Vampire Penguin, 11345 Nuckols Road – The following violations were reported during a routine inspection: facility doesn’t have an employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea.

Ay Mama, 9809 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t name symptoms for the “big six” foodborne illnesses; facility doesn’t have written procedures for cleaning up vomit/diarrhea; cup found in bar hand sink; observed buildup in soda gun at bar; sanitizer tested at 0 ppm in dish machine; temperatures of salsa and sliced tomatoes on the line are elevated; temperatures of raw chicken and sliced tomatoes in reach-in cooler are elevated; sliced tomatoes and salsas in walk-in cooler aren’t date marked; person in charge states that steak can be served undercooked but the menu doesn’t contain a consumer advisory regarding this; silverware is stored uninverted; observed grease buildup on hood vents and inside fryer cabinet; low grouting found near fryer.

Great Steak / Surf City Squeeze, 11800 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; hand sink in back of facility doesn’t have hand soap; hand sink in front of facility is filled with containers; eggs stored above french fries in walk-in cooler; raw steak stored in same prep unit with uncovered ready-to-eat products; blenders are being cleaned with soap and water but are not being sanitized; no date marks found on hot dogs, baked potatoes, sauces and an unidentifiable white liquid; three spray bottles are unlabeled; containers with dry spices are unlabeled; seasoned salt is in an open container without a lid; cardboard found on lower shelf under prep table; sanitizer test strips are expired; toilet and sink are dirty; employee bathroom doesn’t have toilet paper; ceiling vents throughout facility are dusty.

Sparrow’s Pizza, 6313 Rigsby Road – The following violations were reported during a routine inspection: several cans of black olives are dented; ceiling tiles are not smooth and easily cleanable.

APRIL 24

Flying Dragon, 5314 Chamberlayne Road – The following violations were reported during a routine inspection: dried food debris found inside microwave; crisps for wonton soup and produce are stored in raw chicken cardboard boxes; egg cartons are being reused throughout the kitchen; cutting boards are scratched and stained; cardboard boxes are being reused to line shelves and soak up grease; several items are rusty (walk-in cooler wire racks, legs of three-compartment sink, exterior of chest-style deep freezers, and bottom shelves of prep tables); reach-in cooler door gaskets on hot line are dirty; there is mold on wire racks in walk-in cooler; observed debris/grease/buildup on racks and shelves throughout, hood filters, under fryers, in between and under equipment on cook line, and on floors and walls throughout facility; floor tiles throughout kitchen are cracked/damaged; ceiling tiles throughout restaurant are stained.

Jadean’s Smokin Six O, 1126 Westbriar Drive – The following violations were reported during a routine inspection: food manager certification expired last month; filth found inside ice machine; dishes and cups stored on sticky/dirty shelf; some containers are broken; several cutting boards have deep cuts; fryer baskets are dirty; mop isn’t being hung to air dry; some light bulbs are not working; vents in kitchen are dirty.

Greek Cuisine, 11214 Patterson Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; temperatures of a pan of chicken/potatoes and a pan of moussaka prepared yesterday are elevated because of improper cooling methods; prepared foods are tightly covered during the cooling process (moussaka and baked chicken); single and prep unit cutting boards are heavily scratched and stained; lowboy cooler door gaskets are torn; facility doesn’t have test strips for quat sanitizer used at three-compartment sink; observed food debris and dust on wire racks near three-compartment sink.

88 Street Food, 5905 West Broad Street – The following violations were reported during a routine inspection: observed an employee eating at the line; restroom doesn’t have paper towels; ice scoop is stored on a dusty surface on top the ice machine; trash can in large bathroom is uncovered.

APRIL 25

America’s Best Wings, 3075-C Mechanicsville Turnpike – The following violations were reported during a routine inspection: hand sink is blocked by stacks of beverages; raw chicken stored next to produce in walk-in cooler; temperature of par cooked chicken wings held in buckets next to fryer is too low (facility doesn’t have a written policy on par cooking chicken); multiple bulk containers of white powders are unlabeled; items are stored directly on walk-in cooler and freezer floors; bottom shelves of prep tables in kitchen are rusted; prep sink in kitchen is no longer sealed to the wall; cutting boards throughout kitchen are scratched and stained; cardboard is being reused to line prep table shelves; facility couldn’t provide sanitizer test strips; cooler and freezer door gaskets throughout the kitchen are dirty; wire racks throughout the kitchen are dusty, greasy and dirty; ceiling vents and walk-in cooler fan guards are dusty; observed mold on wall behind kitchen prep sink; floor coving under three-compartment sink and near mop sink is missing.

9 Mile Grill, 309 West Nine Mile Road – The following violations were reported during a routine inspection: raw bacon stored over produce and raw beef stored over ready-to-eat food in walk-in cooler; the low temp dish machine sanitizer concentration is at 0 ppm; dried food debris found on can opener blade; observed mold inside soda machine nozzles; observed dried food debris inside all microwaves and on reach-in cooler and freezer doors; crock pot used for warming gravy needs cleaning; facility doesn’t have a food thermometer; food is stored directly on walk-in cooler and freezer floors; heavy grease residue is dripping on sides of hot line equipment (deep fryers, stove, flat top grill, etc.); several items are dirty (walk-in cooler door gaskets, lowboy reach-in cooler door gaskets and interior, reach-in cooler interior); walk-in cooler wire racks are moldy; fan guards in walk-in cooler and freezer are dusty; observed food debris, stains, grease and grime on floors and walls throughout kitchen and under/behind hot line equipment.

APRIL 26

Lokmans Best Wings, 5308 Chamberlayne Road – The following violations were reported during a routine inspection: temperature of cooked noodles is elevated because of improper cooling methods (stored in deep container covered with lid); menu lists catfish but the facility is serving Pangasius; plastic knives and spoons are stored uninverted; cutting boards throughout kitchen are stained and scratched; cardboard is being reused to line shelves; reach-in freezer interior and exterior are dirty; door gaskets on reach-in cooler and freezer are dirty; there is grease buildup on sides of hot line cooking equipment; dried food debris found on base of Ninja blender; outdoor dumpster has no lids and the doors were left open; some ceiling tiles in dining room are damaged; freezer floor is in need of repair; facility doesn’t have a no-smoking sign posted in the non-smoking area.

Papa John’s Pizza, 3067 Lauderdale Drive – The following violations were reported during a routine inspection: dish hand sink has buckets stored in it and it’s missing paper towels; observed food debris buildup on racks throughout; exterior and sides of equipment need cleaning.

Zara Grill, 10865 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a written policy for employee sickness; facility doesn’t have written procedures for cleaning up vomit/diarrhea; hand sink is blocked by a trash can; eggs are stored over ready-to-eat foods in reach-in fridge; temperature of meat wrapped in cling wrap in front refrigerator is elevated; facility doesn’t have a food thermometer; several bins of dry ingredients are unlabeled; single-service containers are stored uninverted on a dirty shelf; cutting board has significant scoring; light panel in back of facility doesn’t have a shield.

APRIL 29

El Paraiso Restaurante, 4034 Crockett Street – The following violations were reported during a routine inspection: soups and other food items are not being date marked when they are removed from the freezer to thaw; to-go boxes are stored uninverted on a dirty shelf; cardboard is being reused to line shelves in reach-in freezers; metal is bent and broken on fryer baskets; wire racks in reach-in coolers are rusted; observed grease buildup on sides of grill, fryers and oven.

Starbucks Coffee Company, 9700 Gayton Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; observed buildup inside coffee machine nozzles; temperatures of whipped cream in three reach-in coolers on the line are elevated; mop isn’t being hung to air dry; counter and floors under/behind appliances on the line are dirty.
