First Watch Restaurant, 11721 West Broad Street – The following violations were reported during a routine inspection: temperatures of pancake batter and waffle batter in walk-in cooler are elevated and they have exceeded the four-hour cooling time; temperatures of pancake batter in make table and container of pancake batter in reach-in cooler are elevated; dump sink in wait station has a leaky faucet; mop sink doesn’t have a dual check valve installed; dumpster found with lid open.

Elephant Thai Restaurant, 11676 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written policy for cleanup of diarrhea/vomit; observed several employees touching clothing and face with gloved hands and not replacing gloves; employee is cutting lettuce with bare hands; hand sink next to three-compartment sink doesn’t have hand soap; temperature of two containers of cooked rice prepared yesterday is elevated (has exceeded the six-hour cooling window); cooked rice is cooling in deep plastic containers; large containers in pantry containing a white substance are unlabeled; a sticky fly trap is placed above a rack holding food and single-service containers; single-service and to-go containers found on shelf with dust buildup; plastic single-use containers are being used as scoops in dry storage food containers; cardboard is lining dry storage shelf; fryer baskets have bent wires; main cutting board and smaller cutting boards along prep unit are scratched and stained; plastic storage containers found with old labels partially dissolved away; observed grease and/or food debris buildup on sides of wok station, floors of reach-in cooler and freezers, and backs of fryers; portable fan has heavy dust buildup; observed grease, dust and food buildup on floors throughout kitchen and ceiling tiles; two light fixtures above dry storage area don’t have shields; hood vents have grease and food buildup.

Fogo de Chão, 11221 West Broad Street – The following violations were reported during a routine inspection: hot water isn’t reaching the required temperature at hand sink near prep/dessert area, hand sink near walk-in fridge and in women’s bathroom; main dish machine and small dish machine at bar aren’t reaching the proper hot water sanitizing temp; temperatures of rice, black beans and pork chops are too low; facility’s written time control policy is unclear and needs more clarification/details; multiple chemical spray bottles are unlabeled; ice cubes used for bar drinks are stored unprotected/uncovered and in a container directly on floor in walk-in; facility doesn’t have a device to check hot water sanitizing at dish machine.

Peter Chang, 11408 West Broad Street – The following violations were reported during a routine inspection: employees couldn’t answer questions concerning employee health nor whether they were trained; observed a worker wash hands in three-compartment sink; hand sink in back is blocked; raw chicken and beef stored over bok choy and sauces in walk-in cooler; dishwasher isn’t being run as the data plate states (data plate indicates a high temp machine, however there appeared to be sanitizer in a marked rinse additive container); can opener and ice machine interior are dirty; debris found on slicer; temperatures of pork dumplings, cut cabbage, cooked noodles, cooked chicken and spring rolls are elevated; no date marks found on any food including open tofu, cooked duck, noodles and chicken; grill cleaner is unlabeled; other chemicals are unlabeled/mislabeled; facility doesn’t have a food thermometer; corn starch, flour and other baking needs are unlabeled; dirty cloths found everywhere; food is stored directly on freezer floor; bowl is being used to scoop noodles; skewers are stored in a tin can; single-use items (corn starch mix cans, plastic buckets for pepper, garlic containers) are being reused; shelf in baking area is bare wood; linen is being used under glassware, cups and tea pots; towels are being placed over food; shelving in freezer is not six inches off the floor; dish racks are being used as shelves; facility doesn’t have a maximum registering thermometer to monitor dish machine; observed buildup inside dish machine; several items/areas are dirty (spray hose attachment at dish area, exterior of pots, shelf under large work table, rice warmers, dry rice bin, can opener holder, hood filters, filter cabinet, cook line shelf, mixer splash, lower shelf by ice bin, wall and floor drain near ice machine, bar floor and floor drain, and walk-in freezer floor and door); unused equipment on premises should be removed; wood block under freezer needs sealing; foil is lining wall; it appears that food is being fried in a saucepan on counter burner but no ventilation is provided.

The Penalty Box, 4350 Pouncey Tract Road – The following violations were reported during a routine inspection: person in charge unable to verify that workers have been trained in employee health; observed slime inside ice machine; dish machine has zero chlorine residual; facility doesn’t have QT-10 test kit for single quat sanitizer; observed lime buildup in steam table; drip tray, fryer cabinet, top of dish machine and oil caddy are dirty; hot water faucet is not operating properly; observed buildup on floor under equipment on cook line, in three-compartment sink area, and around shelving; there is no ventilation above panini grill; there is insufficient light in the walk-in.

Dunkin Donuts, 7300 Staples Mill Road – The following violation was reported during a routine inspection: observed dried milk and food debris on walk-in cooler racks, top of oven and storage shelves.

Bella Luna, 1212 Concord Avenue – The following violations were reported during a routine inspection: certified food protection manager certificate is expired; no date marks found on pasta, chicken wings and grilled chicken; freezer door next to stovetop is broken; there is heavy ice buildup in both freezers; observed food debris buildup on floors and walls in dry storage and around three-compartment sink.

Sumo Sushi Express, 1090 Virginia Center Parkway – The following violation was reported during a routine inspection: small cutting boards on dish shelf and above three-compartment sink are heavily scratched and stained brown.

Amanecer Latino Market, 9301 Quioccasin Road – The following violations were reported during a routine inspection: facility couldn’t provide verification that employees have been trained on health issues; facility doesn’t have written procedures for cleaning up vomit/diarrhea; meat area hand sink doesn’t have a handwash sign posted; raw meat and chicken stored over cooked lengua; there is insufficient sanitizer in the three-compartment sink; temperature of tamales made yesterday is elevated because of improper cooling methods; temperatures of ham and salsa in the make table are elevated because the door was not closed; no date marks found on sliced ham, lengua and slaw.

Arby’s, 4250 Pouncey Tract Road – The following violations were reported during a routine inspection: date mark is incorrect on gyro meat in walk-in cooler; pans of stew prepped today are unlabeled; two wire racks in walk-in used for thawing are rusty; storage racks in walk-in cooler and freezer are dirty.

Chum Chum Onigri, 9073 Staples Mill Road – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; temperature of two containers of rice on prep table are elevated; temperature of container of kimchi stored on prep counter is elevated; no date marks found on two jars of pickled veggies made in the establishment and a container of kimchi; chicken curry, katsu and other foods have the incorrect expiration date written on them; two jars of pickled carrots and radish in under-counter cooler are unlabeled; squeeze bottles of liquid are unlabeled.

Lalo’s Pinches Tacos Bar & Grill, 8026 West Broad Street – The following violations were reported during a routine inspection: brown residue buildup found inside soda gun; sanitizer concentration is at 0 ppm in dish machine (container was empty); temperature of salsa in cups in prep unit is elevated; large container of salsa and refried beans are not date marked; menu doesn’t have a consumer advisory statement for rib eye steak that may be served raw and/or undercooked; several cleaning spray bottles are unlabeled; prep cutting boards and red single boards are heavily scratched and stained brown.

McDonald’s, 10000 Brook Road – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels; temperature of creamer in small standing unit is elevated; employees are wearing bracelets while prepping food; multiple units have torn gaskets; observed heavy ice buildup in fry unit.

Frank’s West, 11238 Patterson Avenue – The following violations were reported during a routine inspection: temperatures of scallops, lasagna and tomatoes in walk-in are elevated; knives stored between prep tables; handle of ice scoop is in contact with the ice.

Chez Max Restaurant, 10622 Patterson Avenue – The following violations were reported during a routine inspection: open drink cups found near prep and on a cart with clean towels and equipment; hand sink in dish wash area doesn’t have hand soap or paper towels; brown residue is building up inside soda gun at bar; no date marks found on containers of French onion soup or tomato-based sauce; observed dead insect bodies on bar floor; employees working in the food service area are not wearing proper hair restraints for beards; prep cutting boards are heavily scratched and stained; observed dust and food debris on surfaces of veggie bins, air curtains and fan system in walk-in cooler, fryer, and inside cooler at bar; light fixtures in dry storage and dish wash area don’t have shields.
