnie’s BBQ, 2097 New Market Road – The following violations were reported during a routine inspection: dried food debris found inside microwave; several spray cleaner bottles are unlabeled; observed several flies in kitchen and several dead bugs in front service area; cutting boards throughout kitchen are scratched and stained; racks in walk-in cooler are rusty; walk-in cooler door gaskets are loose; sides of hot line cooking equipment have heavy grease buildup; hood filters are greasy and dusty; AC vent and floor fan in kitchen are dusty; dry storage wire racks have dried food debris on them; single-door freezer gaskets and door are dirty; dried food stains found on double-door freezer bottom shelf; women’s restroom doesn’t have a covered trash can; there are food stains and grease residue on walls throughout the kitchen; ceiling tile above walk-in cooler is missing/damaged; one light is out in hood ventilation system.

California Pizza Kitchen, 11800 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning vomit/diarrhea; one hand sink on the line doesn’t have hand soap; hand sink in kitchen is blocked by a trash can; black debris found inside ice machine; can opener is dirty/rusty; soda dispenser above ice bin in bar is moldy; fly spray found at bar; temperature of chicken throughout cold holding units is elevated; sugar container is unlabeled; utenils found in water cup with fountain head turned off; shelves with takeout containers is dirty; cutting boards throughout kitchen are deeply scratched and stained; several areas/items are dirty (fryer baskets, ceiling tiles and vents, floors, wire racks in reach-in refrigerator, bottom prep table shelf in dry storage area, bottom of beer cooler chest, and vent in drink refrigerator at bar).

El Torito Mexican Restaurant & Grill, 5600 Williamsburg Road – The following violations were reported during a routine inspection: kitchen hand sink doesn’t have paper towels; no handwash signage is posted at kitchen or bar hand sinks; men’s restroom and kitchen hand sink don’t have hand soap; mold/pink slime found inside ice machine; soda gun nozzle at bar area is moldy; ice bins at beverage station, bar area interiors and soda dispenser nozzles are dirty; facility doesn’t have a working food thermometer; bulk containers of rice and seasoning are unlabeled; observed many flies in front of house area; food is stored directly on walk-in cooler and dry storage floors; cutting boards are heavily scratched; observed grease residue on sides of hot line cooking equipment; food debris found on walk-in cooler storage racks; beer mug freezer door gaskets are dirty; there is a leak in piping underneath dish machine in kitchen.

BB-Q Chicken, 7801 West Broad Street – The following violations were reported during a routine inspection: observed employees eating at the cook line; raw shell eggs stored over sauces in reach-in; food residue found on clean containers; temperatures of raw chicken and rice prepared yesterday are elevated because of improper cooling methods; temperature of cooked chicken in bowl in pass-thru window is too low; temperatures of heavy cream and chicken batters in make table are elevated; kimchi is covered while cooling in the reach-in; utensils used for chicken and other foods are stored in ambient temperature standing water.

Waffle House, 11900 Church Road – The following violations were reported during a routine inspection: food employees are chewing gum; open drink containers found on prep counters; an employee failed to change gloves after cracking shell eggs and before handling clean plates; dish machine isn’t reaching the required surface temperature after several runs; temperatures of sausage gravy and chili in warmer well are too low; incorrect time marked on shell eggs; several employees are wearing bracelets and rings while handling food and clean equipment; employee with beard isn’t wearing a beard guard.

Panda Express, 4501 South Laburnum Avenue – The following violation was reported during a routine inspection: wire racks in walk-in cooler are rusty.

Papa John’s Pizza, 4744 South Laburnum Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate; bag of wings are draining in a hand sink; hand sink in front doesn’t have paper towels; lowboy unit has a torn gasket; there is heavy dust buildup on pizza oven fans; there is a leak from pipe under hand sink near walk-in cooler; ceiling vents and walk-in cooler fans are dusty.

Joen, 7801 West Broad Street – The following violations were reported during a routine inspection: observed an employee eating in the food prep area wearing a glove on one hand; open beverage container found next to single-service item; hand sink is blocked by aprons and washcloths; knives stored on magnetic strip are dirty; there is food residue on a bowl stored clean; temperatures of kimchi, shredded cooked beef, cooked pork, sprouts, mussels and tofu are elevated; no date marks found on cooked meats or eggs; menu doesn’t contain a consumer advisory for items that contain raw eggs; temperature of hard boiled eggs is elevated because of improper cooling methods (covered); rice paddle is stored in ambient temperature standing water; bowls on shelf are stacked while still wet; single-service items are not stored protected/inverted.

Mama Cucina, 4028-O Cox Road – The following violations were reported during a routine inspection: person in charge’s food protection manager certificate is expired; cloths and utensils stored in hand sink; kitchen hand sink doesn’t have hand soap; temperature of salad mix in front prep unit is elevated; cooked sausage isn’t date marked; food items held on time control are not marked with time or identified on the policy (raw chicken, sundried tomatoes, heavy cream, garlic in oil); several pans of pasta in reach-in cooler are stacked and covered during the cooling process; cooking wines and oils are unlabeled; observed small flies at bar area; gaskets are torn on reach-in cooler doors; basting brushes are in disrepair; prep and single cutting boards are scratched and stained brown/black; observed food debris and grease buildup on wire storage racks, prep shelves, blue dishwash racks, bulk spice containers, and inside reach-in coolers and prep units; there are leaks at hand sink and prep sink faucets; walls are damaged under two-compartment sink and behind lowboy cooler at bar; light bulbs are not shielded above reach-in coolers.
