Glen Allen Bistro, 10209 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t produce written procedures for cleaning up vomit/diarrhea; hand sink near oven doesn’t have hand soap; dried food debris found on slicer; temperatures of sliced tomatoes and cheese in the make table are elevated; ceiling panels throughout the kitchen need repair; one ceiling panel is no longer smooth and easily cleanable; observed buildup on hood vents; observed grime buildup on floor around fryer, inside fryer cabinet, and on wall under three-compartment sink; rack above make table and ceiling panels through facility, especially in dish area, are dirty; unused equipment needs to be removed from the facility.

Crimson Lights Bar & Lounge, 5114 Richmond Henrico Turnpike – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; dried food debris found inside microwave; rubber door gasket on bar area Pepsi cooler is torn; cutting boards throughout kitchen are scratched; observed grease buildup on sides of cooking equipment; reach-in freezer interior and unused reach-in cooler interior are dirty; there is a small leak in piping under kitchen hand sink; there are leaks in both faucets at three-compartment sink; observed grease buildup on floor at hot line; ceiling tile near ventilation is dusty; there are food stains on walls of dish room; hood filters are greasy and were last cleaned professionally in May 2023.

Mellow Mushroom, 12171 West Broad Street – The following violations were reported during a routine inspection: certified food protection manager certificate is expired; a server used bare hands to put lemon in a drink; no sanitizer detected at bar dish machine; items in several refrigerator units are elevated (chicken, ham, diced and sliced tomatoes in make table and potatoes and cheese in pizza unit); time is not being tracked for pizza and person in charge couldn’t produce the written time control policy; Hot Shot bug spray found at bar; observed many small flies throughout the establishment; employee with long facial hair is preparing food without a beard guard on.

Dave & Buster’s, 4001 Brownstone Boulevard – The following violations were reported during a routine inspection: hand sink at bar doesn’t have paper towels; soda gun is sitting in ice that is used for beverages; to-go containers are stored unprotected on a dirty shelf; wire racks in reach-in coolers are rusty; several fryer baskets are in disrepair (metal wire is broken and bending); single cutting boards are heavily scratched and stained; observed water draining onto the floor under dish machine (pipes are not aligned over floor drain); cement floor under prep units and bread rack isn’t sealed.

Olive Garden Italian Restaurant, 4467 South Laburnum Avenue – The following violations were reported during a routine inspection: line cook failed to wash hands after adjusting hair restraint; observed carbonara sauce on the hot line at an improper temperature; clean equipment and/or utensils are not stored in a self-draining position that allows air drying; cutting boards are scratched and stained; monitors throughout the kitchen, ceiling tile near ventilation, and produce cooler fan guard are dusty; observed grease buildup on sides of hot line cooking equipment; salad cooler door gaskets are dirty; there is a leak underneath hot line hand sink; dumpster door was left open; observed grease and food debris on floor behind hot line.

Wendy’s, 2200 East Laburnum Avenue – The following violations were reported during a routine inspection: dried food debris found inside microwave; observed dried food stains and soda syrup on both soda dispensers; observed grease buildup on sides of cooking equipment; several items are dirty (cooler under prep table, raw hamburger cooler door gaskets, and gaskets on cooler below fry station); walk-in cooler wire racks are dirty and moldy; wire rack under warmers in dish area are greasy and dirty; floor under fry station is greasy; there are food stains on walls throughout the kitchen; ceiling tiles throughout kitchen and dining area are stained; there is a missing/damaged ceiling tile in dining area and a damaged ceiling tile in kitchen; dust, food stains and grease are dripping from ventilation hoods; there is dust on outside and inside of hoods.

Einstein Brothers Bagels, 3320 Pump Road – The following violations were reported during a routine inspection: temperature of country pepper cream cheese in prep unit is elevated; there is heavy ice buildup inside walk-in freezer on ceiling, floor and around door; prep cutting boards are heavily scratched and stained; observed dried food debris on surfaces of wire storage racks above three-compartment sink, wire storage racks inside walk-in coolers, rolling cart, bulk ingredient bins and the walk-in fan system; ceiling panels are heavily stained.

Pho Saigon, 10190 West Broad Street – The following violations were reported during a routine inspection: open drink found on microwave above prep table; temperatures of noodles in warmer and spring rolls in fryer basket are too low; temperature of top portion of pork in prep unit is elevated; metal wires are broken on fryer baskets; single cutting boards are heavily scratched and stained; observed dried food debris and grease residue on wire storage racks, blenders and prep shelves; old tape labels found on clean containers; observed food debris buildup on floors throughout the kitchen and in walk-in cooler.

Brick Road Coffee Co., 8055 Shrader Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; observed a food employee touching eyeglasses with gloved hands; facility isn’t maintaining parasite destruction records for raw smoked salmon that is served in ready-to-eat form; observed buildup of pink and black residue inside ice machine; menu doesn’t contain a consumer advisory for the smoked salmon that may be served raw and/or undercooked.

Crustacean Boil N’ Grill, 8902-C West Broad Street – The following violations were reported during a routine inspection: temperatures of rice in warmer and potatoes and sausage in steam table are too low; observed several small flies at bar under hand sink; wooden pallets are being used in the walk-in cooler and freezer to store food; metal wires are bending on fryer baskets; observed mildew and food debris buildup on wire racks in walk-in cooler; observed grease and food debris on fryers, on floor under/behind cook line, and under flat top grill; walk-in freezer floor is heavily rusted.

Bellacino’s Pizza and Grinders, 8026-D West Broad Street – The following violations were reported during a routine inspection: dried food debris found on blade of can opener; observed employees working in the food service area without proper hair restraints for beards; all prep cutting boards are heavily scratched and stained; broken and cracked lid and containers are being used; baking sheet pans are heavily pitted; observed food and dust buildup on surfaces of wire storage racks, heat lamps, and inside reach-in cooler; observed mildew and food buildup on wire racks and fan system in walk-in cooler; there is a leak on faucet at hand sink near three-compartment sink; several ceiling panels are heavily stained; floor tiles in walk-in cooler and near dish machine are broken.

Waffle House, 5414 Williamsburg Road – The following violations were reported during a routine inspection: the dish machine isn’t reaching the required surface temperature; temperatures of hashbrowns, ham and tomatoes in prep top are elevated; facility doesn’t have a way to test the dish machine on site; observed dirt and grease on racks, gaskets and doors of units, under grill and in between cooking equipment; under the line, the dish machine and the walk-in floor are dirty; ceiling tiles and wall tiles are dusty.

Chipotle Mexican Grill, 7000 Forest Avenue – The following violation was reported during a routine inspection: clear plastic lids are not stored in a protected manner.
