APRIL 15

New Dragon City, 2220 John Rolfe Parkway – The following violations were reported during a routine inspection: bulk flour bins and spices and sauce bottles on cart are unlabeled; used egg cartons are being used to drain utensils on; prep and single cutting boards are heavily scratched and stained; metal wires are bending and breaking on fryer baskets; observed dust and food debris on wire racks in walk-in cooler and dry storage; lighting in walk-in cooler is inadequate.

Potbelly Sandwich Shop, 10921-E West Broad Street – The following violations were reported during a routine inspection: hand sink in back room doesn’t have hot water; observed buildup in soda nozzles in lobby, especially in tea dispensers; temperature of meatballs in uncovered warmer on line is too low; three large bins filled with sugar, oil and salt in back room are unlabeled; cutting boards on the line are heavily scratched and stained yellow; observed grime buildup on wire racks next to oven; food debris found inside reach-in units on the line; ceiling light in dry storage room is flickering; there are holes in ceiling panels in back room; food splatter found on wall behind warmers; ceiling vent in dry storage room is dirty; floor under appliances and ceiling panels and fans on the line are dirty; observed cobwebs above door in dry storage room.

Baker’s Crust Artisan Kitchen, 11800 West Broad Street – The following violations were reported during a routine inspection: person in charge’s food manager certification is expired and no other employees are certified; person in charge couldn’t provide verification that employees have been trained on illness reporting requirements; hand sink on the line doesn’t have hand soap; hand sinks on the line and at the bar don’t have paper towels; facility couldn’t provide parasite destruction records from salmon provider; bowl of raw sausage stored over uncovered pans of fries in reach-in refrigerator; raw shrimp stored next to asparagus and raw shell eggs stored above sauces in make tables; unwrapped cheese slices stored next to an unwrapped raw burger patty in small fridge on cook line; observed buildup inside soda gun nozzle at bar; dried food debris found on fry cutter; no sanitizer is detected at bar dish machine; there are elevated food temperatures in several units (mashed potatoes, bechamel, blanched fries, pizza sauce, broccoli, cooked onions, mozzarella, tomatoes and cheese); time isn’t being tracked for blanched fries and fried onion straws; improper temperatures observed for cooked pasta, pico de gallo, gravy and alfredo sauce because of improper cooling methods; bulk containers of flour and sugar are unlabeled; mouse droppings found in back storage room and dry storage area; roach droppings found in corner of the wall in dish area; side door leading to parking lot/dumpster area was propped open; a couple beverage bottles are touching the ice that the bar is using for drinks; wet wiping cloths were left out on the make table instead of being stored in a sanitizer bucket between uses; person in charge states that fruits and vegetables are not being washed prior to cutting (salad spinner broke and has not been replaced); scoop and crepe tool are stored in ambient temperature standing water; knives are being stored in sanitizer; single-service containers are stored unwrapped and uninverted in the kitchen; small individual cutting boards are heavily scratched; some plastic food storage containers are cracked/broken; metal is bent and broken on fryer baskets; gasket is torn on bar lowboy refrigerator; there is space in between hood vents; racks in dish area are rusty; multiple refrigeration units (pizza, omelet and fry units) are running high; observed scum buildup inside dish machine; facility couldn’t provide sanitizer test strips; walk-in fan guards are dusty; there is ice buildup in reach-in freezer; there is standing liquid in bottom of bar unit with kegs; sticky residue found inside small refrigerator at bar; water is leaking at spray hose faucet at pre-wash sink; a dual check valve needs to be installed at mop sink faucet; drain line at soda gun is missing (soda gun holder is directly above ice used for customers’ drinks); ceiling vents have heavy dust; observed buildup of food debris on support beam located behind trash can at cook line; observed greasy residue buildup on shelf above waffle iron; coving tiles behind cook line are missing/broken; floor grout is eroded in some areas; wall corners are in poor condition; observed grease buildup on hood vents and dripping grease on ledge under vents.

Subway, 11234 Patterson Avenue – The following violations were reported during a routine inspection: sliced meats and tomatoes are stacked and covered during the cooling process; prep cutting boards are heavily scratched and stained; observed food debris on wire racks; ceiling panels are not smooth and easily cleanable.

APRIL 16

Ninja Hibachi, 10162 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction records from salmon provider; observed black slime residue inside ice machine; temperatures of raw beef and raw salmon in prep unit are elevated (person in charge states that this unit lost power during the previous night’s storm); cutting boards are heavily scratched and stained; observed buildup of grease and food debris on surfaces of grill, fryers, wire storage racks, storage shelves, squeeze bottles, and spice and sauce containers; observed food debris and grime on floors and walls; there is grease buildup on wood around pass-thru window on grill side.

APRIL 17

Chick-fil-A, 4920 West Broad Street – The following violations were reported during a routine inspection: pink mold found inside ice machine in back of house; cutting board at salad make table is scratched; outdoor dumpster lids and doors were left open; dumpster lid is in need of repair; floor under deep fryers is greasy; ventilation hoods are greasy, particularly above chargrill machine.

Buz and Ned’s Real Barbecue, 8205 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; facility doesn’t have written procedures for cleaning up vomit/diarrhea; open drink cup found in back food prep area; raw eggs stored over open corn in walk-in cooler; observed buildup of dirt inside ice machine; date marking is inconsistent (ribs have April and May dates and other foods have future and past dates); dry ingredients bin is unlabeled; paper towel is used on bar racks; storage bin in walk-in freezer is cracked; several items/areas are dirty (can rack, exterior of grease filter machine, large pots found on top of Henny Penny, foiled-lined drip pans, and bottom of beer box); there is ice buildup in two-door fridge; dual check valve needs to be installed on faucet with red hose; bar three-compartment sink is dripping; employee restroom door was left open; coving tile is broken; wall structure by fire extinguisher is in disrepair; there is a hole in the wall in big kettle room; ceiling panels are missing in storage area and by the back door; mop room wall is in poor repair; there is low/no grout throughout kitchen; ceiling vent in storage area and fly fan by rear door are dusty.

Tropical Smoothie Café, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: can opener blade is dirty; observed food debris inside microwave and dust/dried food debris on exterior; door gasket on smoothie cooler is torn; walk-in cooler and freezer door kick plates are in disrepair; outdoor dumpster doesn’t have a door; large and small cutting boards throughout the kitchen are heavily scratched and stained; front of house ceiling fans, wall behind ice machine and ceiling vents/tiles throughout kitchen are dusty.

Wingstop, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: black mold found on ice dispenser of soda machine; cutting board is scratched; lowboy cooler door gaskets are dirty; fan guards in walk-in cooler and hot line freezer are dusty; mop sink doesn’t have a dual check valve installed; cardboard and garbage receptacles in outdoor refuse area are not closed/covered; observed grease and food debris underneath deep fryers on hot line; floor and drain under bag in box have food debris; ceiling vents are dusty; dried food debris found on wall behind sauce station.

Con Salsa Venezuelan Cuisine LLC, 3016-A Mountain Road – The following violation was reported during a routine inspection: in-house juice bottles don’t have the proper warning label (unpasteurized).

APRIL 22

Subway, 3135 Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed cheese and other dried food debris inside microwave; two dead mice found in glue traps as well as mouse droppings found on make line floor; an employee with long flowing hair isn’t wearing a hair restraint in the kitchen; reach-in cooler underneath meat slicer isn’t holding at the proper temperature; walk-in freezer fan guards and ceiling ventilation above oven are dusty; observed food stains and mildew on wall behind three-compartment sink; ceiling tile in back of house is stained.

First Watch, 5310 West Broad Street – The following violations were reported during a routine inspection: whisk found in hand sink; observed rust stains and a small amount of mold inside ice machine; tub of blue cheese isn’t date marked; carrots in walk-in cooler are past their expiration date; scoop at grill is stored in ambient temperature standing water; several cutting boards are scratched and stained; dish machine isn’t dispensing sanitizer (test strip reads 0 ppm sanitizer); observed dried food debris on top of dish machine; dried food debris found on cooler door gaskets along hot line; walk-in freezer door is dirty; walk-in cooler fan guards are dusty; side door to the dumpster was left open; floors in both walk-in cooler and walk-in freezer are dirty.
