The Brass Tap, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: staff are eating French fries on hot line in kitchen; hand sink is blocked by trash cans; raw eggs stored over brownies; raw bacon stored over produce; raw beef stored over cooked chicken wings; black debris found inside soda machine gun at expo station; dirty knives stored as clean on knife rack; dried food debris found inside microwave; observed several fruit flies near prep sink in kitchen; cutting boards used throughout kitchen are scratched and stained; there is grease buildup on sides of hot line cooking equipment; wire racks near expo station and ceiling fans near ventilation are dusty; top of dish machine and wall behind three-compartment sink are dirty; floor drains have mold and food debris inside them.

European Taste, 10604 Patterson Avenue – The following violations were reported during a routine inspection: sanitizer concentration in dish machine is at 0 ppm; black residue found inside ice machine; plates stored near trash can are not inverted; soiled cardboard boxes are being reused to store food and other items; gaskets on reach-in cooler doors are torn and held on by tape; observed food debris and grease on surfaces of stove, wire storage racks, spice containers, fryer baskets, under grill and inside reach-in cooler; ceiling panels are not smooth and easily cleanable.

Cracker Barrel, 3620 Mayland Court – The following violations were reported during a routine inspection: dish plate in dish machine didn’t reach the required temperature; temperature of canister of whipped cream is elevated; plastic containers are stacked while still wet in dry storage area; wire racks in dish room are rusty; sides of refrigerated units and syrup stand at the serve line are dirty; cracked light shield and cracked floor tiles found in dish room; there are several cracked ceiling panel dividers throughout the kitchen; observed grime buildup on floor behind grill; there are splatters on ceilings throughout the kitchen; observed severe mold buildup on wall next to dish machine.

PITSA, 11321-B Nuckols Road – The following violations were reported during a follow-up inspection: a wok-like pan last used yesterday is filled with oil and food debris; observed food debris inside fryer baskets; cream cheese is past its expiration date; mop sink doesn’t have a dual check valve installed; floor drain is soiled; observed small flies near floor drain.

MAY 13

Hibachi House, 3456 Lauderdale Drive – The following violations were reported during a routine inspection: observed an open water cup and a water bottle without a secured lid; observed an employee crack eggs for cooking and fail to wash hands and change gloves before continuing to cook; temperatures of two pans of rice are elevated because of improper cooling methods (pans are in reach-in cooler, stacked, with lids on them); eggs in a carton found sitting out on counter; pans of cooked rice are not date marked; can of fly spray found by three-compartment sink and dishwasher; small flies found among discarded food debris at the bottom of an unbagged trash can; employee with a beard isn’t wearing a beard guard; cutting boards are scratched; shelving is dirty; hose in mop area found with pressurized spray nozzle; floors throughout kitchen and under stove have deep cracks and holes; light bulbs are out in the mop area.

Panera Bread, 9960 Brook Road – The following violations were reported during a routine inspection: oil spray, several knives and a metal container found inside hand sink near baker’s area; ice chute at soda station in lobby is dirty; ladles hanging at utensil rack are dirty with buildup; temperature of cream cheese in reach-in cooler at front counter is elevated; several plastic containers are stacked while still wet; observed a large crack on black cart near ice machine; soup cooker unit door is unable to close; racks inside reach-in cooler at front counter are rusty; there is limescale buildup on exterior of dish machine; facility couldn’t provide sanitizer test strips for sink or sanibuckets; several items/areas are dirty (monitors on prep line, wire racks throughout kitchen, food scale in back room, exterior of low boy units, legs of racks holding bread slicers at front counter, walls in back room area and walk-in cooler ceiling; mop isn’t hung to air dry; observed low grouting throughout kitchen.

Dunkin Donuts, 10600 Patterson Avenue – No violations reported during a routine inspection.

Crafty Crab, 4995 Nine Mile Road – The following violations were reported during a routine inspection: person in charge couldn’t provide the employee health policy; hand sink on hot line is being used to store food; hand sink in dish area has bucket of dirty silverware stored in it; various hand sinks in kitchen do not have paper towels because the towel dispensers are broken; raw seafood stored over sheetcake/cornbread in Pepsi reach-in cooler by hot line; raw eggs stored over seafood in reach-in cooler; dried food debris found inside microwave; dish machine sanitizer level is too low; there is no time stamp or time control policy used on breading on cook line; menu doesn’t contain a consumer advisory warning for raw and/or undercooked foods; facility doesn’t have a food thermometer; menu lists catfish but facility is serving swai; observed several fruit flies at bar area; food is stored directly on walk-in freezer floor; rice spoon is stored in a container of ambient temperature standing water; cutting boards used throughout the kitchen are scratched and stained; wire racks in walk-in cooler are rusty; observed grease buildup on sides of hot line cooking equipment; there are black stains on beer cooler door gasket; bar reach-in cooler has dirty gaskets; walk-in cooler fan guards are moldy; trash cans in women’s restroom are uncovered; observed dried food debris and food stains on stainless steel walls.

Aahar Lounge by Lazeez, 2601-B Tuckernuck Drive – The following violations were reported during a routine inspection: food debris found on knife stored clean; food boxes are stored directly on walk-in freezer floor; one cutting board at make table is scratched and stained yellow; three-compartment sink faucet has a leak; ceiling vents are dusty; floor tile outside staff restroom is cracked.

Mexico Restaurant, 5213 Williamsburg Road – The following violations were reported during a routine inspection: observed several employees fail to wash hands after handling raw meat, handling dirty cups and plates, and after cleaning; an employee handled tortilla chips with bare hands; dried food debris found on slicer blade and other surfaces; temperatures of cooked veggies and pico de gallo in prep unit are elevated; refried beans, salsas and cooked meats in the walk-in cooler are not date marked; improper cooling methods observed for rolled tacos and sliced tomatoes (put in prep unit during cooling) and salsas (covered); single-service containers are not stored in their original packaging in shed; single cutting boards are heavily scratched and stained; prep unit is not holding foods at the proper temperature; faucet at dish area hand sink is in disrepair; all wire storage racks are rusty; gaskets are torn on reach-in cooler; observed grease and food debris buildup under grill and on wire racks in walk-in cooler and dish area; there are broken floor tiles and low grout throughout the kitchen; back door is rusted and in disrepair; door frame to hot water heaters is rusted and damaged; there is food debris and grime on floors and walls; ceiling vents are dusty.

Mi Guerrero Grill, 61 South Laburnum Avenue – The following violations were reported during a routine inspection: employees handled raw animal foods and switched tasks without washing hands; kitchen hand sink doesn’t have hand soap or paper towels; foods being hot-held in the steam table are not at the proper temperatures; raw shell eggs are stored on a shelf at room temperature; two spray bottles in kitchen are unlabeled; beef intestines and ground beef are thawing at room temperature; two cases of food stored directly on kitchen floor; multiple cutting boards are heavily scratched and stained; rice is stored in non-food safe plastic bags; there are torn door gaskets on coolers throughout the kitchen; one deep fryer basket is in disrepair; facility doesn’t have sanitizer test strips; women’s restroom doesn’t have a covered trash can; there are incorrect ceiling tiles used in the food prep area; there are stained, damaged and missing ceiling tiles throughout the establishment; hot line back wall is greasy; there is a hole in the wall behind prep sink.

Zaxby’s, 4798 South Laburnum Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide the employee health policy; observed dried food debris and mold on and inside soda machine/ice/water dispensers in front of house; several cutting boards are heavily scratched; several items/areas are dirty (fry line reach-in cooler interior, salad cold well, make line cooler doors and dish drying racks); dried food debris found on reach-in freezer door and door gaskets; dried food stains found on back of reach-in cooler and on doors; central kitchen hand sink has a leak; wall behind breading station is dirty and dusted with flour; ceiling tile and ceiling ventilation are dusty.

Gogi Bibimbap, 8902-A West Broad Street – The following violations were reported during a routine inspection: kimchi, chicken, beef and several other food items are not date marked; cutting boards throughout the kitchen are scratched; observed heavy grease buildup on fry baskets and sides of hot line cooking equipment; wire racks in Nesquik cooler and walk-in cooler are moldy; reach-in cooler door gaskets are dirty and moldy; ice machine not in use has heavy grease buildup; mold and mildew found on wall behind three-compartment sink; ceiling tile near vents is dusty; there are some food stains on wall near make table; floor under deep fryers is greasy.

Vietnam 1, 6215-B West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t name the “big six” reportable foodborne illnesses and the associated symptoms or verify employee knowledge; employee drink found uncovered; observed an employee touch nose and fail to change gloves after; hand sink and employee bathroom are missing paper towels; raw eggs stored over cooked noodles and raw jalapenos in walk-in cooler; can opener blade is dirty; menu lists crabmeat but “seafood combination” is being served; vegetables in bags are stored directly on walk-in cooler floor; cellphone found on top of clean pan next to dish machine; clean containers are stacked while still wet; faucet at three-compartment sink has a leak; screen door next to mop sink is dusty; light shield above fryer is cracked; ceiling vents in kitchen are rusty.
