West Coast Provisions, 301 Maltby Boulevard – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; an employee’s open canned beverage found in prep area; observed an employee tasting food with gloved hand and failed to remove gloves and wash hands after; employee failed to remove gloves and wash hands after handling dirty dishes and cell phone prior to putting away clean dishes; employee handled herbs with bare hands; cook line hand sink doesn’t have paper towels; dried food debris found on fry cutter; no sanitizer detected at three-compartment sink; observed scummy food residue on slicer blade and food residue on blade guard; temperatures of sliced cheese, corn, lettuce and donut batter at make table are elevated; make table isn’t maintaining foods at the proper temperature; raw shell eggs at the bar are not refrigerated; time isn’t being tracked for blanched fries, butter and breading on a time control policy; drink menu doesn’t contain a reminder statement for drink that contains raw eggs; spray bottle of cleaner and stainless steel cleaner stored next to food; fish in lowboy is thawing in reduced oxygen packaging; some containers of food are unlabeled; observed large flies throughout kitchen and dining room; fly observed in container of crab in top of make table; metal strainer is damaged; sides of equipment at cook line are greasy; there is food debris in freezer on line; racks in walk-in are rusty and soiled; mixer splash guard is soiled; mop sink doesn’t have a dual check valve installed; wet mops are not being hung to air dry; multiple lights are out in the hood; observed grease buildup on floor at cook line; wall in dish area is moldy; hood vents have dripping grease and buildup.

Subway, 9621 West Broad Street – The following violations were reported during a routine inspection: observed dried food debris on slicer stored above three-compartment sink; plastic container tops are stored dirty; temperature of meatballs on the line is too low; shakers on the line are unlabeled; plastic hanging in the walk-in cooler is broken; walk-in cooler gasket is torn and unable to seal; coffee pot filter is cracked; wire rack in walk-in freezer is rusty; observed buildup on wire racks throughout kitchen; observed food debris buildup on shelf storing metal containers and microwave; floor tiles throughout kitchen are cracked; there is chipped paint and dents on walls in backroom; there is a large hole in staff door; mop isn’t hung to air dry; several areas are dirty (ceiling vents throughout kitchen, walls at three-compartment sink and dry storage area, floors throughout kitchen, and walk-in cooler ceiling and vent).

Awful Arthur’s Seafood Company, 10498 Ridgefield Parkway – The following violations were reported during a routine inspection: observed dried food debris on can opener blade; fish in prep unit and walk-in cooler is thawing in reduced oxygen packaging; single-use items and plates are stored on dusty shelves; large cutting board is heavily scratched and stained; broken lids, bowls and containers stored on racks; metal is bending on fryer baskets; observed buildup of grease, food debris and dust on surfaces of fryers, grill, oven, wire storage racks, shelves, beer lines, inside prep units, and inside coolers and glass shelves at bar; observed buildup of food debris and grime on floors throughout kitchen and walk-in cooler; floors in walk-in cooler and at bar are no longer sealed, smooth and easily cleanable.

Ciao Capri, 10478 Ridgefield Parkway – The following violations were reported during a routine inspection: food manager certification is expired; container of tomatoes in prep unit is moldy; pink residue found inside ice machine; containers of rice and pasta more than 24 hours old are not date marked; a wiping cloth is covering cooked eggplant in prep unit; plates and single-service items are stored on a dusty surface; observed heavy dust, food and grease buildup on surfaces of fryers, grill, wire storage racks, prep counters, and inside prep unit; observed food debris buildup on floors, under and behind cook line.

MAY 1

La Rancherita, 3423 Cox Road – The following violations were reported during a routine inspection: hand sink behind counter doesn’t have hand soap; several containers of raw chicken are stored next to and above raw vegetables; temperatures of house-made salsa and milk are elevated; one chemical spray bottle is unlabeled; several bins of white powders in dry storage area are unlabeled; ice cream scoop found in ambient temperature standing water; spoons and forks are not stored inverted on top of ice cream counter; two cutting boards are heavily scratched; observed heavy buildup on hood vents; exterior vents are dusty; floor tiles are cracked and grouting is low near three-compartment sink.

Acacia Midtown, 2363 Roux Street – The following violations were reported during a routine inspection: dried food debris found inside lettuce spinner; tuna is thawing in reduced oxygen packaging in walk-in cooler; cutting boards throughout kitchen are scratched and stained; food debris found in fry station gaskets and interior; hood filters are greasy and sides of hot line cooking equipment have some grease buildup; unused panini press stored in kitchen is dusty and dirty; observed grease and food debris on floor underneath and behind hot line cooking equipment.

Sheetz, 8711 Quioccasin Road – The following violation was reported during a routine inspection: temperature of chicken tenders is too low.

Fresh Wok Restaurant, 5146 Nine Mile Road – The following violations were reported during a routine inspection: two employee beverages found uncovered on hot line; cup of coffee placed in hand sink; soap dispenser in men’s restroom is in disrepair and is out of hand soap; raw shrimp stored over cooked chicken and cooked shrimp; bottle of soy sauce stored among the cleaning chemicals; tilapia is thawing in reduced oxygen packaging; single-service items are being reused as scoops throughout the kitchen; sushi to-go containers are not stored inverted; multiple fry baskets are in disrepair; observed grease and dried food debris buildup on all bulk container bins, soap and paper towel dispensers, reach-in cooler door gaskets, hand sinks, shelving beside cook line, and sides of cooking equipment; walk-in cooler fan guards are dusty; floor tile on hot line is cracked; floor plate in walk-in cooler is cracked/broken.

Redemption BBQ and Market, 3420 Lauderdale Drive – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures in place for employee health; temperature of bacon in portioned food boats is elevated; temperature of two containers of collards in reach-in prep unit is elevated because of improper cooling methods (covered containers); employee with beard isn’t wearing a beard guard; lights above prep table do not have shields.

Panera Bread, 1370 North Parham Road – The following violations were reported during a routine inspection: temperature of shredded cheese is elevated; clean silverware near dish machine are not stored inverted; ceiling vents and walls on line and fans in walk-in cooler are dusty; floor behind appliances on line are dirty.

MAY 3

Subway, 4386 South Laburnum Avenue – The following violations were reported during a routine inspection: stainless steel containers found inside hand sink; temperatures of produce/TCS items on the line are elevated; reach-in fridge under ovens has a broken gasket; sanitizer container connected to three-compartment sink is empty; stainless steel containers found on a dirty tray below slicer prep area; mop isn’t hung to air dry; light in the walk-in cooler isn’t bright enough and it is being blocked by packages.

MAY 9

IHOP Restaurant, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: sanitizer in both dish machine and three-compartment sink tested at 0 ppm; temperatures of pancake batters in reach-in cooler and on cutting board at grill are elevated; temperature of milk in dessert reach-in cooler is elevated; temperatures of liquid egg and ham steak in unit next to grill are elevated; silverware on counter isn’t stored inverted; wire racks in dessert reach-in cooler are rusty; long white cutting boards are heavily scratched and stained black; interior of fryer cabinets and sides of fryer and grills are dirty; ceiling vents are rusty; light in the walk-in cooler is flickering.

Charles’ Kitchen, 10835 West Broad Street – The following violations were reported during a routine inspection: all food manager certifications have expired; pink slime found on metal rim of ice machine; several items/areas are dirty (inside of microwave, wire racks in kitchen, shelf above units, fan blowing next to three-compartment sink, and bar area reach-in cooler); vents in kitchen are dusty; light fixture in back of kitchen is missing its shield.
