Akoya Kitchen, 10921-F West Broad Street – The following violations were reported during a routine inspection: corn starch container isn’t labeled; observed wrap still on stove shelf; shelves on the line are dusty.

Chicken Fiesta, 8211 Brook Road – The following violations were reported during a routine inspection: black residue buildup found inside ice machine; temperatures of tomatoes (top portion) and shredded chicken in prep units are elevated; several pans of beans and tamales in walk-in cooler are not date marked; there is a significant gap in the bottom of the back door that leads outside; frozen yucca fries are stored unprotected in chest freezer; to-go boxes are stored unprotected on dirty shelves above prep units; prep unit cutting boards are heavily scratched and stained black; metal wires are bent and breaking on fryer baskets; wire storage racks in dry storage and walk-in cooler are rusty; dirty cardboard is lining shelves in walk-in cooler and is attached to clean rack in dishwash area; observed food debris and grease on wire storage racks, under grill and on fan grates in walk-in cooler; there is dust buildup on ceiling vents and panels; observed food debris and grime on floor under cook line and inside coolers.

Habanero Mexican Grill, 9101-A Quioccasin Road – The following violations were reported during a routine inspection: food manager certification is expired; person in charge couldn’t provide written procedures; raw eggs stored over large salsa containers in refrigerator; tongs with food debris stored with clean utensils in pantry; temperature of steak in warmer is too low; temperatures of raw ground beef in lowboy unit and salsas in make table are elevated; several prepared foods in refrigerator are not date marked; beef is thawing at room temperature; door on lowboy under make table has a torn gasket; paint is peeling off wire racks throughout kitchen; ceiling vent in prep area is rusty; trim is peeling off the wall near bathroom hand sink; there is chipping paint on red wall and pantry; mop isn’t hung to air dry.

Tavern at Triangle Park, 7110-F Patterson Avenue – The following violations were reported during a routine inspection: food manager certification is expired; raw tilapia is stored over raw cut vegetables in lowboy; inside of ice machine is dirty; temperature of rice in warmer is too low; temperatures of shredded cheese, sliced meats, diced tomatoes and sliced tomatoes in make tables are elevated; menu is missing asterisk next to steak linking it to the consumer advisory about undercooked foods; fly trap is hung from the ceiling directly above food stored on counter; ice cream scoop is stored in ambient temperature standing water; single-service containers are stored uninverted at bar counter; several cutting boards are heavily scratched and stained black; wire racks throughout kitchen are rusty; toaster built into counter is broken; ice machine is leaking; several items/areas are dirty (floors, ceiling vents, walk-in cooler fan, ceiling panels throughout kitchen, and wall behind dish machine); trim outside walk-in cooler is peeling from wall; cup lid holder is held together with duct tape; walk-in cooler door is unable to fully close; wall behind dish machine has a hole and chipping paint; floors throughout kitchen are in disrepair; mop isn’t hung to air dry.

Marco’s Pizza, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: two employees with beards are not wearing beard guards; single-service containers are not stored inverted; cutting boards and butcher block are scratched and stained; reach-in cooler has a torn gasket; walk-in fan vents and wire racks are dusty; mop isn’t hung to air dry.

Me’Kong Restaurant, 6004 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food manager; raw chicken stored over beef in reach-in cooler; raw fish stored over produce in three-door reach-in cooler; mold found in upper area of ice machine in kitchen; chlorine level at chemical dish machine in bar area is at 0 ppm; a container of white powder on hot line is unlabeled; food is stored directly on both walk-in cooler and walk-in freezer floors; in-use utensils are stored in ambient temperature standing water; walk-in cooler racks are rusty; deep fryer baskets are in disrepair; several cutting boards are heavily scratched; hood filters are greasy; reach-in cooler door gaskets throughout kitchen are dirty; back reach-in cooler fan guards are moldy; green dumpster lid is broken.

Boychik’s Deli, 4024-B Cox Road – The following violations were reported during a routine inspection: hand sink in kitchen is blocked by trash and trash debris; there is 0 ppm sanitizer in dishwasher; salads in take-out containers and opened deli meats in front display container aren’t date marked; Windex bottles are stored in a cupboard with food items and to-go containers; an employee with a long beard isn’t wearing a beard guard; single-service containers are stored uninverted on the line; cutting boards are scratched and stained; equipment has rusty bottom shelves; several areas/items are dirty (wire racks and fan vents in walk-in refrigerator, beside and under line equipment, on top of oven hood, under display case, walk-in cooler floor, wall behind dish machine and kitchen ceiling); concrete floor in front of walk-in refrigerator isn’t sealed; light in walk-in refrigerator is too dim.

McDonald’s, 8210 Brook Road – The following violations were reported during a routine inspection: temperatures of shredded lettuce, sliced tomatoes and gravy inside reach-in cooler are elevated; ice bucket stored uninverted next to ice machine; single-service containers for pancakes stored uninverted on prep line counter; mop sink faucet has a leak.

Waffle House, 5102 Williamsburg Road – The following violations were reported during a routine inspection: dried egg residue found on mixer next to grill; edges of prep cutting boards are jagged; wire storage racks in cooler and dry storage are rusted; observed food debris and dust on wire storage racks in cooler and dry storage, utensil holders, rolling carts, and above three-compartment sink.

Pizza Hut, 5100 Williamsburg Road – The following violations were reported during a routine inspection: employee restrooms doesn’t have handwash signage posted; observed dried food debris on top of dish machine and grease residue on the exterior; food debris found inside reach-in coolers; wire racks throughout kitchen are greasy; walk-in cooler exterior, freezer doors, floor under wire racks in dish room, deep fryers and under three-compartment sink are greasy and dirty; walk-in cooler wire racks are moldy; observed grease buildup on sides of cooking equipment and reach-in coolers; food transport carts throughout kitchen and wall at mop sink are dirty; walls behind and underneath three-compartment sink and dish machine are greasy and moldy; wall behind pizza oven is greasy; ceiling vents and ceiling around pizza oven are dusty.

Quickway Japanese Hibachi, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: an employee failed to wash hands after handling a cell phone; employee didn’t use soap when washing hands; person in charge couldn’t provide parasite destruction documentation from salmon provider; heads of cabbage are stored directly on dirty wire racks in walk-in cooler; temperatures of beef, shrimp and veggies on the grill are too low; tuna is thawing in reduced oxygen packaging in the walk-in cooler; raw broccoli was not thoroughly washed to remove contaminants before being cut, cooked and served; to-go containers are stored on dirty shelves; used cardboard is lining shelves and racks in walk-in cooler; observed food debris and grease buildup on all wire storage racks and under grill.

West Coast Provisions, 301-A Maltby Boulevard – The following violations were reported during a follow-up inspection: food debris found on slicer blade, blade guard and fry cutter; time is not being tracked for blanched fries, herb butter and breading; tuna is thawing in reduced oxygen packaging; observed heavy grease and food debris on floor and wall behind equipment at cook line and under the ice machine.

MAY 21

The Lilly Pad, 9680 Osborne Turnpike – The following violations were reported during a routine inspection: cutting board is scratched and stained; observed grease buildup on sides of hot line cooking equipment; keg cooler rubber door gaskets are dirty; trash can in women’s restroom isn’t covered.

Chuy’s, 11229 West Broad Street – The following violations were reported during a routine inspection: open drink cup found above food prep area; observed pink and black residue buildup inside both ice machines; employees are wearing jewelry on arms and hands while preparing food; to-go containers stored on dirty wire racks; prep unit cutting boards are heavily scratched and stained brown; metal wires are bending and breaking on fryer baskets; some lids and containers are cracked; observed dust and food debris buildup on portable fans, wire storage racks, wheels of fryers and inside blender lid; there is a leak under the hand sink at prep area; observed low grout areas on floors throughout the kitchen.
